marcado con carbono-11 &#40;<span class="elsevierStyleSup">11</span>C&#41; fue el primer radiotrazador amiloide utilizado para la detecci&#243;n <span class="elsevierStyleItalic">in vivo</span> del dep&#243;sito cerebral de amiloide en pacientes con demencia por enfermedad de Alzheimer &#40;EA&#41;<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">1&#8211;3</span></a>&#46; En la actualidad&#44; su valor como biomarcador de EA es similar al p&#233;ptido &#946;-amiloide 1-42 detectado en el l&#237;quido cefalorraqu&#237;deo &#40;LCR&#41;&#46; La mayor&#237;a de estudios sobre <span class="elsevierStyleSup">11</span>C-PIB PET se han realizado con pacientes con deterioro cognitivo&#44; con el fin de detectar la presencia de EA en este contexto&#59; para este fin&#44; en el entorno cl&#237;nico se ha validado un an&#225;lisis visual cualitativo<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">4&#8211;7</span></a>&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">La hidrocefalia a presi&#243;n normal idiop&#225;tica &#40;HPNi&#41; es una causa potencialmente tratable de trastorno cognitivo mediante la derivaci&#243;n ventricular quir&#250;rgica&#46; En estos pacientes&#44; existe una alteraci&#243;n de la din&#225;mica del LCR<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">8</span></a>&#44; y adem&#225;s se han descrito algunas alteraciones estructurales en la sustancia blanca y gris cerebral<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">9&#44;10</span></a>&#46; Por otra parte&#44; en este tipo de pacientes se ha detectado una comorbilidad importante con la EA<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">11&#44;12</span></a>&#46; Debido a la alteraci&#243;n en la din&#225;mica del LCR&#44; podr&#237;a ser que ello influyera sobre la distribuci&#243;n cerebral del <span class="elsevierStyleSup">11</span>C-PIB&#44; sin que hubiera una relaci&#243;n con la EA&#46; Esta posible influencia puede suponer una dificultad adicional en la lectura de im&#225;genes para un informe de tipo binario en el diagn&#243;stico de EA&#44; motivando a que se tuviera en cuenta a la hora de abordar el estudio de un posible dep&#243;sito cerebral de amiloide en pacientes con HPNi valorado mediante <span class="elsevierStyleSup">11</span>C-PIB PET&#47;TC&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">Nuestro grupo ha iniciado un estudio a largo plazo para investigar si el dep&#243;sito cerebral de amiloide evaluado mediante <span class="elsevierStyleSup">11</span>C-PIB PET&#47;TC puede constituir un factor pron&#243;stico de la evoluci&#243;n del estatus cognitivo tras la implantaci&#243;n de una derivaci&#243;n ventricular cerebral&#46; En una primera fase&#44; hemos comparado el patr&#243;n de distribuci&#243;n cerebral de <span class="elsevierStyleSup">11</span>C-PIB de un grupo de pacientes con HPNi seleccionados para cirug&#237;a de derivaci&#243;n ventricular con un grupo control de poblaci&#243;n normal&#44; para determinar si hab&#237;a diferencias&#46; Creemos que esta informaci&#243;n puede contribuir a un mejor conocimiento de la enfermedad&#44; as&#237; como a una interpretaci&#243;n m&#225;s precisa de este tipo de gammagraf&#237;a cerebral&#46; El objetivo del estudio fue evaluar el patr&#243;n de distribuci&#243;n cerebral de <span class="elsevierStyleSup">11</span>C-PIB en pacientes con HPNi en comparaci&#243;n con sujetos con un nivel cognitivo normal&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materiales y m&#233;todos</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Grupo de pacientes</span><p id="par0020" class="elsevierStylePara elsevierViewall">El grupo de estudio incluy&#243; a 13 pacientes&#44; 6 varones y 7 mujeres con una edad media de 73<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>6 a&#241;os&#44; seleccionados para cirug&#237;a de derivaci&#243;n ventricular&#46; Los criterios de inclusi&#243;n fueron&#58; diagn&#243;stico cl&#237;nico probable o posible HPNi de acuerdo con los criterios de Relkin et al&#46;<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">13</span></a>&#44; hallazgos