El carcinosarcoma (CS), también llamado «tumor maligno mixto mulleriano», es el subtipo más común de SU, seguido por el leiomiosarcoma y el sarcoma del estroma endometrial<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">1</span></a>. En general, los SU muestran un comportamiento clínico agresivo y mal pronóstico; de hecho, casi la mitad de las pacientes con SU recaen en los 2 años siguientes al inicio del tratamiento, incluso en los estadios iniciales<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,6</span></a>. El pronóstico se estima según la clasificación de la FIGO<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">7,8</span></a> y las características histológicas<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">9</span></a>. Además, los SU no tienen marcadores tumorales específicos y, consecuentemente, las imágenes radiológicas y metabólicas tienen un papel crucial en el manejo clínico de estas pacientes. La detección precoz de la recurrencia de la enfermedad es crítica, incluso aunque el manejo actual del SU, especialmente en el campo de la reestadificación, sigue siendo poco satisfactorio. Actualmente, la reestadificación y la estrategia de seguimiento consisten en la exploración física y los estudios de imagen convencionales, que incluyen la ecografía, la tomografía computarizada (TC) y/o la resonancia magnética (RM)<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12</span></a>. Estas técnicas pueden mostrar una sensibilidad (SE) limitada y en ocasiones pueden no detectar la recurrencia, especialmente en pacientes asintomáticas. Otra limitación es la dificultad para diferenciar entre tejido patológico, congruente con la recidiva, y el edema agudo o la fibrosis en pacientes sometidas a cirugía o radioterapia<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">13–15</span></a>. En esta área, la tomografía por emisión de positrones/tomografía computarizada con <span class="elsevierStyleSup">18</span>F-fluorodesoxiglucosa (PET/TC con <span class="elsevierStyleSup">18</span>F-FDG) ha demostrado ser útil en muchos tipos de cáncer, especialmente si la PET/TC se realiza tras un adecuado intervalo de tiempo entre tratamientos. Los estudios sobre el papel de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en pacientes con SU son limitados<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">16,17</span></a> y muestran resultados diferentes. Nuestro objetivo fue evaluar la exactitud (AC) y el impacto clínico de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en la detección de la recurrencia tumoral en pacientes previamente tratadas de SU.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material y métodos</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Pacientes</span><p id="par0010" class="elsevierStylePara elsevierViewall">Se han examinado retrospectivamente unos 35.000 estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG (realizados en unos 25.000 pacientes) mediante un sistema RIS-PACS especializado en nuestra unidad de Medicina Nuclear desde agosto de 2005 a diciembre de 2016. De este gran grupo de pacientes, 41 mujeres (edad media<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>desviación estándar: 51<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17 años; rango 27-77) tenían diagnóstico histológico de SU. Todas las pacientes se sometieron a una histerectomía por vía abdominal más salpingo-ooforectomía bilateral y en 10 casos también se realizó una linfadenectomía pelviana bilateral. La mayoría de las pacientes (25/41) no recibieron tratamiento adyuvante tras la cirugía; se administró radioterapia externa pelviana adyuvante en 4 casos y quimioterapia en 12. Los SU se clasificaron de la siguiente manera: leiomiosarcoma en 13 casos, CS en 21 casos y sarcoma del estroma endometrial en 7 casos. En el momento del diagnóstico, 22 pacientes estaban en el estadio <span class="elsevierStyleSmallCaps">i</span> de la clasificación de la FIGO, 8 en el estadio <span class="elsevierStyleSmallCaps">ii</span>, 3 en el estadio <span class="elsevierStyleSmallCaps">iii</span> y 8 en el estadio <span class="elsevierStyleSmallCaps">iv</span>. Los diferentes tipos histológicos (CS, leiomiosarcoma y sarcoma del estroma endometrial) estaban bien balanceados considerando la edad, el estadio de la FIGO y el manejo terapéutico.