The orbit is decompressed and the right nasal cavity is free.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0005">Case report</span><p id="par0005" class="elsevierStylePara elsevierViewall">A 3-year-old child presented at our Referral Centre with fever (38<span class="elsevierStyleHsp" style=""></span>°C), purulent secretions and tearing in the right eye with ipsilateral palpebral edema, cutaneous hyperemia and purulent discharge from the right nasal fossa. His past medical history was negative. After a first evaluation, orbital cellulitis was suspected. An ocular swab was positive for Moraxella. A combined intravenous antibiotic (Ceftriaxone<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>Clindamycin) and steroid therapy, in association with antibiotic drops (Ofloxacin) were administered. After 48<span class="elsevierStyleHsp" style=""></span>h of treatment, with no improvement in his symptoms, a CT scan of the head and orbit was performed, showing a bone density mass arising from the right osteomeatal complex and involving the ethmoid sinus, the medial wall of the maxillary sinus and the inferior-medial portion of the homolateral orbit (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). A diagnosis of paranasal sinus osteoma was made, and surgical removal of the mass was planned. An endoscopic transnasal approach was performed, with subtotal removal of the osteoma. The surgical procedure consisted of complete antero-posterior ethmoidectomy and removal of the osteoma using a bone cavitation technique with the aid of a diamond burr. The main surgical steps are shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>. There were no complications after surgery, with a complete resolution of symptoms and normalization of the laboratory tests. Three days after surgery, the patient was discharged.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">The last follow-up, after 12 months, showed no clinical or endoscopic signs of recurrence (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Discussion</span><p id="par0015" class="elsevierStylePara elsevierViewall">Computed tomography with contrast is the gold standard for diagnosis of orbital cellulitis, by studying the anatomic structures of the orbit such as the orbital walls, extraocular muscles, adipose tissue and paranasal sinuses. It provides information about the extent of the disease and identification of potential sources of infection, for example, sinus diseases or foreign bodies.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">1–9</span></a> Moreover, it permits orbital cellulitis to be subdivided according to Chandler's classification, so as to choose the best clinical management, to monitor the efficacy of the treatment and, if needed, to plan the surgical approach<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a> CT scanning should be undertaken in all patients presenting neurological symptoms or advanced ophthalmological signs such as worsening of visual acuity, gross proptosis, chemosis, ophthalmoplegia or diplopia. It should also be performed when there is no clinical improvement after 24<span class="elsevierStyleHsp" style=""></span>h of medical treatment and in the case of pyrexia which has not resolved over 36<span class="elsevierStyleHsp" style=""></span>h.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">1,2</span></a> In these patients, a sinus CT scan is mandatory within 24<span class="elsevierStyleHsp" style=""></span>h of presentation. MRI is useful to recognize intracranial complications including cavernous sinus thrombosis, epidural, subdural and leptomeningeal involvement, and cortical infections.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a> Osteoma is a rare cause of orbital cellulitis. It is usually sporadic but in a minority of cases, these tumors are observed in combination with intestinal polyposis and cutaneous soft tissue tumors as an expression of Gardner's syndrome.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">3–9</span></a> It most frequently involves the frontal sinus followed by the ethmoid, maxillary and sphenoid sinuses.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a> Sinonasal osteomas are extremely rare in the pediatric population; only 17 cases of osteoma arising from the ethmoid sinus with extension to the orbit have been reported in the literature<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">3–9</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Moreover, most of these cases involved children older than 10 years. Osteomas may remain asymptomatic for a long time, before causing facial deformation or compression of adjacent structures. More commonly, they are an incidental finding on radiographic studies.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">2</span></a> Signs and symptoms depend on the location, size and growth direction of the tumor. Progressive headache and chronic inflammation remain the most common symptoms.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a> Osteomas can also present clinically with serious and sometimes life-threatening complications: mucoceles, sinonasal infection, orbital cellulitis, cerebrospinal fluid leakage, pneumatocele, meningitis, or cerebral abscess.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">4</span></a> This benign tumor should be followed up in the absence of symptoms, but surgery is the only treatment option when persistent symptoms or complications are present.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a> Moreover, in the case of orbital cellulitis, surgery is indicated when there is clinical deterioration or failure to respond to antibiotic therapy.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">1–9</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">Surgical approaches to treat osteomas of the paranasal sinuses can be divided into external approaches, such as lateral rhinotomy for ethmoidal osteomas, a coronal approach for osteomas of the frontal sinus, endoscopic drill-out approaches, and combined endoscopic and external procedures. The most appropriate surgical approach is chosen based on the size and location of the tumor and the experience of the surgeon.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a> Up to now, the gold standard for treatment of paranasal osteomas is the endoscopic endonasal approach. The advantages of this approach are the minimal invasiveness of this procedure, the avoidance of external wounds and consequent cosmetic problems, the shorter hospitalization time, and the preservation of the anatomy and physiology of the paranasal sinuses.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">3,5,6</span></a> Choice of approach is an even more important consideration in the pediatric population, because of the esthetic impact of open procedures. Endoscopic procedures also have several disadvantages; in particular, endoscopic 2D vision requires longer surgical training. Moreover, in children, the narrow nasal cavities make endoscopic procedures more challenging.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">5</span></a> In the literature, studies by Alotaibi et al.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">5</span></a> and Pons et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">3</span></a> reported good surgical outcomes in children who underwent endonasal endoscopic surgical removal of paranasal osteomas. In our case, we used the endoscopic drill cavitation technique; the osteoma was drilled at its center to obtain a cavitation with a thin bony shell that was fractured and removed. A small amount of residual tumor was left attached to the lamina papyracea and the lower part of the maxillary sinus to avoid surgical damage to definitive teeth, to protect the orbit, and to avoid interfering with the maxillofacial and skull-base growth of the patient. This surgical approach was effective in treating the triggering cause of the orbital cellulitis in our patient; there was a rapid improvement in his clinical condition and a long-lasting result without relapse of disease in a follow-up period of 12 months.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Conclusion</span><p id="par0025" class="elsevierStylePara elsevierViewall">Although extremely rare in the pediatric population, osteomas can be an etiological cause of orbital cellulitis and must be considered in infections that do not respond to medical treatment. Acta Otorrinolaringológica Española, Órgano Oficial de la Sociedad Española de Otorrinolaringología y Cirugía de Cabeza y Cuello, y de la Academia Iberoamericana de Otorrinolaringología, es la revista más importante en Español dedicada a la Otorrinolaringología.
