C: Axial CT image, PET/CT fusion and PET of the mass in the left posterior gastrocnemius territory.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Arterial vascular complications secondary to infective endocarditis are common<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> although pseudoaneurysms of the popliteal vein are rare entities characterized by presenting as a pulsatile mass in the popliteal fossa, frequently related to previous surgical interventions in the knee. The differential diagnosis of popliteal aneurysm includes Baker's cyst and fluid sac in the popliteal fossa.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In this context we find a 34-year-old male patient with HIV who reports asthenia, 10 kg weight loss, low-grade fever, and night sweating for two months. The chest x-ray detected cardiomegaly with growth of left cavities, so an echocardiogram was performed, which detected a severely unstructured aortic valve with prolapse of the posterior right coronary leaflet, with vegetations. He is diagnosed with subacute bacterial endocarditis due to <span class="elsevierStyleItalic">Streptococcus mitis</span>. He underwent surgery and a mechanical aortic prosthesis was implanted with a favorable outcome.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Forty-eight hours after surgery, he is presented with intense acute pain in the left gastrocnemius region. Given the suspicion of thrombophlebitis, a Doppler ultrasound is performed, which detects the absence of flow in the anterior tibial vein and a vascular lesion in communication with the posterior tibial artery, as well as a cystic image in the popliteal fossa suggestive of Baker's cyst, which is why it is ruled out deep venous thrombosis.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The persistence of the symptoms associated with signs of inflammation in the left lower limb raises the suspicion of a septic origin of the aneurysm, which is why PET/CT is requested to rule out a possible septic origin. During the examination, the CT angiography study (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A) shows a right posterior tibial mass with contrast caudal to the mass. The PET/CT fusion image with [18F]FDG shows a hypermetabolic halo that surrounds the lesion of the posterior side of the left gastrocnemius (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B and C) suspicious of infectious behavior. In the rest of the examination, especially in the cardiac area, no other pathological findings were detected.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">The vascular lesion is repaired and parts of the thrombus and wall are sent to microbiology confirming as a pseudoaneurysm of fungal origin. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim.