</p><p id="par0015" class="elsevierStylePara elsevierViewall">En estas 41 pacientes se realizaron 73 PET/TC con <span class="elsevierStyleSup">18</span>F-FDG como parte de su seguimiento. El intervalo de tiempo medio entre el tratamiento inicial (cirugía más otros eventuales tratamientos) y el estudio PET/TC fue de 12 meses (4-36) y todas las PET/TC fueron realizadas como mínimo 4 meses tras el inicio de los tratamientos. Entre ellos, 47 estudios se realizaron por sospecha de recurrencia con base en síntomas clínicos y/o hallazgos en pruebas de imagen convencionales (a 33 pacientes les realizaron un estudio PET/TC, a 7 pacientes les realizaron 2 estudios PET/TC). Veintiséis estudios se realizaron como seguimiento en pacientes asintomáticas (a 7 pacientes les realizaron un estudio PET/TC, a 4 pacientes les realizaron 2 estudios, a una paciente le realizaron 3 estudios y a 2 pacientes les realizaron 4 estudios). Las características basales de todas las pacientes incluidas se resumen en la <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>. El seguimiento medio fue de 32 meses (rango 12-120 meses).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG: adquisición e interpretación</span><p id="par0020" class="elsevierStylePara elsevierViewall">Se realizaron los estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en las pacientes con SU tras como mínimo 6<span class="elsevierStyleHsp" style=""></span>h de ayuno. El nivel de glucemia fue inferior a 150<span class="elsevierStyleHsp" style=""></span>mg/dL. Se administró una actividad de 3,5-4,5<span class="elsevierStyleHsp" style=""></span>MBq/kg de <span class="elsevierStyleSup">18</span>F-FDG por vía intravenosa y se adquirieron las imágenes a los 60<span class="elsevierStyleHsp" style=""></span>min de la inyección del radiofármaco desde la base del cráneo hasta medio muslo en un tomógrafo PET/TC Discovery ST (General Electric Company, Milwaukee, WI, EE. UU.) con los parámetros estándar (TC: 80<span class="elsevierStyleHsp" style=""></span>mA, 120<span class="elsevierStyleHsp" style=""></span>kV; PET: 2,5-4<span class="elsevierStyleHsp" style=""></span>min por posición de cama, avance de PET de 15<span class="elsevierStyleHsp" style=""></span>cm); la reconstrucción se realizó con una matriz de 128<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>128 y un campo de visión de 60<span class="elsevierStyleHsp" style=""></span>cm. En casos concretos, según indicación, se amplió el estudio más allá de medio muslo. Se indicó a las pacientes que debían orinar antes de la adquisición de la imagen, no se administraron contrastes orales ni intravenosos ni se realizó preparación intestinal en ninguna paciente; se obtuvo el consentimiento informado antes de cada estudio.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Las imágenes PET se analizaron visual y semicuantitativamente con la medida del valor estandarizado de captación máximo (SUVmáx). Todas las captaciones focales fuera de la distribución fisiológica se consideraron como indicativas de enfermedad, definidas como una discreta actividad de <span class="elsevierStyleSup">18</span>F-FDG superior a la de los tejidos adyacentes en el análisis visual. Se midió el SUV de las lesiones detectables dibujando una región de interés sobre el área de máxima actividad y se calculó el SUVmáx como el mayor valor de SUV de los píxeles dentro de la región de interés.</p><p id="par0030" class="elsevierStylePara elsevierViewall">El estándar de referencia fue la confirmación histológica o clínica y la información por imagen durante el seguimiento tras la realización de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG. Se realizó comprobación mediante cirugía o biopsia guiada por imagen siempre que fue posible. En caso contrario, los resultados de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG se confirmaron según la evolución clínica o radiológica, especialmente en pacientes con enfermedad diseminada. Las pacientes con estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG positivos se consideraron como verdaderos positivos (VP) si las exploraciones confirmaban la naturaleza maligna de las lesiones, y falsos positivos (FP) si las exploraciones adicionales no mostraron lesiones malignas. Las pacientes con estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG negativos se consideraron como verdaderos negativos (VN) si las exploraciones adicionales no mostraban lesiones neoplásicas, y como falsos negativos (FN) si mostraban lesiones malignas.