Intravesical migration of intrauterine devices (IUDs) as a result of inadvertent perforation of the uterus during insertion is a rare complication (1/1,000).<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Irritative symptoms together with the absence of the device in exploration should arouse the suspicion of this situation.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Although the endoscopic removal is considered the first treatment option,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> bladder wall involvement may require laparoscopic treatment.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">We present the case of a 37-year-old woman with a history of IUD placement 15 years ago, without any complications related to the procedure. The patient was found to be pregnant within the first months of IUD insertion. As the device was not identified in an ambulatory gynecology exploration, it was assumed that it had been expelled unnoticeably.</p><p id="par0020" class="elsevierStylePara elsevierViewall">She is referred to urology consultation due to an irritative voiding syndrome, with long evolution (years) of dysuria and macroscopic hematuria associating repeated urinary infections. Urine culture was negative. Cystoscopy showed a calcified foreign body of approximately 2<span class="elsevierStyleHsp" style=""></span>cm located in the bladder dome (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). In the CT scan that was carried out to complete the evaluation, a metallic device was observed at the previously mentioned level with potential transmural affectation (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">Finally, a laparoscopic partial cystectomy was performed by transperitoneal approach, obtaining a segment of the bladder wall with an embedded calcified IUD device (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>C).</p><p id="par0030" class="elsevierStylePara elsevierViewall">The migration of an IUD into the bladder is a slow process, which explains the late development of urinary symptoms. These are usually irritative, non-specific symptoms, repeat urinary infections and/or hematuria. As it is a safe and effective technique, the endoscopic removal is the first treatment option.