neurorradiol&#243;gicos de hidrocefalia sin evidencia de obstrucci&#243;n del LCR &#40;&#237;ndice de Evans &#62;<span class="elsevierStyleHsp" style=""></span>0&#44;30 y borramiento de los surcos de la convexidad cerebral&#41; criterios de una din&#225;mica del LCR tributaria de tratamiento quir&#250;rgico y ausencia de datos sugestivos de una HPN secundaria&#46; Los criterios de exclusi&#243;n fueron la sospecha de HPN secundaria &#40;meningitis&#44; hemorragia subaracnoidea&#44; hemorragia&#44; traumatismo o tumor cerebral&#41;&#44; afasia o deterioro sensitivo severo y aquellas comorbilidades que pudieran comprometer la evaluaci&#243;n de la respuesta posquir&#250;rgica a la derivaci&#243;n ventricular&#46; La <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a> muestra las caracter&#237;sticas cl&#237;nicas de los pacientes&#46; Ninguno de los pacientes mostr&#243; un deterioro cognitivo cl&#237;nicamente sugestivo de una EA&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Grupo control</span><p id="par0025" class="elsevierStylePara elsevierViewall">El grupo control incluy&#243; a 7 sujetos sanos&#44; 3 varones y 4 mujeres con una edad media de 64<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>10 a&#241;os&#44; una exploraci&#243;n neurol&#243;gica normal&#44; y remitidos por la unidad de deterioro cognitivo de nuestro hospital&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">El estudio fue aprobado por el comit&#233; &#233;tico de nuestra instituci&#243;n&#44; y en todos los participantes se obtuvo su consentimiento informado por escrito&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">S&#237;ntesis de <span class="elsevierStyleSup">11</span>C-PIB</span><p id="par0035" class="elsevierStylePara elsevierViewall">La radios&#237;ntesis de <span class="elsevierStyleSup">11</span>C-PIB se llev&#243; a cabo en el departamento de medicina nuclear de nuestro hospital&#46; Dicha s&#237;ntesis se realiz&#243; mediante el proceso en un solo paso a partir de <span class="elsevierStyleSup">11</span>C-triflato de metilo&#46; El proceso completo de s&#237;ntesis ya ha sido descrito previamente<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">14</span></a>&#46; El producto final administrado conten&#237;a 1&#44;28<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>0&#44;8<span class="elsevierStyleHsp" style=""></span>&#956;g de PIB&#46; La actividad espec&#237;fica fue de 138<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>35<span class="elsevierStyleHsp" style=""></span>GBq&#47;&#956;mol y su pureza radioqu&#237;mica fue superior al 99&#37;&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Adquisici&#243;n de las im&#225;genes</span><p id="par0040" class="elsevierStylePara elsevierViewall">En todos los pacientes y controles&#44; los estudios <span class="elsevierStyleSup">11</span>C-PIB PET&#47;TC se adquirieron con un equipo Biograph<span class="elsevierStyleSup">&#174;</span> Pico-3D LSO &#40;Siemens Healthcare Molecular Imaging&#44; Hoffman Estates&#44; IL&#44; EE&#46;UU&#46;&#41;&#46; Previamente a la adquisici&#243;n de la PET se coloc&#243; una v&#237;a endovenosa en el brazo a todos los pacientes&#46; Previamente a la administraci&#243;n del radiotrazador se pidi&#243; a los pacientes que durante 20<span class="elsevierStyleHsp" style=""></span>min permanecieran en dec&#250;bito supino en una habitaci&#243;n tranquila y con iluminaci&#243;n tenue&#46; Todos los pacientes recibieron una dosis ev&#46; de 555<span class="elsevierStyleHsp" style=""></span>MBq de <span class="elsevierStyleSup">11</span>C-PIB&#46; La adquisici&#243;n de la <span class="elsevierStyleSup">11</span>C-PIB PET consisti&#243; en una imagen est&#225;tica entre los 60 y 90<span class="elsevierStyleHsp" style=""></span>min tras la inyecci&#243;n del radiotrazador&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">Se utiliz&#243; la informaci&#243;n proporcionada por la TC para corregir la atenuaci&#243;n de las im&#225;genes de la PET&#46; Las im&#225;genes se reconstruyeron con matriz de 128<span class="elsevierStyleHsp" style=""></span>&#215;<span