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Evaluación del impacto clínico de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG</span><p id="par0035" class="elsevierStylePara elsevierViewall">El impacto clínico de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG se consideró positivo si las lesiones adicionales detectadas en ella y que no se habían demostrado en otras técnicas de imagen convencionales (ecografía, RM y/o TC con o sin contraste según la indicación clínica) modificaron el manejo o descartaron lesiones falsamente positivas de las técnicas de imagen convencionales evitando tratamientos innecesarios o apoyando una intención curativa. Por el contrario, el impacto clínico se consideró negativo si la PET/TC llevó a realizar procedimientos invasivos no necesarios tanto diagnósticos como terapéuticos. Se consideró que los resultados de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG no tenían impacto clínico si esta y las pruebas de imagen convencionales fueron congruentes, o bien si los hallazgos discordantes no afectaron al manejo posterior.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Análisis estadístico</span><p id="par0040" class="elsevierStylePara elsevierViewall">Todos los análisis estadísticos se llevaron a cabo utilizando el Statistical Package for the Social Sciences versión 23.0 para Windows (IBM, Chicago, Illinois, EE. UU.) y el MedCalc Software versión 17.1 para Windows (Ostend, Bélgica). Se consideró estadísticamente significativo un valor de p<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleItalic"><</span><span class="elsevierStyleHsp" style=""></span>0,05. El análisis descriptivo de las variables cualitativas incluyó el cálculo de las frecuencias simples y relativas. Las variables cuantitativas se describieron con la media, la desviación estándar y los valores mínimo y máximo. Se calculó mediante el teorema de Bayes la SE, la especificidad (SP), el valor predictivo positivo (VPP), el valor predictivo negativo (VPN) y la AC, con un intervalo de confianza (IC) del 95%.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Resultados</span><p id="par0045" class="elsevierStylePara elsevierViewall">Con base en los métodos de referencia mencionados, se confirmó el diagnóstico definitivo de recurrencia en 33 de 73 (45%) estudios, mientras que no hubo recurrencia en 40 estudios. Se obtuvo confirmación histológica en 21 estudios, mientras que en 12 se evaluó según la evolución clínica y/o de imagen.</p><p id="par0050" class="elsevierStylePara elsevierViewall">La PET/TC con <span class="elsevierStyleSup">18</span>F-FDG fue positiva en 30 estudios (41%) que mostraron la presencia de al menos una lesión hipermetabólica congruente con recurrencia local de SU o lesión metastásica, mientras que en los 43 estudios restantes (59%) no se hallaron áreas de incremento de captación de la <span class="elsevierStyleSup">18</span>F-FDG. De los 30 estudios positivos, 25 se habían realizado con el fin de reestadificar (por sospecha de recurrencia local o a distancia con base en hallazgos clínicos o de imagen convencional) y 5 por seguimiento en mujeres asintomáticas. Los sitios de recurrencia en la PET/TC incluyeron recurrencia local pelviana en 3 casos, recurrencia pelviana local y metástasis ganglionar retroperitoneal en 3 casos, recurrencia pelviana local y metástasis a distancia en 2 casos, metástasis ganglionares retroperitoneales en 6 casos, metástasis a distancia en 14 casos y metástasis ganglionares retroperitoneales y metástasis a distancia en 2 casos. Las metástasis a distancia incluyeron el pulmón en 9 casos, el hueso en 3, el peritoneo en 3, el tejido muscular en uno y la enfermedad diseminada en 2 casos: uno con enfermedad pulmonar y hepática (<a class="elsevierStyleCrossRef" href="#fig0005">fig. 1</a>) y otro con enfermedad ósea y pulmonar. El valor medio del SUVmáx de las lesiones de recurrencia fue 5,8 (rango 2,2-44).