One of these potential complications’ lower urinary tract obstruction, which may be due to urethral stricture or more often to vesicourethral anastomotic stricture. This complication may cause urinary retention in the patient, which, although in principle can be resolved by urethral catheterization, suprapubic catheterization or dilatations, additional surgery may sometimes be required to definitively resolve the problem, with the consequential increase in the risk of postoperative incontinence.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The primary objective of this study was to determine the incidence of vesicourethral anastomotic stricture in our patients who underwent surgery for prostate cancer. Additionally we sought to find out whether the association of surgery with radiation therapy increased the risk of anastomotic stricture compared with patients treated with radical prostatectomy only.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Materials and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">We retrospectively reviewed the medical records of patients operated for prostate cancer at our hospital during the period between January 2000 and December 2008, with clinical follow-up of at least 12 months. A total of 258 patients met the foregoing requirements. Of these 33 were operated by the same surgeon using the laparoscopic approach (LRP), while the remaining 225 were operated retropubically (RRP); the operations were performed by 15 different surgeons including consultant physicians and house physicians in recent years.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The surgical site was irradiated using the three-dimensional conformal technique (3D-CRT) administered by means of a linear accelerator with 18<span class="elsevierStyleHsp" style=""></span>MeV photons in 5 fractions per week and a dose of 2.0<span class="elsevierStyleHsp" style=""></span>Gy per fraction. The characteristics of the patients treated with surgery and radiotherapy are listed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">Vesicourethral anastomotic stricture was diagnosed in most cases based on clinical suspicion and was confirmed endoscopically in all patients. Only exception was the application of preoperative radiological tests performed (retrograde and voiding urethrography) that give clinical suspicion and uretrocystoscopy thus, allowing accurate diagnosis in the majority of patients. Adverse effects were classified in accordance with the scale described in version 4.0 of the <span class="elsevierStyleItalic">Common Terminology Criteria for Adverse Events</span>.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">For the statistical analysis of differences in the incidence of anastomotic stricture in each group, we used Fisher's exact test. We analyzed the differences between the median preoperative PSA using the nonparametric Mann-Whitney test, while differences in average ages were analyzed using the “<span class="elsevierStyleItalic">t</span>” test.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">There were no significant differences (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.782) in the average age of patients treated with radical prostatectomy alone (63.5 years, range: 46–70) and those treated with prostatectomy associated with radiotherapy (64 years, range: 46–77). The median PSA before treatment was slightly higher among patients with prostate cancer treated with surgery and radiotherapy (10.9<span class="elsevierStyleHsp" style=""></span>ng/ml, range 3.4–29.9) than among those treated with surgery alone (9.5<span class="elsevierStyleHsp" style=""></span>ng/ml, range 4.0–31.2), although these differences were not significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.459).</p><p id="par0040" class="elsevierStylePara elsevierViewall">None of the patients treated had to discontinue combined radiotherapy due to acute GU toxicity, although at least 4 (16%) patients had evidenced grade 2 GI toxicity, only one case was badly tolerated, which lead to a reduction of the administered dose to 64.8<span class="elsevierStyleHsp" style=""></span>Gy.</p><p id="par0045" class="elsevierStylePara elsevierViewall">One of the irradiated patients had vesicourethral anastomotic stricture immediately after surgery prior to the initiation of radiotherapy. Having not suffered recurrence after 36 months, we included him in the irradiated group of stricture-free patients. The characteristics of the 4 patients treated with surgery and radiation therapy that subsequently developed vesicourethral anastomotic stricture is summarized in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. Stenosis appeared 1, 5, 8 and 22 months after completion of radiotherapy. In all the cases, treatment was by means of endoscopic section of the stricture. Two of the four patients suffered recurrence of the strictures and thus required further surgery. After surgical correction of the anastomotic stricture, one of the patients is continent and does not require incontinence pads; another has grade 1 incontinence and requires one pad per day, and the other two suffer grade 2 and grade 3 incontinence respectively. The latter case required the placement of an artificial urinary sphincter.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Of the remaining 233 patients treated only with radical prostatectomy, 36 (15.4%) developed vesicourethral anastomotic stricture after a mean follow-up of 55 months. The average time between surgery and the diagnosis of stenosis was 8.1 months (1–32). Within this group of patients the incidence of surgery for stenosis among those operated using the retropubic approach was 15.3%, while among those who were operated using the laparoscopic approach it was 13.3%. There was therefore no statistically significant difference between the two approaches (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.000). The final treatment of anastomotic stricture was endoscopic section; 11 patients (30.5%), presented stricture recurrence on at least one occasion. The characteristics of this group of patients are given in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Comparatively, we did not find differences in the incidence of vesicourethral anastomotic stricture in patients treated only with radical prostatectomy (15.4%) and those treated with prostatectomy and radiotherapy (16%) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.599) (<a class="elsevierStyleCrossRefs" href="#tbl0010">Tables 2 and 3</a>).