These lesions are hyperintense on T2.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Paragangliomas are neuroendocrine tumours of neuroectodermal origin that overexpress somatostatin receptors. They can be sporadic or familial and are located in sympathetic and parasympathetic head and neck, intrathoracic or intra-abdominal-pelvic ganglia. Those of the head and neck are usually benign and non-secretory due to their parasympathetic origin and in some cases are associated with susceptibility mutations; the case we present is associated with the succinate dehydrogenase type B (SDHB) gene, which makes it necessary to rule out multiple paragangliomas.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">We present three members of a family carrying the SDHB mutation whose familial genetic study was performed thanks to the imaging findings of the index case. The other two cases were asymptomatic.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Index case: 37-year-old male with no significant personal or family history, presenting with bradycardia and paroxysmal flutter, with negative cardiological study and metanephrines and left laterocervical tumour with MRI compatible with carotid paraganglioma, who came to rule out multifocal involvement. Due to the availability of the service and the greater usefulness for assessing soft tissues, a PET/MR with 68Ga-DOTA-TOC was performed, which showed marked overexpression of somatostatin receptors in the carotid lesion and small disseminated foci, with paragangliomas in the proximal location to the carotid lesion, right dorsal paravertebral region and right inguinal region (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">Considering a possible hereditary origin, a genetic study was performed which showed an SDHB mutation (change in C.337_340 of the GACT) and family screening was indicated (paternal transmission).</p><p id="par0025" class="elsevierStylePara elsevierViewall">With the positive genetic test in the father (case #2, 63 years old) and the patient's paternal uncle (case #3, 53 years old), it was decided to complete the study with PET/MRI with 68Ga-DOTA-TOC in both asymptomatic relatives. In case 2, foci of somatostatin receptor overexpression were identified in the left jugular vein and in the bilateral carotid bifurcation (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>). In case 3, there is evidence of an intense focus in the left internal carotid intrapetral tract, small foci inferoposterior to both thyroid lobes and in both inguinal regions (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><p id="par0030" class="elsevierStylePara elsevierViewall">Previously it was believed that only 10% of paragangliomas were familial, although some series report up to 25%, but advances in genomic sequencing techniques have made it possible to describe hereditary paraganglioma syndromes associated with, among others, mutations of various SDH subunits and this percentage is increasing.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">It is important to identify familial presentations because they may be associated with multiple endocrine neoplasia type II, neurofibromatosis type 1, or Von-Hippel-Lindau syndrome. In addition, some of these tumours may be aggressive due to their catecholamine-secreting and/or metastatic capacity. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