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Así, 29 de las 30 captaciones en la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG fueron consideradas como enfermedad a distancia o recurrente, con la excepción de un caso en el que la PET/TC mostró la presencia de una lesión ósea lítica con discreta captación de <span class="elsevierStyleSup">18</span>F-FDG en el cóndilo femoral izquierdo, no confirmada en la gammagrafía ósea con <span class="elsevierStyleSup">99m</span>Tc-metil-difosfonato ni en el posterior estudio PET/TC de seguimiento realizado 6 meses después, en ausencia de tratamiento entre los 2 estudios (<a class="elsevierStyleCrossRef" href="#fig0010">fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Según las referencias estándar, en el análisis por estudios los hallazgos de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG mostraron 29 VP, 39 VN, un FP y 4 FN, configurando una SE, SP, VPP, VPN y AC de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en la detección de la recurrencia del SU del 88% (IC 95% 71-96), 98% (IC 95% 85-100), 97% (IC 95% 81-100), 91% (IC 95% 77-97) y 93%, respectivamente.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Entre los 4 estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG que fueron FN, 2 pacientes tenían en la TC de tórax múltiples nódulos subcentimétricos en ambos pulmones sin avidez por la <span class="elsevierStyleSup">18</span>F-FDG. El seguimiento clínico y radiológico confirmó la presencia y crecimiento de estos nódulos que eran indicativos de metástasis (<a class="elsevierStyleCrossRef" href="#fig0015">fig. 3</a>). En otras 2 mujeres, la TC mostró un aumento de múltiples ganglios retroperitoneales y una pequeña lesión en la pelvis sospechosa de recidiva local, pero la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG no mostró evidencia de captación anormal de radiotrazador. Cuatro meses después del estudio PET/TC, las lesiones descritas en la TC aumentaron de tamaño y la biopsia confirmó su naturaleza recurrente.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Si se consideran solo las pacientes estudiadas con fines de reestadificación en presencia de sospecha clínica o radiológica (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>47), la SE, SP, VPP, VPN y AC de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG fueron 89% (IC 95% 70-97), 100% (IC 95% 79-100), 100% (IC 95% 83-100), 86% (IC 95% 64-96) y 94%, respectivamente (los resultados de la PET/TC se consideraron VP en 25 casos, VN en 19 y FN en 3). No se encontraron FP. Entre los 25 VP, 3 pacientes tenían recidiva local en la pelvis, 3 tenían recidiva local y metástasis ganglionares retroperitoneales, 2 tenían recidiva local y metástasis a distancia, 5 tenían solo infiltración ganglionar retroperitoneal, una tenía recurrencia en adenopatías retroperitoneales y metástasis a distancia y 11 tenían solo metástasis a distancia.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Para el seguimiento en pacientes asintomáticas (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>26) la SE, SP, VPP, VPN y AC de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG fue del 80% (IC 95% 30-99), 95% (IC 95% 74-100), 80% (IC 95% 30-99), 95% (IC 95% 74-99) y 92%, respectivamente (4 estudios PET/TC se consideraron VP, 20 VN, uno FP y otro FN). Entre los 4 VP, uno mostró enfermedad ganglionar retroperitoneal, otro adenopatías retroperitoneales más metástasis a distancia y 2 solo metástasis a distancia.</p><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Impacto clínico de la PET/TC</span><p id="par0080" class="elsevierStylePara elsevierViewall">Se documentó un impacto positivo de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en 9 de 73 estudios (12%); todos los estudios se habían realizado con fines de reestadificación en presencia de sospecha de recurrencia. En los otros estudios de reestadificación con PET/TC con <span class="elsevierStyleSup">18</span>F-FDG no hubo impacto clínico debido a la concordancia entre la PET/TC y los estudios de imagen convencional. Todos los estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG realizados durante el seguimiento en pacientes asintomáticas demostraron no tener impacto clínico. La PET/TC con <span class="elsevierStyleSup">18</span>F-FDG influyó en el manejo de 8 de 41 pacientes (20%); en 2 casos mostró la presencia de lesiones óseas ávidas de <span class="elsevierStyleSup">18</span>F-FDG no detectadas por TC, lo que llevó a aplicar tratamiento de quimioterapia, y en 6 casos mostró ausencia de captación de <span class="elsevierStyleSup">18</span>F-FDG y descartó FP de la RM/TC, evitando un tratamiento innecesario. De estas 6 pacientes, 3 tenían nódulos pulmonares dudosos que desaparecieron en controles posteriores, en un caso había un engrosamiento pleural que desapareció tras tratamiento antibiótico y en otro caso con sospecha de múltiples lesiones hepáticas se confirmó más adelante la presencia