During the work-up, she was found to have hypercalcemia [serum calcium: 14.9<span class="elsevierStyleHsp" style=""></span>mg/dL (normal 8–10<span class="elsevierStyleHsp" style=""></span>mg/mL), ionic calcium: 1.51 mmol/L (normal 2.2–2.6 mmol/L)], hypercalciuria [urinary-calcium excretion: 364.0<span class="elsevierStyleHsp" style=""></span>mg/24<span class="elsevierStyleHsp" style=""></span>h (normal 50–150<span class="elsevierStyleHsp" style=""></span>mg/24<span class="elsevierStyleHsp" style=""></span>h)], and chronic kidney disease stage 3 [creatinine: 1.6<span class="elsevierStyleHsp" style=""></span>mg/dL (normal 0.6–1.1<span class="elsevierStyleHsp" style=""></span>mg/dL)]. Other paraclinical studies included: PTH: 9.3<span class="elsevierStyleHsp" style=""></span>pg/mL (normal 10–53<span class="elsevierStyleHsp" style=""></span>pg/mL), prolactin: 123.9<span class="elsevierStyleHsp" style=""></span>ng/mL (normal 2–29<span class="elsevierStyleHsp" style=""></span>ng/mL), negative hCG blood test, normal head, neck, chest, abdominal and pelvic CT results, normal gadolinium-enhanced MRI of the sella turcica, and normal bone-marrow biopsy. In the setting of non-PTH-mediated hypercalcemia, <span class="elsevierStyleSup">18</span>F-FDG PET/CT was requested to discard malignancy; the study revealed hypermetabolism of the parotid and submandibular glands, myocardium, and muscles of the appendicular and axial skeleton (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>); a biopsy was taken from the left gluteus maximus, the hottest spot identified in the <span class="elsevierStyleSup">18</span>F-FDG PET/CT, which depicted non-caseating granulomas, with multinucleated giant cells and lymphocytic infiltrates, consistent with sarcoidosis. After oral prednisone 10<span class="elsevierStyleHsp" style=""></span>mg daily was started, there was improvement of symptoms, calcium levels (ionic-calcium: 1.21<span class="elsevierStyleHsp" style=""></span>mmol/L), and kidney function (creatinine: 1.12<span class="elsevierStyleHsp" style=""></span>mg/dL), which confirmed the diagnosis. Follow-up <span class="elsevierStyleSup">18</span>F-FDG PET/CT demonstrated significant decrease of hypermetabolism of the affected sites (<a class="elsevierStyleCrossRefs" href="#fig0010">Figs. 2 and 3</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Sarcoidosis is a multisystem disease characterized by noncaseating granulomatous inflammation that most commonly affects lungs (90%). Muscle involvement is common, identified in up to 50–80% of random muscle biopsies of patients with sarcoidosis, but usually asymptomatic.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> According to autopsy studies cardiac compromise can be seen in up to 25% of patients, usually affecting the myocardium. It can be asymptomatic, as was the case of this patient, or severe and fatal if not treated.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> Hypercalcemia is manifested in 10–20% of patients, and is secondary to the overexpression of 1 α-hydroxylase enzyme in the macrophages present in the granuloma, which leads to an overproduction of 1,25[OH]2D3 (calcitriol), subsequently increasing intestinal calcium absorption, osteoclasts activity, and tubular calcium resorption.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The diagnosis of sarcoidosis is based on clinical, radiological and histopathological findings. Nevertheless, given its unknown etiology, it is an exclusion diagnosis. Clinical course is variable, usually determined through the assessment of the inflammatory activity, which also happens to guide the therapeutic approach.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> Routine diagnostic procedures, such as chest X-ray and CT, are useful for identifying pulmonary sarcoidosis but not so much assessing active inflammation. On the other hand, <span class="elsevierStyleSup">18</span>F-FDG PET/CT allows visualizing the inflammatory activity, due to the accumulation of FDG in inflammatory cells.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> In this case it was not only useful in guiding the most appropriate biopsy site, but also in monitoring the course of the disease and treatment response.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The assessment of the inflammatory activity, not only in sarcoidosis but in different entities, may imply a challenge for the clinician, especially when faced with unspecific symptoms. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
