Glycemic response during hypoglycemia by the glucagon test was evidenced. In the etiological study, hyperinsulinemia (292?µUI/mL) and elevated C-peptide (12.2?mg/mL) with a blood glucose level of 31?mg/dl were obtained. Due to the clinical suspicion of congenital hyperinsulinism (CHI), abdominal ultrasound and abdominal MRI were performed with negative results. Genetic testing confirmed paternal hereditary mutation in ABCC8.</p><p id="par0010" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">18</span>F-DOPA PET/CT was performed 28 days after birth in order to rule out CHI and to distinguish focal from diffuse form of CHI. Correct preparation is a key factor for the accurate interpretation; this includes blood glucose monitoring and administration, if necessary, of continuous glucose perfusion (ensure glycemia of 60–75?mg/dl). A 4-h fasting is mandatory, but perhaps the most difficult preparation item in the guideline is the suspension of medication that may interfere with the study, such as glucagon (12?h before). Carbidopa premedication was not performed in this case because it is not recommended in the evaluation of insulinoma or ß-cell hyperplasia in children with CHI. The reason is that carbidopa can reduce and even block not only physiological but also pathological pancreatic uptake.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Contrast enhanced PET/CT (Biograph 6 True Point; Siemens) acquisition was undertaken 50?min after slow intravenous injection of <span class="elsevierStyleSup">18</span>F-DOPA. PET/CT study was performed using pediatric protocol (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Maximum Intensity Projection (MIP) image, physiological uptake was observed in striated bodies, urinary system, liver, gallbladder, biliary tract and duodenum (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). Focal accumulation of <span class="elsevierStyleSup">18</span>F-DOPA was evidenced in the pancreatic tail (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B), with no correlation in the enhanced CT. A semiquantitative analysis of the lesion showed a maximum Standardized Uptake Value (SUVmax) value of 6.5, while mean Standardized Uptake Value (SUVmean) of the pancreatic parenchyma was 3.2. The ratio SUVmax lesion/SUVmean of the parenchyma was 1.65, therefore suggestive of focal form of CHI.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Another mild uptake located in the pancreatic head with SUVmax 3.3 was observed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>C), with ratio SUVmax lesion/SUVmean parenchyma 0.84, suggestive of physiological uptake (pancreatic head may have greater physiological activity than the rest of the gland).<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Abdominal pre-surgical angio-CT was performed, where no anatomical variants were identified in the visceral branches of the abdominal aorta, and no different enhancement areas in pancreatic tail were observed.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In our patient, the information supplied from <span class="elsevierStyleSup">18</span>F-DOPA PET/CT study had a diagnostic and therapeutic impact. Due to the identification of a focal form of CHI with PET/CT, the precise preoperative planning enables a laparoscopic surgery approach, and although laparoscopic distal pancreatectomy is usually performed for benign conditions, there are few reported cases of CHI with this type of surgical approach. A laparoscopic spleen-preserving distal pancreatectomy was performed (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A y B). We used four trocars, and the operative time was 90?min. Conversion to open surgery was not necessary. This case shows that even in advanced surgical cases, such as spleen-preserving distal pancreatectomy, laparoscopic pancreatectomy can be performed safely, even in a newborn patient, without prolonged operative time or unnecessary risk.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The diagnosis was confirmed by histopathological analysis of the resected piece. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
