In all these circumstances, an individual case, be it clinical case, clinical scientific note, case of the month, or as desired to define, it can be exemplary, teaching, and merit publication. In any case, clinical cases have not been ‘rejected’ in <span class="elsevierStyleItalic">Actas Urológicas Españolas</span>, as expressed by Fariña-Pérez,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> but they have been relegated to a minimum in all medical journals, and in the case before us, in all the urological journals concerned with the improvement of its impact factor. The fact that there are on-line journals focusing solely on case reports, as Fariña-Pérez<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> points out, only shows that wherever is a safe market niche, there will be a trader willing to exploit it. However, that does not imply that it is the future or guarantee of proper operation.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Thomson Scientific considers the impact factor of a journal in terms of a ratio. The higher the numerator (greater number of citations of the journal analyzed expressed in the two years prior to the one being analyzed) and the lower the denominator (lower number of source articles), the higher a factor corresponds. There is nothing personal in this rule. The articles that produce no or very little citation, and only increase the denominator, cannot be favored by a journal adapted to our times, where space and performance of the same count as well.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In the changes that <span class="elsevierStyleItalic">Actas Urológicas Españolas</span> has conducted from January 2011 there is room for cases. There are two new sections that deal with it. The section ‘skill and ingenuity’ describes the experience generated by clinical cases with enough teaching to design new techniques, or to better understand the processes, as a structured article. To give some examples are entities of difficult and peculiar diagnosis or treatment,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> cases that are used to design new techniques<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> or to raise similar but different problems.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> The section ‘Casuistry’ publishes case series, with a structured summary as well, which makes it possible to better define some evidence generator aspects, because they are rare entities<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> or short series that correspond to a particular problem<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8–10</span></a> and provide valid scientific evidence.
