TGF-β possibly inhibits IL-1 induced IL-6 secretion in POAG.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Glaucoma is a leading cause of irreversible blindness, characterized by retinal ganglion cell (RGC) loss and optic nerve degeneration.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">1</span></a> As a consequence, thinning of the neuroretinal rim and cupping of the optic nerve are observed in glaucoma.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">1,2</span></a> Based on the state of the iridocorneal angle, glaucoma is classified into either open-angle glaucoma (OAG) or closed-angle glaucoma (CAG), both of which can occur primarily or secondarily.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Primary open-angle glaucoma (POAG) is the most highly reported type of glaucoma clinically<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">6</span></a> and is problematic due to the presence of raised intraocular pressure (IOP), which is not found in other common OAG subtypes such as normal-tension glaucoma (NTG).<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Hence, currently available treatments for glaucoma aims to control IOP. However, most drugs attempt to lower IOP through modification of the aqueous dynamic,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">7</span></a> without necessarily treating the underlying mechanisms of high IOP. As shown by previous studies, the underlying mechanism is mostly due to trabecular meshwork (TM) outflow resistance,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">8</span></a> reportedly associated with localized extracellular matrix (ECM) deposition in the TM.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">8–10</span></a> Moreover, the administration of IOP-lowering drugs have proven to be insufficient in some patients, for whom additional incisional surgery was required to achieve the desired IOP levels.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">11</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Drugs that aim to lower IOP through antifibrotic and neuroprotective properties may have great clinical impact for patients with glaucoma. In POAG, increased ECM is closely associated with the upregulation of transforming growth factor-beta2 (TGF-β2).<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">8,12</span></a> Interleukin-6 (IL-6) has been shown to interact with TGF-β to modulate fibrosis.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">13,14</span></a> And though several studies have observed changes in aqueous humor IL-6 levels of POAG, discrepancies still exist<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">13,15,16</span></a> therefore rendering this inconclusive. Thus, this present study aims to establish the changes of IL-6 levels in the aqueous humor of POAG patients through a meta-analysis of all eligible studies.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A comprehensive literature search was conducted from PubMed, Scopus, and Web of Science. Keywords such as IL-6, aqueous humor, and glaucoma were used in combination. The literature search was updated until 30 November 2019. The selection criteria are case–control studies evaluating the aqueous humor IL-6 levels among POAG patients. Data extracted from the studies include citation information, sample size, mean and SD of age, IOP, and aqueous IL-6 level. Meta-analysis was performed using RevMan ver 5.3, Meta-Essential 1.4, and Open Meta-Analyst.</p><p id="par0030" class="elsevierStylePara elsevierViewall">A random-effect model was used to allow heterogeneity in individual studies. Hedge's standardized mean difference (SMD) with 95% confidence interval (CI) was used to assess the IL-6 level between control and POAG. Individual study and pooled effect sizes were evaluated using a forest plot. The inverse of the variance in each effect size of the individual study was used to assess the weight of studies. Heterogeneity was evaluated with <span class="elsevierStyleItalic">Q</span>-test and <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span>. Meta-regression was used to analyze the source of heterogeneity. Begg's funnel plot and Egger's regression test were used to determine publication bias. A sensitivity test was performed by sequentially omitting one study each time to evaluate the stability of the results. The value of 0.05 was indicative of statistical significance.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">From the literature search, 32 studies were potentially suitable for systematic review. Among these, 14 studies were selected based on inclusion criteria, while the rest were excluded on the basis of varying sampling method (3 studies utilized serum and tears as samples), use of non-POAG subjects (4 studies with acute primary angle closure (APAC) glaucoma, 2 studies with NTG, and 3 studies with secondary glaucoma), and data that did not fit into data analysis (2 studies did not provide the value of IL-6 levels, 3 studies evaluating efficacy of treatment in glaucoma, and 1 study utilizing method other than ELISA or multiplex bead immunoassay). The characteristics of the included studies are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Three of fourteen studies (indicated with an asterisk in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>) were included for analysis with estimated mean and SD obtained using a previously described equation utilizing the median, range, and sample size.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">28</span></a> All studies used cataract patients without other ophthalmic problems as control. Studies included in this meta-analysis measured IL-6 level with ELISA or multiplex bead immunoassay.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The forest plot for pooled SMD and 95% CI based on the detection method was shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. Pooled SMD results showed no significant change in IL-6 levels (SMD<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.07; 95% CI: −0.73 to 0.59; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.83), with high heterogeneity among studies (<span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>94%, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.00001). Because multiplex bead immunoassay is more sensitive than conventional ELISA in detecting antibodies,<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">29</span></a> we then analyzed pooled SMD from multiplex bead immunoassay. In contrast, pooled SMD from multiplex bead immunoassay showed a significantly lower IL-6 level in POAG compared to control (SMD<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.40; 95% CI: −0.70 to 0.09; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01) and homogenous than ELISA method (multiplex bead immunoassay, <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>39%, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.15; ELISA, <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>98%, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.00001; estimated IL-6 levels based on multiplex bead immunoassay, <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>53%, <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.12). Begg's funnel plot (<a class="elsevierStyleCrossRef" href="#sec0035">Supplemental Fig. S1</a>) and Egger's test were applied for all pooled studies and no publication bias was observed (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleInf">Egger's test</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.05). Subgroup analysis was not followed by funnel and Egger's test because the pooled studies less than 10 may not able to distinguish publication bias.