Computed tomography (CT) scan of the chest showed a giant mass in the posterior mediastinum. Subsequently, the patient underwent biopsy, and histopathology of the mass revealed the diagnosis of poorly differentiating ganglioneuroblastoma (GNB) with a high mitotic karyorrhectic index which is unfavorable. The patient was referred to 68Ga tetraazacyclododecanetetraacetic acid-DPhe1-Tyr3-octreotate (<span class="elsevierStyleSup">68</span>Ga DOTATATE) positron emission tomography (PET)/CT for initial staging. MIP image showed intense 68Ga DOTATATE uptake from the thoracic outlet to the renal hilus level. Coronal and sagittal fused PET/CT images revealed tracer avid conglomerate mass almost filling the entire posterior mediastinum and extending to the retroperitoneum. Transaxial CT, PET and fused PET/CT images showed that the masses surround the major vessels and the heart in the mediastinum. 68Ga DOTATATE PET/CT also depicted multiple metastatic lymph nodes and bone lesions. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> Neuroblastoma, GNB, and ganglioneuroma are three subtypes of neuroblastic tumors arising from the sympathetic nervous system. These tumors consist of different proportions of Schwann cells and neuroblasts. Therefore, neuroblastic tumors are divided into three subgroups according to their degree of cellular maturation and differentiation. GNB is a mixed tumor containing both primitive neuroblasts and mature ganglion cells. Most of the neuroblastic tumors occur in the first five years of life and adult-onset is extremely rare. Among the neuroblastic tumors, especially GNB is less common in adults. The most common sites of origin are the adrenal medulla, retroperitoneum, and posterior mediastinum respectively. Radical surgery is the best effective treatment method for the mediastinal GNB. Chemotherapy is the treatment of choice for inoperable tumors. Cisplatin, etoposide, vincristine, cyclophosphamide, and doxorubicin are the most commonly used chemotherapy agents. Adult-onset GNB exhibits a poorer prognosis than pediatric GNB patients and tumors larger than eight cm in diameter prone to metastasize to other distant organs. Therefore accurate staging before the treatment decision is crucial<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. 123I-metaiodobenzylguanidine scintigraphy and 18F-fluorodeoxyglucose (18F FDG) PET/CT were both proved to be useful in the staging and monitoring of neuroblastic tumors. Due to the extreme rarity of adult GNB, only a few cases describing 18F FDG PET/CT findings were reported in the literature<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a>. However, a case of adult-onset GNB on 68Ga DOTATATE has not been reported yet. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