class="elsevierStyleHsp" style=""></span>128 pixels&#44; utilizando el m&#233;todo iterativo de maximizaci&#243;n de la esperanza de los subgrupos ordenados &#40;6 iteraciones y 16 subgrupos&#59; el zoom fue de 2&#44;5&#44; filtro FWHM de 2<span class="elsevierStyleHsp" style=""></span>mm y factor de ajuste del sinograma de 1&#44;5&#41;&#46; Los cortes axiales se orientaron en paralelo al eje fronto-occipital&#46;</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">An&#225;lisis de las im&#225;genes</span><p id="par0050" class="elsevierStylePara elsevierViewall">Cada uno de los estudios fue analizado por 2 m&#233;dicos nucleares experimentados&#44; que no tuvieron acceso a la informaci&#243;n cl&#237;nica del paciente durante la elaboraci&#243;n del informe&#46; Las im&#225;genes se presentaron en escalas de grises y de color microdelta&#46; Se llev&#243; a cabo un an&#225;lisis visual de la intensidad de retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB&#44; mediante una escala de 1 a 4 para la sustancia blanca y gris por separado&#46; Considerando nuestra experiencia previa en la lectura de im&#225;genes <span class="elsevierStyleSup">11</span>C-PIB PET&#47;TC se asign&#243; el grado m&#225;ximo de intensidad a la retenci&#243;n habitualmente elevada del <span class="elsevierStyleSup">11</span>C-PIB observada en la sustancia blanca del cerebelo de los sujetos normales&#46; El resto de grados se asign&#243; compar&#225;ndolos con dicha captaci&#243;n&#46; Por consiguiente&#44; la intensidad de la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en las sustancias gris y blanca cerebral se puntu&#243; de la siguiente manera&#58; 1&#44; retenci&#243;n ligera &#40;&#43;&#41;&#59; 2&#44; retenci&#243;n moderada &#40;&#43;&#43;&#41;&#59; 3 retenci&#243;n alta &#40;&#43;&#43;&#43;&#41;&#59; y 4&#44; retenci&#243;n muy alta &#40;&#43;&#43;&#43;&#43;&#41;&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">Se analizaron y puntuaron las siguientes regiones topogr&#225;ficas corticales&#58; frontal&#44; temporal&#44; parietal y occipital en 4 cortes axiales&#44; uno infratentorial y 3 supratentoriales&#58; inferior&#44; medio y superior&#59; en 2 cortes sagitales y en 2 coronales&#46; La sustancia blanca se analiz&#243; y puntu&#243; en las siguientes regiones&#58; cerebelo&#44; protuberancia&#44; c&#225;psula blanca inferior y regi&#243;n periventricular media y superior&#46; Para cada paciente se elabor&#243; un informe completo en t&#233;rminos de resultado positivo&#44; negativo o dudoso&#46; Para dicho informe cl&#237;nico&#44; se consider&#243; un estudio <span class="elsevierStyleSup">11</span>C-PIB positivo cuando se obtuvo una puntuaci&#243;n de 4 &#40;&#43;&#43;&#43;&#43;&#41; o 3 &#40;&#43;&#43;&#43;&#41; en cualquier regi&#243;n cortical&#46; Aquellas regiones corticales con puntuaci&#243;n igual o inferior a 2 &#40;&#43;&#43;&#41; fueron informadas como negativas&#46; Adem&#225;s&#44; aquellas regiones sin una diferenciaci&#243;n clara entre la intensidad de la retenci&#243;n del <span class="elsevierStyleSup">11</span>C-PIB en la sustancia cortical y blanca se interpretaron como dudosas&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">La puntuaci&#243;n de cada regi&#243;n para cada paciente se decidi&#243; en funci&#243;n de la informaci&#243;n mostrada en los cortes axiales&#44; sagitales y coronales&#46; La atrofia cerebral se puntu&#243; de forma global entre 0 y 4 de acuerdo con la escala de Scheltens et al&#46;<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">15</span></a> en las im&#225;genes de la TC&#46; Las discrepancias que aparecieron entre los facultativos que efectuaron la valoraci&#243;n de las im&#225;genes se resolvieron por consenso&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">La puntuaci&#243;n relativa para la sustancia gris y blanca de las diferentes regiones en los controles y en el grupo de pacientes se expres&#243; en forma de proporci&#243;n&#44; donde el numerador estaba compuesto por el n&#250;mero de puntos totales&#44; esto es&#44; el n&#250;mero de &#43; aplicado al an&#225;lisis visual de cada regi&#243;n&#44; y en