<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">30</span></a> Further analysis using meta-regression indicated that there is no relation between the level of IL-6 with 3 continuous variables, which were age, year of publication, and sampling size (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), Thus, heterogeneity observed in this study is likely because each individual study showed an opposing result in each other or estimated mean and SD may not represent the right value to perform a meta-analysis. A sensitivity analysis was conducted for all pooled studies (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A) and pooled studies from multiplex bead immunoassay (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>B) by eliminating one individual study each time. However, the associations detected in pooled analyses remained unchanged, suggesting that the findings were statistically stable and robust.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0045" class="elsevierStylePara elsevierViewall">In determining the role that IL-6 plays in the pathogenesis of POAG, this study provides a robust summary of the evidence surrounding aqueous humor IL-6 levels in POAG patients and controls. When studies utilizing both ELISA and multiplex bead immunoassay were included in the analysis, the results indicated that aqueous humor IL-6 levels did not change in POAG. However, this may be due to the low sensitivity and reproducibility of ELISA compared to multiple bead immunoassay, as concluded from previous reports on the vast superiority of multiplex bead immunoassay compared to conventional ELISA method for detecting specific antibodies.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">29</span></a> Because the inherent difference in assay performance may have resulted in false-negative outcome and heterogeneity of pooled studies, additional analysis exclusively toward studies utilizing multiple bead immunoassays showed results consistent of a reduced IL-6 level in POAG.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Several groups have reported a significant decrease of IL-6 level in POAG.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">15,18,21</span></a> In contrast, a report of stimulated IL-6 secretion was observed in TM exposed toward TGF-β2,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">13</span></a> which indicates that complex mechanisms and multiple factors may affect the level of IL-6 within the eye. Both TGF-β and IL-6 are known as factors that regulate fibrosis through differential pathways (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). Through canonical and non-canonical pathways, TGF-β stimulates its downstream target, Smad2/3 and Janus kinase (JAK1)/signal transducer and activator of transcription 3 (Stat3), respectively, to result in the upregulation of epithelial-to-mesenchymal transition (EMT) markers such as type I collagen (Col-I) and a-smooth muscle actin (α-SMA). These markers are responsible for the progression of fibrotic changes in the TM.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">13,31–33</span></a></p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The signal transduction of IL-6 is mediated by classical (through membrane IL-6 receptor (mIL-6R)) and trans-signaling (through soluble IL-6 receptor (sIL-6R)) mechanisms. As a proinflammatory cytokine, IL-6 activates JAK1/Stat3 through sIL-6R, leading to the upregulation of Col-I mRNA transcript in dermal fibroblasts.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">34</span></a> Contrastingly however, IL-6/sIL-6R may also elicit inhibitory effects toward TGF-β2 induced p-Smad2 and α-SMA protein expression by downregulating the TGF-β1/2 receptor (TGF-β1/2R) in TM cells.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">13</span></a> Interestingly, negative regulation of IL-6 signaling in intestinal epithelial cells was modulated by TGF-β.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">35</span></a> On the other hand, the administration of IL-6 stimulates TGF-β1 expression in IL-6-deficient mice,<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">14</span></a> implying a cross-talk between IL-6 and TGF-β.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Since TGF-β2 upregulation increases sIL-6R in POAG,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">13</span></a> the interaction between IL-6/sIL-6R may inhibit TGF-β2 induced fibrotic changes in POAG. Despite this expectation, this study found that IL-6 levels were lower in POAG, when detected using bead multiplex immunoassay studies. This therefore implicates that imbalance in trans-signaling IL-6 may not effectively inhibit both canonical and non-canonical pathways of TGF-β2 signaling, which then results in the accumulation of ECM in TM. A possible explanation for this decrease has been shown in monocyte culture, wherein TGF-β was capable of inhibiting IL-1 induced IL-6 expression.<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">37</span></a> Therefore, the reduction of IL-6 in this case is possibly due to low stimulation from IL-1, as shown in another meta-analysis suggesting a tendency for IL-1 to decrease in POAG (<a class="elsevierStyleCrossRef" href="#sec0035">Supplemental Fig. S2</a>).</p><p id="par0065" class="elsevierStylePara elsevierViewall">In conclusion, based on the sensitivity and robustness of bead multiplex immunoassay in detecting IL-6, this study clearly demonstrated that IL-6 levels are significantly lower in POAG eyes than in controls. However, further studies are needed to verify this conclusion. A meta-analysis was performed to determine the overall change in IL-6 of GPAA patients compared a controls.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 14 studies were selected. Analysis combining the results of studies using both detection approaches did not show any changes in the level of IL-6 in GPAA (SMD<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.07; 95% CI: −0.73 to 0.59; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.83), possibly due to the heterogeneity of ELISA data. Since multiplex bead immunoassay is more sensitive than conventional ELISA in detecting antibodies, further isolated analysis of multiplex bead immunoassay results revealed that GPAA patients had a lower level of IL-6 in the aqueous humor compared a controls (SMD<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.40, 95% CI: −0.70 to 0.09; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.01).
