Indeed, both of these Nuclear Medicine studies are included in the 2015 guidelines of the European Society of Cardiology as options for the diagnosis of prosthetic infectious endocarditis in cases in which the usual measures are doubtful.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In these doubtful cases the functional alterations detected by <span class="elsevierStyleSup">18</span>F-FDG PET/CT can be superimposed with the anatomical localization to achieve correct diagnosis of the presence or not of infection. Several studies have demonstrated the clearly superior diagnostic accuracy of PET/CT with <span class="elsevierStyleSup">18</span>F-FDG compared with the modified Duke criteria usually used,<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a> significantly reducing the number of doubtful cases. This technique may also constitute a means of monitoring response to antibiotic treatment.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Moreover, PET/CT is able to detect possible foci of septic peripheral embolism and even unknown neoplastic processes.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,11</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">However, the performance of the technique, and overall, the preparation of the patient prior to the study should be carefully undertaken since they are fundamental to achieving maximum diagnostic efficacy to determine the presence or not of infection. The latest guidelines of the European Association of Nuclear Medicine, which are not yet available online, include recommendations to this respect.</p><p id="par0025" class="elsevierStylePara elsevierViewall">In this issue of REMNIM Aguadé et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> provide an exhaustive description of all the details that should be taken into account in the preparation of patients in order to achieve optimum diagnostic quality of the study as well as how to make and interpret the images obtained.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Basically, patient preparation includes the consumption of a diet rich in fats and poor in carbohydrates and fasting of 12–18<span class="elsevierStyleHsp" style=""></span>h together with the administration of low dose heparin. Accordingly, during the procedure the myocardium will use very little glucose which is its usual source of energy and will instead use fatty acids, thereby reducing the physiological uptake of <span class="elsevierStyleSup">18</span>F-FDG by the myocardium and allowing the identification of areas of inflammation-infection. Nonetheless, it should be taken into account that the physiological uptake of <span class="elsevierStyleSup">18</span>F-FDG by the myocardium may vary greatly not only among patients but also in the same patient at different times. However, following the directions provided, our mission is to attempt to minimize this as far as possible. The article provides diet models and useful food suggestions.</p><p id="par0035" class="elsevierStylePara elsevierViewall">On the other hand, the processing and interpretation of the images in these cases are not standardized. Aguadé et al. give a series of technical recommendations to achieve high quality images and improve the diagnostic capacity of the study. Among these recommendations, of note are the fusion with angio-CT, synchronization with the ECG in both the PET and the CT, the possibility of delayed images at 2–3<span class="elsevierStyleHsp" style=""></span>h in doubtful cases, iterative reconstruction with systems of image improvement while preserving adequate spatial resolution using filters which are not too smooth, systems of correction of metallic artifacts, and overall, careful reorientation for the most adequate analysis of the valvular plane and other structures in each case. They also include specific recommendations of image interpretation to determine the presence or not of infection and consider semiquantitative values (SUVmax, SUV ration) as complementary data to visual evaluation.</p><p id="par0040" class="elsevierStylePara elsevierViewall">With regard to quantification, the results of Kanapinn et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> are of interest. They propose performing an initial PET/CT study after the implantation of the device without signs of infection, which may thereafter be used as a reference for comparison in a second study if there is suspicion of infection. Of note also is the study by Memmott et al.,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> who obtained the best results when the SUV ratio was obtained using hepatic activity as the area of reference, highlighting that it is important for each center to carefully analyze the results based on the equipment and protocols used.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The close collaboration of a multidisciplinary team is the best strategy of action for decision making in the diagnosis and treatment of cases of endocarditis of cardiac prostheses or devices.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,15</span></a> A multidisciplinary approach is also essential for performing and interpreting PET/CT with <span class="elsevierStyleSup">18</span>F-FDG, including nuclear medicine with specialists in radiology and cardiology to obtain the greatest diagnostic effectiveness.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,9</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">The detection of inflammatory and infectious involvement in the vascular system is also an increasingly more frequent indication for PET/CT with <span class="elsevierStyleSup">18</span>F-FDG, as shown in two of the images of interest also published in this issue. One image corresponds to infection of an axillary bifemoral by-pass<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> and the other shows recurrence of Takayasu arteritis,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> demonstrating the greater accuracy of the diagnosis and extension of involvement with this study compared to other techniques.</p><p id="par0055" class="elsevierStylePara elsevierViewall">A new tracer has been developed, <span class="elsevierStyleSup">18</span>F-Fluoromaltotriose. This tracer is taken up by maltodextrin in Gram-positive and Gram-negative bacteria, and experimental studies have shown its specific uptake in bacterial infections,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> with clear differentiation of non-infectious inflammatory lesions. This is a very promising perspective in this field and may also be potentially useful in effective monitoring of response to antibiotic therapy.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Myocardial perfusion and viability, ventricular function and synchrony, myocardial metabolism, myocardial blood flow, cardiac symphathetic innervation, detection of amyloidosis and cardiac sarcoidosis as well as the detection of cardiac and vascular infection and inflammation are all included in the wide scope of action and optimistic future of Nuclear Cardiology as recently described by Iskandrian, the pioneer and promoter of Nuclear Cardiology in the Plenary Session of the ICNC 2017.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Tormo IC.