el denominador hab&#237;a el n&#250;mero obtenido si todas las regiones de cada paciente o control hubieran obtenida una puntuaci&#243;n m&#225;xima &#40;&#43;&#43;&#43;&#43;&#41;&#46; Ello se llev&#243; a cabo por separado para las regiones infratentorial y supratentorial&#44; tanto de la sustancia blanca como gris&#46;</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">An&#225;lisis estad&#237;stico</span><p id="par0070" class="elsevierStylePara elsevierViewall">Se compararon las diferencias categ&#243;ricas expresadas como frecuencias mediante la prueba exacta de Fisher para muestras independientes&#46; El an&#225;lisis estad&#237;stico se realiz&#243; con la version 15&#46;0 del Statistical Package for Social Sciences &#40;SPSS<span class="elsevierStyleSup">&#174;</span>&#41; para Windows &#40;SPSS Inc&#46;&#44; Chicago&#44; Il&#44; EE&#46;UU&#46;&#41;&#46; La significaci&#243;n estad&#237;stica se estableci&#243; con un valor de p igual o inferior a 0&#44;05&#46;</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Resultados</span><p id="par0075" class="elsevierStylePara elsevierViewall">En el grupo control&#44; con respecto a la retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB&#44; 4&#47;7 sujetos mostraron una intensidad ligera en todas sus regiones&#44; y en 3&#47;7 se observ&#243; por lo menos una regi&#243;n con una retenci&#243;n moderada&#46; La retenci&#243;n cortical se localiz&#243; en la regi&#243;n parietal y frontal superior &#40;1&#47;3&#41;&#44; en la regi&#243;n parietal &#40;1&#47;3&#41; y en la regi&#243;n temporal &#40;1&#47;3&#41;&#46; No hubo ning&#250;n paciente con una retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB alta ni muy alta &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>&#41;&#46; La <a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a> muestra la intensidad de la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en diferentes regiones de la sustancia blanca&#46; Respecto a la retenci&#243;n del radiotrazador en la sustancia blanca de los sujetos control&#44; en 6&#47;7 fue m&#225;xima en el cerebelo&#44; en 6&#47;7 en la protuberancia y en 4&#47;7 en la regi&#243;n periventricular superior&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">En el grupo de pacientes&#44; con respecto a la retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB&#44; 3 mostraron una intensidad alta en casi todas las regiones&#44; en 2 hubo por lo menos una regi&#243;n con retenci&#243;n elevada y 8 pacientes mostraron una intensidad de retenci&#243;n del radiotrazador ligera o moderada&#44; similar a los controles &#40;<a class="elsevierStyleCrossRef" href="#tbl0020">tabla 4</a>&#41;&#46; En relaci&#243;n a la sustancia blanca en el grupo de pacientes&#44; 10 mostraron una retenci&#243;n alta o muy alta en todas las regiones&#46; Adem&#225;s 3 pacientes mostraron una retenci&#243;n moderada del <span class="elsevierStyleSup">11</span>C-PIB en algunas regiones&#44; especialmente en &#225;reas periventriculares &#40;<a class="elsevierStyleCrossRef" href="#tbl0025">tabla 5</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">En un an&#225;lisis de profundidad&#44; hemos comparado la puntuaci&#243;n obtenida en la sustancia blanca de regiones infratentorial y supratentorial de los 2 grupos&#46; Los resultados para el grupo control fueron de 54&#47;56 puntos en la sustancia blanca infratentorial y de 70&#47;84 en la sustancia blanca supratentorial&#44; mientras que para el grupo de pacientes estos resultados fueron respectivamente de 92&#47;104 y de 122&#47;156 puntos&#46; Concretamente en la sustancia blanca periventricular superior&#44; los resultados fueron de 25&#47;28 y de 40&#47;52 puntos en el grupo control y de pacientes&#44; respectivamente&#46; Cuando se compararon ambos grupos&#44; hubo diferencias significativas &#40;54&#47;56 vs&#46; 92&#47;104&#59; p<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#44;05&#41; en la sustancia blanca infratentorial&#44; pero no hubo diferencia estad&#237;sticamente significativa en la sustancia blanca global supratentorial entre pacientes con HPNi y controles &#40;70&#47;84 vs&#46; 122&#47;156&#59; p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;327&#59; ns&#41;&#46; Sin embargo&#44; se apreci&#243; una tendencia a una menor puntuaci&#243;n en la sustancia blanca superior de los pacientes con HPNi respecto a los controles &#40;25&#47;28 vs&#46; 40&#47;52&#59; p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;134&#41;&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">Para el &#225;mbito cl&#237;nico se emiti&#243; un informe completo clasific&#225;ndose a 3 pacientes como positivos&#44; 2 como dudosos y 8 como negativos&#46;</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discusi&#243;n</span><p id="par0095" class="elsevierStylePara elsevierViewall">La HPNi se caracteriza cl&#237;nicamente por una tr&#237;ada cl&#225;sica de s&#237;ntomas&#58; deterioro cognitivo&#44; trastornos de la marcha e incontinencia urinaria<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">13</span></a>&#46; Los s&#237;ntomas cognitivos pueden evolucionar hacia una demencia&#44; siendo considerada como una causa potencialmente tratable de demencia mediante la derivaci&#243;n quir&#250;rgica ventricular<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">16</span></a>&#46; Aunque se desconoce su etiolog&#237;a exacta&#44; se ha apuntado una fisiopatolog&#237;a caracter&#237;stica del proceso ligada a un flujo an&#243;malo del LCR&#46; Mientras que en condiciones normales el LCR fluye hacia afuera del cerebro a trav&#233;s de los ventr&#237;culos&#44; en los pacientes con HPNi puede existir un fen&#243;meno de flujo retr&#243;grado de LCR hacia el cerebro a trav&#233;s de los ventr&#237;culos<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">17</span></a>&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">En la literatura se ha visto que&#44; hasta un 40-60&#37; de los pacientes con HPNi&#44; muestran un dep&#243;sito histol&#243;gico de amiloide cerebral de causa incierta<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">11</span></a>&#46; Ello podr&#237;a estar potencialmente relacionado con un menor aclaramiento de LCR&#44; y a una modificaci&#243;n en el equilibrio existente entre la prote&#237;na &#946;-amiloide soluble e insoluble&#44; o a un origen com&#250;n con la EA<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">18</span></a>&#46; Por otra parte&#44; en este tipo de pacientes pueden aparecer algunas alteraciones estructurales que afectan a regiones de la sustancia blanca&#44; como queda reflejado por la hiperintensidad en la se&#241;al o la atrofia visible en las im&#225;genes de la RM<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">19</span></a>&#46; En el an&#225;lisis visual b&#225;sico del presente estudio&#44; nos hemos centrado en el patr&#243;n de retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en una poblaci&#243;n de pacientes con HPNi sin deterioro cognitivo sugestivo de EA&#44; e investigamos si exist&#237;a alguna diferencia con el mismo patr&#243;n de retenci&#243;n de sujetos sanos&#46; Esto es importante porque nuestra hip&#243;tesis plantea que quiz&#225;s los diferentes factores previamente se&#241;alados podr&#237;an alterar potencialmente el n&#250;cleo de los criterios de diagn&#243;stico por la imagen habitualmente empleados para considerar un estudio con <span class="elsevierStyleSup">11</span>C-PIB positivo o negativo&#44; esto es&#44; el contraste entre la retenci&#243;n del radiotrazador en la sustancia gris y blanca&#44; desarrollado primariamente para identificar los casos de EA&#46; Recientemente se han iniciado algunos estudios prospectivos que han estudiado el dep&#243;sito de amiloide cerebral como factor pron&#243;stico de una evoluci&#243;n cognitiva satisfactoria tras la cirug&#237;a de derivaci&#243;n ventricular en casos de HPNi&#46; En este sentido&#44; este estudio b&#225;sico podr&#237;a justificarse para evitar una posible interpretaci&#243;n err&#243;nea de un estudio con <span class="elsevierStyleSup">11</span>C-PIB&#44; para conocer si existe un patr&#243;n espec&#237;fico de distribuci&#243;n en la HPNi&#44; as&#237; como para detectar