Presentamos un extraño caso de un tumor vesical cuya primera manifestación metastásica tras dos años del diagnóstico fue una polineuropatía. Este paciente fue tratado durante un año mediante inmunoterapia con BCG por un carcinoma vesical superficial. Progresivamente aparecieron síntomas neurológicos centrales hasta su fallecimiento un mes más tarde. Una carcinomatosis meníngea fue identificada como la causa. No existían metástasis óseas, que es la vía más común hacia las leptomeninges. En base a nuestro caso, discutimos sobre las vías de acceso a las meninges.
We report a strange case of a bladder whose first metastasic manifestation, after two years of the diagnosis, was a periferic polyneuropathia. This patient was treated with inmunotherapy with BCG for superficial carcinoma of the bladder during one year. Gradually central neurological simptoms appeared and the patient died one month later. A meningeal carcinomatosis was identify as the cause. No bone metastases existed, which is the most frequent way of tumours extension towards leptomeninges. We argue about the way to arrive at meninges.