de angiomas hepáticos (<a class="elsevierStyleCrossRef" href="#fig0020">fig. 4</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discusión</span><p id="par0085" class="elsevierStylePara elsevierViewall">Los SU son un grupo heterogéneo de tumores uterinos raros y agresivos. Muestran una elevada tendencia a la recurrencia local y las metástasis a distancia y tienen mal pronóstico<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">5,6</span></a>. La recomendación actual para la detección de recurrencia de SU incluye la exploración física periódica y estudios de imagen convencionales como la ecografía, la TC o la RM. A pesar de esto, el manejo postratamiento en el SU persiste como un reto. La diversidad de histologías y rarezas de estos sarcomas ha contribuido a la falta de consenso en el tratamiento óptimo y el apropiado seguimiento postratamiento<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">10–12</span></a>. Se ha descrito en la literatura un posible papel para la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en el manejo de los SU, demostrando resultados prometedores en la caracterización<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">18–22</span></a>, la estadificación<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">23,24</span></a>, la reestadificación<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">24–30</span></a> y el pronóstico<a class="elsevierStyleCrossRefs" href="#bib0330"><span class="elsevierStyleSup">31–34</span></a>, con una buena AC global.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Teniendo en cuenta solo el seguimiento postratamiento, existen varios estudios en la literatura basados en el papel de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG con resultados discordantes; además, los artículos basados en la evidencia sobre este tema demuestran la necesidad de recabar más información sobre el papel de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en el SU, en particular sobre el diagnóstico y el cambio de manejo derivado de los hallazgos de la PET<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">16,17</span></a>.</p><p id="par0095" class="elsevierStylePara elsevierViewall">De acuerdo con los datos de la literatura, la SE y la SP acumuladas en el análisis por paciente de los estudios PET/TC con <span class="elsevierStyleSup">18</span>F-FDG en el seguimiento postratamiento del SU fueron del 92,1% (IC 95% 82,4-97,4) y el 96,2% (IC 95% 87-99,5), respectivamente (<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>). También, en nuestro estudio encontramos un rendimiento diagnóstico similar de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG, con una SE y SP del 88 y el 98%, respectivamente; el rendimiento diagnóstico de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG fue bueno tanto para el grupo de reestadificación como para el grupo de seguimiento, ligeramente superior en el primero (SE 89 vs. 80% y SP 100 vs. 95%). Park et al.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">26</span></a> analizaron 48 estudios PET o PET/TC con <span class="elsevierStyleSup">18</span>F-FDG de 36 pacientes, 18 estudios por sospecha de recurrencia y 30 como seguimiento, y demostraron un buen rendimiento diagnóstico, con mayores SE y SP en las pacientes que se realizaron la PET/TC por sospecha de recurrencia en comparación con los controles de seguimiento, congruente con nuestros resultados. Por el contrario, Ho et al.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">23</span></a> estudiaron mujeres con CS y mostraron que la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG tiene un valor limitado en el seguimiento posterapéutico o la reestadificación tras finalizar el tratamiento.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">En nuestro análisis mostramos solo un caso FP de la PET/TC (<a class="elsevierStyleCrossRef" href="#fig0010">fig. 2</a>) y 4 casos de estudios FN. Estos resultados FN pueden ser debidos al volumen y la forma de las lesiones que, en algunos casos, son la causa de la infraestimación de la señal metabólica debido al efecto de volumen parcial; este efecto está causado por 2 fenómenos diferentes<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">35</span></a>: la pérdida de definición de la imagen tridimensional introducida por la limitada resolución espacial del sistema y el efecto de la fragmentación del tejido debido al muestreo de la imagen; el efecto de volumen parcial afecta principalmente a objetos menores que la mitad de la resolución del escáner. Deben tenerse en cuenta otras posibles explicaciones, como las características histológicas del tumor (por ejemplo, el grado de diferenciación) o la expresión de receptores.