B-D, tinción hematoxilina-eosina en la que se objetiva abundante tejido adiposo formado por adipocitos maduros y sin atipias alternado con un componente fibroblástico de células fusiformes que se disponen formando fascículos.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0005">Caso clínico</span><p id="par0005" class="elsevierStylePara elsevierViewall">La paciente es una niña de 4 años, sin antecedentes de interés, que acude derivada desde pediatría por aparición de masa cervical que no remite tras tratamiento antibiótico.</p><p id="par0010" class="elsevierStylePara elsevierViewall">A la exploración física presenta una tumoración cervical anterior de 3<span class="elsevierStyleHsp" style=""></span>cm, móvil, no dolorosa a la palpación, que alcanza la escotadura esternal.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Se realiza ecografía cervical que informa de lesión ovoidea de 4,3 x 1,5<span class="elsevierStyleHsp" style=""></span>cm localizada en tejido celular subcutáneo compatible con lipoma y se completa el estudio con resonancia magnética (<a class="elsevierStyleCrossRef" href="#fig0005">fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">Se solicita una biopsia con aguja gruesa ecoguiada en la que se describe tejido adiposo maduro sin signos de malignidad.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Con el diagnóstico de sospecha de lipoma se decide llevar a cabo tratamiento conservador mediante control clínico semestral y ecográfico anual.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Tras tres años de crecimiento lento progresivo se decide realizar tratamiento quirúrgico procediendo a la exéresis completa de la lesión mediante una cervicotomía.</p><p id="par0035" class="elsevierStylePara elsevierViewall">El fragmento quirúrgico que se extrae presenta un aspecto encapsulado de 6 x 4 x 3,5<span class="elsevierStyleHsp" style=""></span>cm, alternando áreas amarillentas junto a zonas blanquecinas de aspecto fibrosado. Al análisis microscópico se presenta como lóbulos de tejido adiposo separados por septos fibrosos compatible con <span class="elsevierStyleItalic">lipoblastoma</span> (<a class="elsevierStyleCrossRef" href="#fig0010">fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">Se realizan técnicas inmunohistoquímicas que resultan: actina y CD31 negativo, Ki67 menor del 1% y CD34 focalmente positivo, llegando así al diagnóstico de lipofibromatosis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Discusión</span><p id="par0045" class="elsevierStylePara elsevierViewall">La lipofibromatosis es una neoplasia benigna infrecuente, localizada en el tejido celular subcutáneo de extremidades, aunque puede aparecer en otras localizaciones<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Habitualmente cursa de manera asintomática, consultando los pacientes por deformidad estética o por la sintomatología derivada de la compresión de estructuras adyacentes.</p><p id="par0055" class="elsevierStylePara elsevierViewall">En cuanto a la epidemiología, la edad de aparición en promedio es al año de vida, presentado lesiones antes de los 3 años en la mayoría de los pacientes. No hay una clara predominancia por sexos<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0060" class="elsevierStylePara elsevierViewall">El diagnóstico diferencial debe realizarse con entidades como: fibromatosis, fibroma aponeurótico calcificante, hamartoma fibroso de la infancia, lipoblastoma, hamartoma lipofibromatoso, lipoma, malformación linfática macroquística, miofibroma y rabdomiosarcoma<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Para el diagnóstico se utilizan ecografía y resonancia magnética nuclear, pero el diagnóstico definitivo es histológico, presentándose como un tumor constituido por tejido adiposo separado en lóbulos por fascículos de fibroblastos o miofibroblastos con positividad inmunohistoquímica CD99 y CD34 en células fusadas, y S100 en adipocitos<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">4–6</span></a>.</p><p id="par0070" class="elsevierStylePara elsevierViewall">El tratamiento de elección es la exéresis completa.
