Three years later in January 1941, Saul Hertz administered <span class="elsevierStyleSup">131</span>I for the first time to a patient with hyperthyroidism in the Massachusetts General Hospital. Later, in May 1946, the same Hertz and Arthur Roberts published the results obtained in 29 patients with hyperthyroidism treated with <span class="elsevierStyleSup">131</span>I, and around three months later, Seidlin et al. demonstrated good response in a patient with differentiated metastatic thyroid cancer following three treatments with radioiodine<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>. An effective, accessible and inexpensive treatment for thyroid disease had been born, and it can be said that this is when Nuclear Medicine was born, along with its diagnostic and therapeutic applications, and thus, the concept of theragnosis, with an infinity of patients with thyroid disease having been diagnosed and treated with <span class="elsevierStyleSup">131</span>I since then.</p><p id="par0010" class="elsevierStylePara elsevierViewall">However, this dilated history is not exempt from large shadows since along almost 60 years and in view of the good results obtained, the procedures have undergone practically no modifications. The scientific literature produced was generally retrospective, not multicentric or randomized and with results that were not sufficiently representative, leading to new proposals, and overall controversy, regarding the use of radioiodine. It is true that this was more manifest for differentiated thyroid cancer than for hyperthyroidism, having had a largely uneventful history. In 2021, the HiLo study demonstrated the possibility of successfully administering low ablative doses of <span class="elsevierStyleSup">131</span>I, mainly with the intention of avoiding an in-patient treatment regimen<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. Seven years later, the good results with low doses during the follow-up of patients with differentiated thyroid cancer were confirmed as well as a low rate of disease recurrence, while awaiting the results of currently ongoing prospective, randomized multicenter clinical trials in the more or less near future.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In 2016, the American Thyroid Association published guidelines for the management of thyroid nodules and differentiated thyroid cancer (ATA 2015), with recommendations causing great debate in the community of Nuclear Medicine. In different publications, the European nuclear medicine physicians explained the reasons why the EANM did not support these guidelines, highlighting that they marginalize the role of Nuclear Medicine since different recommendations are based on equivocal interpretation of the evidence available to date or are even not based on sufficient evidence<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>. Consequently, representatives of both the European and American scientific societies involved met in January 2018, and the fruit of this meeting was the Martinica Principles which attempt to respond, not always satisfactorily, to the controversy generated. These same societies established a working group with the aim of annually identifying subjects which may have a significant impact on clinical management, publishing articles of opinion reflecting the collaboration, consensus, and the suggestions of the experts. This group has recently published a consensus document which establishes that scanning with <span class="elsevierStyleSup">131</span>I is an indispensable component of dynamic stratification and theragnosis of risk, allowing individualized therapeutic management of patients with differentiated thyroid cancer<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>. This could be interpreted as the first step for redirecting the role of <span class="elsevierStyleSup">131</span>I in the diagnosis and treatment of thyroid disease. We hope that our old, young hearted, radioiodine again takes its corresponding place, endorsed by solid scientific evidence so that its theragnostic indications are undebatable. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