</p><p id="par0105" class="elsevierStylePara elsevierViewall">En nuestro estudio, la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG ha demostrado un posible impacto clínico en las pacientes con SU. Una correcta reestadificación y detección de la enfermedad permite optimizar el manejo y el abordaje terapéutico, evitando procedimientos innecesarios en un porcentaje significativo de pacientes. En nuestro análisis obtuvimos que en 8 de 41 pacientes (20%) la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG cambió el manejo. En 2 mujeres, al iniciar quimioterapia tras la detección de lesiones óseas no visualizadas por otras técnicas convencionales, y en 6 mujeres, al demostrar resultados FP de la TC/RM y evitar tratamientos innecesarios.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Otros estudios han mostrado evidencias similares<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">24,26</span></a>: Park et al.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">26</span></a> demostraron que la PET o la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG tenía un impacto en el manejo clínico en 12/36 pacientes (33%), lo que aportó la opción de iniciar un tratamiento previamente no planificado en 8 casos y evitó un tratamiento ya planificado innecesario en 4 casos. En otros estudios<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">23,27</span></a>, la PET con <span class="elsevierStyleSup">18</span>F-FDG permitió un cambio en la gestión del tratamiento en el 25% de las pacientes; en otro estudio, de Rebollo Aguirre et al.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">24</span></a>, el manejo clínico tras la PET/TC cambió en el 30%. En todos los estudios de la literatura, como también en el nuestro, es evidente que una de las fortalezas de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG es intrínseca a su naturaleza de rastreo corporal.</p><p id="par0115" class="elsevierStylePara elsevierViewall">En nuestro trabajo, el SUVmáx medio de las lesiones recurrentes fue 5,8 (rango 2,2-44), menor que los documentados en otros artículos<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">23,28,29</span></a>, posiblemente debido al elevado número de pacientes y estudios evaluados y la distribución homogénea de sus características clínico-patológicas.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Las limitaciones de nuestro estudio son la naturaleza retrospectiva del diseño del estudio, la falta de confirmación anatomopatológica sistemática de las lesiones (a pesar de que este abordaje no sea clínica y éticamente factible) y el relativo bajo número de pacientes analizadas, también debido a la rareza de los SU.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusiones</span><p id="par0125" class="elsevierStylePara elsevierViewall">Pese a los prometedores resultados de la literatura y nuestra experiencia, el papel definitivo de la PET/TC con <span class="elsevierStyleSup">18</span>F-FDG debe explorarse en estudios prospectivos multicéntricos bien diseñados con confirmación anatomopatológica que evite un posible sesgo, con el fin de confirmar o rebatir nuestros conocimientos sobre el posible papel en el algoritmo diagnóstico del SU. Los resultados histopatológicos y/o el seguimiento clínico/de imágenes durante al menos 12 meses se consideraron el estándar de referencia. Se calculó la precisión diagnóstica y el impacto clínico de la <span class="elsevierStyleSup">18</span>F-FDG PET/TC.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Resultados</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">treinta y tres estudios <span class="elsevierStyleSup">18</span>F-FDG PET/TC fueron positivos, mientras que los 40 estudios restantes fueron negativos. Histopathology results and/or clinical/imaging follow-up for at least 12 months were considered the reference standard. The diagnostic accuracy and clinical impact of <span class="elsevierStyleSup">18</span>F-FDG PET/CT was calculated.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Thirty-three <span class="elsevierStyleSup">18</span>F-FDG PET/CT were positive, while the remaining 40 studies were negative.