diferencias con el patr&#243;n de la EA y que ser&#237;a una circunstancia importante a tener en cuenta &#40;<a class="elsevierStyleCrossRef" href="#fig0005">fig&#46; 1</a>&#41;&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">En primer lugar&#44; en relaci&#243;n a la situaci&#243;n de la sustancia blanca de nuestra poblaci&#243;n control&#44; la retenci&#243;n del <span class="elsevierStyleSup">11</span>C-PIB mostr&#243; una puntuaci&#243;n alta o muy alta en todos los sujetos &#40;&#43;&#43;&#43; o &#43;&#43;&#43;&#43; para todas las regiones&#41;&#44; localiz&#225;ndose la mayor retenci&#243;n fundamentalmente en el cerebelo &#40;6&#47;7&#41; o en la protuberancia &#40;6&#47;7&#41;&#46; Solamente hubo un control en el que se observ&#243; una diferencia de intensidad leve entre ambos &#40;solamente una &#43;&#41;&#46; De forma global&#44; en la sustancia blanca periventricular&#44; la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB fue ligeramente menos intensa &#40;&#43;&#43;&#43;&#41; en el corte medio de todos los controles&#44; con valores en la sustancia blanca de la c&#225;psula interna y periventricular tan elevados como &#43;&#43;&#43;&#43; en el 50&#37; y de &#43;&#43;&#43; en el 50&#37; restante&#46; Ello coincide con la integridad funcional y probablemente estructural de la sustancia blanca&#44; mostrando el mismo patr&#243;n ya descrito<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">1&#44;20</span></a>&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">Sin embargo&#44; el comportamiento es diferente si evaluamos la sustancia blanca en pacientes con HPNi&#46; En 3&#47;13&#44; la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB fue inferior a la de los controles&#44; si bien la retenci&#243;n m&#225;xima observada en la sustancia blanca para cada paciente se localiz&#243; en el cerebelo en 12&#47;13 y en la protuberancia en 10&#47;13&#46; En general&#44; la intensidad de la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en los pacientes con HPNi mostr&#243; una tendencia a ser inferior en la sustancia blanca supratentorial respecto a los controles y&#44; especialmente&#44; hubo diferencias en la sustancia blanca periventricular superior &#40;<a class="elsevierStyleCrossRefs" href="#tbl0015">tablas 3 y 5</a>&#41;&#44; as&#237; como en la sustancia blanca infratentorial&#44; donde se apreciaron diferencias estad&#237;sticamente significativas &#40;54&#47;56 vs&#46; 92&#47;104&#59; p<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#44;05&#41;&#46; La visi&#243;n global de la <a class="elsevierStyleCrossRef" href="#tbl0025">tabla 5</a> muestra que la intensidad de retenci&#243;n en las estructuras de la sustancia blanca presenta m&#225;s variabilidad que en el grupo control &#40;<a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a>&#41;&#44; y que existen 4&#47;13 pacientes en los que la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB est&#225; ligeramente disminuida en todas las regiones&#44; incluyendo el cerebelo&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">Estos resultados pueden estar relacionados con los cambios descritos en t&#233;rminos de hiperintensidad focal y de alteraci&#243;n de la sustancia blanca en las im&#225;genes de RM&#44; incluyendo el tensor de difusi&#243;n mediante RM&#44; en pacientes con HPNi y ya descritas ampliamente en la literatura&#44; y que pueden diferenciar a estos pacientes de los controles y de los pacientes con EA<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">19&#44;21</span></a>&#46; Sin embargo&#44; consideramos que el conocimiento de las propiedades del <span class="elsevierStyleSup">11</span>C-PIB en la sustancia blanca normal y alterada constituye un punto clave&#46; Goodheart et al&#46; demostraron una reducci&#243;n en la retenci&#243;n del <span class="elsevierStyleSup">11</span>C-PIB en &#225;reas de hiperintensidad de la sustancia blanca &#40;HSB&#41;&#44; especialmente en &#225;reas periventriculares<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">22</span></a>&#46; Los autores describen que la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en