The 18F-FDG PET/CT surveillance showed, a year after surgery, an appearing focal uptake lateral to the right ischium suspicious for recurrent metastatic melanoma (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). Contemporary MR imaging showed a lesion abutting or arising from the right sciatic nerve with features presumably representing a peripheral nerve sheath tumor rather than metastatic melanoma. The patient was kept under observation. Subsequent PET/CT monitoring showed a progressive growth of the tracer-avid lesion at 6 months (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B) and a year (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>C&D) after the initial exam with matched enlargement on MR imaging (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A&B). The lesion progression prompted a CT-guide biopsy with a fine needle aspiration diagnosis of metastatic melanoma. Based on this pathologic impression, the patient underwent surgery with the comprehensive histologic verdict changed to the definitive diagnosis of tenosynovial giant cell tumor (TGCT, <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>C).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Giant cell tumors may arise from bone, soft tissue, synovium or tendon sheath. The ones involving the synovium and tendon sheath are characterized by (1) the localized form namely nodular tenosynovitis or TGCT and (2) the diffuse features also known as pigmented villonodular synovitis (PVNS) first reported in 1941 by Jaffe et al.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">1,2</span></a> Initially thought to be a reactive synovial process, this lesion is presently considered neoplastic based on its chromosomal abnormalities, autonomous growth and potential recurrence with incomplete resection. TGCT is mostly encountered in a patient population of third and fourth decades of life with slight female gender predilection. TGCT appears as a nodular lesion by tendon sheath or synovial lining of a joint or bursa of knee, ankle, wrist, finger and foot with 18F-FDG avidity on PET/CT.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> MR imaging features are usually suggestive of TGCT at the expected tenosynovial sites mentioned above (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A&B). In our case, the atypical peri-sciatic location, away from any tenosynovial structure, leads to a MR impression of a benign neurogenic tumor. The concerning continuous growth of the tracer-avid lesion on serial PET/CT exams and the fine needle aspiration samples containing atypical pigmented cell with brown hemosiderin lead to the erroneous initial impression of metastatic melanoma.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a> Surgical resection with comprehensive histologic evaluation of the entire specimen reached the correct final diagnosis of TGCT.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Nguyen B. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