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">Vesicourethral stenosis in patients with prostate cancer is a potential complication that can occur after both radical prostatectomy and radiation therapy. Its incidence after radical prostatectomy varies widely between 1% and 60%.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> Its origin is multifactorial and involves factors related to the surgeon himself (experience), to the patient (smoking, diabetes, hypertension, coronary heart disease), to the surgical technique (perineal, retropubic, laparoscopic approach) or even to events that occurred prior to the surgical intervention (TURP or preoperative radiation therapy), during surgery (intraoperative bleeding) and also after surgery (anastomotic leak, pelvic hematomas, postoperative acute urinary retention).</p><p id="par0065" class="elsevierStylePara elsevierViewall">Vesicourethral stricture has also been described as an adverse effect of radiotherapy applied to prostate cancer, both as the primary and secondary treatment after radical prostatectomy. Like postoperative anastomotic stricture, its actual incidence is difficult to know, especially if we take into account that appears in numerous publications it is included under the generic term of genitourinary toxicity, without specification of whether it consists of dysuria, hematuria, stricture or incontinence. In other cases the limitation is because it is not considered as a complication in the classification used to assess actinic GU toxicity. Similarly to what occurs with surgery, the complications of postoperative radiation therapy (including vesicourethral anastomotic stricture) may depend on several factors and may be acute or chronic. The mechanism of radiation-induced lesions on the bladder and urethra is not known with precision. In animal models, it has been found that the toxicity was dependent on the dose-volume, and that ionizing action could take place on urothelial cells, the smooth muscle, the vascular endothelium and/or stromal cells.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> One of the ultimate consequences of this process is the increase in deposits of collagen and of the extracellular matrix (radiation-induced fibrosis) that leads to a loss of elasticity (compliance) and tissue retraction.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">There are few references in literature to the factors that predispose to increase toxicity of postoperative radiotherapy, with contradictory results observed in some cases. Thus, Monti et al. observed a statistically significant correlation between dose volume and GU toxicity, whereas Nath et al.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a> found no relationship between the dose administered and subsequent GU toxicity. Regardless of the small sample sizes of these studies, 35 and 50 patients respectively, these differences may be attributable to the shorter follow-up of one of the series (22 months versus 55 months), which can be a determining factor when assessing the toxicity attributable to chronic radiation.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a> In this regard, Abdalla et al. observed that for a dose of 66<span class="elsevierStyleHsp" style=""></span>Gy, overall GU toxicity decreased from 20% during the first year to 8% in the fourth year. However, for a dose of 70<span class="elsevierStyleHsp" style=""></span>Gy, this toxicity went from 18% during the first 12 months to 46% at 4 years, although the percentage of grade 2–3 toxicity remained below 5%.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> It can therefore be said that in general, moderate to severe GU toxicity (≥grades 2–3) of postoperative radiotherapy is low and, within it, the percentage of urinary tract obstructions, including vesicourethral anastomotic stricture is still lower, although the literature on it does not give an idea of its actual incidence. <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the overall data on GU toxicity in patients with prostate cancer treated with radiotherapy after radical prostatectomy by different authors. Note that the incidence of acute grade 2 toxicity ranges from 8% to 18.2%, while chronic toxicity ranges from 2% to 16%. On the other hand, both acute and chronic ≥grade 3 toxicity ranges from 0% to 8.5%. Since these are overall figures, the incidence of anastomotic stricture is even less than the foregoing figures, without ruling out that its real significance is probably underestimated, i.e., anastomotic stricture resolved solely with dilation would be grade 2 toxicity, while if it requires surgery, including endoscopic section, it would be grade 3. It is also striking how many authors solve anastomotic stricture with dilation. This contrasts with our experience because in all our patients, we had to resort to surgery to solve the problem permanently.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Comparatively, our overall figures of anastomotic stricture, both in patients treated with or without radiation therapy, are higher than many of the series published in literature. We did not observe any significant differences between the two groups, which seem to indicate that the impact of postoperative radiotherapy in the genesis of this complication may be low. Similarly, we did not observe significant differences in the incidence of anastomotic stricture between prostatectomized patients who did not receive radiation therapy and were operated retropubically or laparoscopically. However, these results may also be influenced by differences in size between the samples.In summary, in our series, postoperative radiotherapy in prostatectomized patients does not appear to entail a significant increase in the risk of vesicourethral anastomotic stricture. It is possible that other factors unrelated to postoperative radiation contribute to the appearance of this common complication. Of the total patients, 258 met the foregoing requirements. Among them, 25 (9.6%) received postoperative radiotherapy, 12 (48%) received adjuvant radiotherapy and 13 (52%) received salvage radiotherapy. The mean age of the patients that received radiotherapy was 64 (46–77) years. The mean pre-radiotherapy PSA was 2.3 (0.04–26.1)<span class="elsevierStyleHsp" style=""></span>ng/ml.