la sustancia blanca en pacientes clasificados visualmente con una HSB importante &#40;grado visual &#62;<span class="elsevierStyleHsp" style=""></span>3&#41;&#44; es significativamente inferior a la de pacientes que no presenta una HSB importante &#40;grado visual &#60;<span class="elsevierStyleHsp" style=""></span>3&#41;&#46; Coincidimos con los autores en que la explicaci&#243;n m&#225;s probable para estos resultados reside en que la sustancia blanca de las regiones con HSB puede presentar unas caracter&#237;sticas diferentes relativas a la retenci&#243;n del <span class="elsevierStyleSup">11</span>C-PIB respecto a la sustancia blanca normal&#46; En este sentido&#44; Stankoff et al&#46; se&#241;alaron que la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en cerebros animales y humanos depende de la cantidad de mielina presente<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">23</span></a>&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">Otro factor que puede influir en la imagen de retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en la sustancia blanca de pacientes con HPNi es el menor flujo cerebral sangu&#237;neo existente&#44; que podr&#237;a recuperarse tras la derivaci&#243;n ventricular&#44; como ya ha sido descrito<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">24</span></a>&#46; Esta alteraci&#243;n importante puede ser secundaria a comorbilidad con una enfermedad vascular cerebral<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">25&#44;26</span></a>&#44; as&#237; como tambi&#233;n a un estr&#233;s mec&#225;nico en las &#225;reas periventriculares&#44; que en la actualidad se est&#225; evaluando como factor etiol&#243;gico implicado en la degeneraci&#243;n neuronal<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">27</span></a>&#46; En nuestro estudio&#44; el componente TC apreci&#243; algunas alteraciones focales secundarias a enfermedad vascular cerebral severa en dos pacientes&#44; pero no mostraron ninguna alteraci&#243;n difusa de la sustancia blanca&#46; Por otra parte&#44; en 7&#47;13 pacientes el grado de atrofia cerebral detectado fue similar a la de los controles&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">En segundo lugar&#44; respecto a la retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB&#44; en el grupo control se observ&#243; una retenci&#243;n ligera &#40;&#43;&#41; en casi todas las regiones corticales&#46; No obstante&#44; 3&#47;7 controles mostraron una retenci&#243;n cortical del <span class="elsevierStyleSup">11</span>C-PIB moderada &#40;&#43;&#43;&#41; en 4 regiones aisladas&#59; 2 parietales&#44; un frontal superior y un temporal&#46; En nuestro grupo control&#44; el patr&#243;n de retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB podr&#237;a describirse como uniformemente muy bajo&#44; en concordancia con lo ya descrito previamente<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">1</span></a>&#46;</p><p id="par0130" class="elsevierStylePara elsevierViewall">La poblaci&#243;n de pacientes del estudio mostr&#243; un comportamiento diferente del <span class="elsevierStyleSup">11</span>C-PIB&#46; Tres de 13 pacientes con HPNi mostraron una retenci&#243;n muy intensa del radiotrazador en todas las regiones corticales&#46; Las im&#225;genes se informaron como cl&#237;nicamente positivas&#46; Una primera consideraci&#243;n ser&#237;a que estos hallazgos podr&#237;an estar reflejando la incidencia normal del dep&#243;sito de amiloide ya conocido en una poblaci&#243;n normal&#44; as&#237; como dep&#243;sitos de amiloide relacionados con una EA&#46; Ser&#237;a deseable establecer un seguimiento de estos pacientes&#44; as&#237; como un an&#225;lisis comparativo m&#225;s profundo con una cohorte grande de pacientes con EA y <span class="elsevierStyleSup">11</span>C-PIB positivo&#44; para poder dilucidar el significado de estos hallazgos&#46;</p><p id="par0135" class="elsevierStylePara elsevierViewall">En relaci&#243;n a los 10 pacientes restantes con HPNi&#44; 3&#47;10 no mostraron ninguna retenci&#243;n cortical que fuera superior a la observada en los controles&#44; siendo