A <span class="elsevierStyleSup">18</span>F-choline PET/CT scan was performed (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). It showed uptake in hilar-mediastinal lymph nodes, more evident in right hilum. In addition, a subcentimeter nodule in the anterior segment of right upper lobe (RUL) without significant <span class="elsevierStyleSup">18</span>F-choline uptake was identified in CT. In view of these findings, a <span class="elsevierStyleSup">18</span>F-DCFPyL PET/CT was performed to assess prostate-specific membrane antigen (PSMA) expression (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). It showed more foci of high uptake intensity in enlarged hilar-mediastinal lymph nodes (right hilum, SUVmax, 40). The known subcentimeter nodule at RUL also showed high uptake (SUVmax, 18.5).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">As thoracic findings were suspicious of metastatic disease, with prognostic and management implications, histopathological evaluation was advised. An endoscopic ultrasound-guided fine-needle aspiration biopsy (EUS-FNAB) of right hilar lymph node, assisted by sonographic elastography, was conducted, being positive for malignant tumor cellularity suggestive of adenocarcinoma of prostatic origin.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Mediastinal lymph node metastases are rare in PCa.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Even with a high tumor burden. For the last decades PET/CT with choline-based tracers has been the traditional imaging modality of choice in BCR following patients.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> It is common to find hilar-mediastinal lymph with uptake being attributed to inflammatory processes. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
