We agree with the author in that angle support lenses have come a long way to demonstrate that they are not reliable not even with sophisticated designs. This is supported by an important clinical study as in the case of the Alcon Cachet lens (Alcon Laboratories, Fort Worth, USA).</p><p id="par0010" class="elsevierStylePara elsevierViewall">Even though said lens possibly gave the best results as regards intraocular performance and particularly short and medium term endothelial respect, its long-term results are yet to be seen. In addition, we agree in the necessity of carrying out a protocol on all patients implanted with the Alcon Cachet lens as with all patients with other anterior chamber lenses utilized to date. We propose a systematic yearly study of corneal endothelium including, if possible, anterior chamber depth study with Visante OCT (Carl Zeiss, Jena, Germany) to determine the proximity of the lens to the endothelium and the narrowing of the chamber angle.</p><p id="par0020" class="elsevierStylePara elsevierViewall">A previous publication of our group<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">1</span></a> described the anatomical changes of the anterior chamber in the course of life<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">1</span></a> and the anatomical changes of a phakic lens implanted in a young patient in relation to the involved anatomical elements (endothelium, iris, lens and chamber angle). Said publication concluded that 10–15 years after the implant endothelial risk could be high even without evidence appearing in central endothelium studies.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Having made the point, we will now reply the questions about our work.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">2</span></a> Endothelial loss after the explant described in the article was 5%. It is likely that patient age and surgical technique explain the low rate of endothelial density loss. One case having 1500 cells/mm<span class="elsevierStyleSup">2</span> was a patient that compulsively rubbed his eyes and, when he stopped, the endothelial count was stable and therefore lens explant was no longer necessary due to the satisfactory visual condition and the evident stability of the endothelium. This patient remains in half-yearly checkups without significant changes.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Out of the 25 eyes followed up for 5 years, none of the patients exhibited counts below 1500 cells/mm<span class="elsevierStyleSup">2</span>.</p><p id="par0035" class="elsevierStylePara elsevierViewall">It is necessary to emphasize once again the importance of following-up patients implanted with the Alcon Cachet lens.