visualmente muy similar a un control sano&#44; y 7&#47;10 mostraron cierto grado de retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB&#44; similar o m&#225;s intensa que los controles&#44; predominantemente de forma sim&#233;trica y en &#225;reas parasagitales superiores&#44; como ya describieron Kondo et al&#46;<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">28</span></a>&#46; quienes lo consideraron como un patr&#243;n caracter&#237;stico de EA&#46; Nuestro an&#225;lisis visual de los pacientes puede estar influido por un efecto colateral&#44; y la falta de correcci&#243;n del efecto de volumen parcial tambi&#233;n puede jugar un papel&#46; Sin embargo&#44; estos posibles factores de confusi&#243;n tambi&#233;n deber&#237;an haber influido en el an&#225;lisis visual de nuestra poblaci&#243;n control&#44; aunque este no fuera el caso&#46;</p><p id="par0140" class="elsevierStylePara elsevierViewall">En general&#44; en la HPNi&#44; creemos que frecuentemente existe un menor contraste de la retenci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB entre la sustancia gris y blanca&#46; Sospechamos que la circulaci&#243;n lenta de LCR puede contribuir a una mayor concentraci&#243;n de <span class="elsevierStyleSup">11</span>C-PIB en el torrente sangu&#237;neo y potencialmente en las regiones superiores&#44; mostrando una distribuci&#243;n con gradiente visual en concordancia con el gradiente de presi&#243;n del LCR y con la dificultad de drenaje desde el torrente sangu&#237;neo hacia el LCR&#44; como se ha descrito y sugerido previamente<a class="elsevierStyleCrossRefs" href="#bib0305"><span class="elsevierStyleSup">29&#44;30</span></a>&#46; Adem&#225;s&#44; la alteraci&#243;n funcional y estructural de la sustancia blanca secundaria al estr&#233;s mec&#225;nico y a la leucoencefalopat&#237;a vascular cerebral conocidas en estos pacientes&#44; contribuyen a reducir el contraste resultante entre la sustancia gris y blanca&#46;</p><p id="par0145" class="elsevierStylePara elsevierViewall">Este estudio presenta algunas limitaciones&#46; La principal es que la muestra es reducida&#46; Esta circunstancia puede afectar a la reproducibilidad de nuestros resultados&#46; La incidencia de pacientes positivos para <span class="elsevierStyleSup">11</span>C-PIB puede explicarse por la incidencia normal del dep&#243;sito amiloide cerebral o por una anatom&#237;a patol&#243;gica de EA no detectada en el grupo control&#46; Por el momento&#44; no disponemos de ninguna muestra histol&#243;gica cerebral para determinar la presencia de amiloide&#44; pero autores como Leionen et al&#46; han demostrado la concordancia existente entre los dep&#243;sitos de amiloide visualmente detectados mediante una PET con flutemetamol y la biopsia histol&#243;gica<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">31&#44;32</span></a>&#46; Es necesario efectuar un seguimiento de estos pacientes&#44; pero creemos que estos resultados pueden ser de inter&#233;s para aumentar el conocimiento sobre esta entidad y para aquellos estudios pron&#243;sticos en curso&#46; Por otra parte&#44; no hemos evaluado la perfusi&#243;n de nuestros pacientes ni controles en el momento de efectuarse la PET&#44; siendo este punto un posible objeto de investigaci&#243;n en el futuro&#46;</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusiones</span><p id="par0150" class="elsevierStylePara elsevierViewall">Los pacientes con hidrocefalia a presi&#243;n normal idiop&#225;tica muestran un aumento de la retenci&#243;n cortical de <span class="elsevierStyleSup">11</span>C-PIB en comparaci&#243;n con sujetos sanos&#44; y una menor retenci&#243;n del radiotrazador en la sustancia blanca&#46; Ello puede conducir a un menor contraste existente en la retenci&#243;n del <span class="elsevierStyleSup">11</span>C-PIB entre la sustancia gris y blanca cerebrales&#46; Esta circunstancia debe tenerse en cuenta a la hora de informar un estudio gammagr&#225;fico de amiloide como positivo o negativo&#46; Es necesario un an&#225;lisis m&#225;s profundo y longitudinal de estos hallazgos para dilucidar su significado&#44;
