LE autofluorescent retinography.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Optic disc drusen have a prevalence of 0.4% in children,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> occur more frequently in females and Caucasians and could be hereditary. They consist in acellular, intracellular and extracellular deposits that frequently calcify with time<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> although the exact physiopathogeny of their formation remains unknown.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In general, drusen evolve slowly and in many cases need years to develop fully. Drusen can be deep in the first decade and become superficial and therefore visible during the second decade of life.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Supplementary tests supporting diagnosis comprise fluorescein angiography, autofluorescence retinography, axial and coronal tomography of cranium and orbit, spectral domain optical coherence tomography, and enhanced depth optical coherence tomography of the optic disc and the retinal nerve fiber layer. Mode B echography (gold standard) supplements the final diagnosis, showing drusen as a hyperechogenic dotted image with posterior acoustic shadow, even in the hidden forms.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1,3–5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The complications of optic disc drusen in children include visual field defects (51%), vascular complications such as subretinal, retinal and vitreous peripapillary hemorrhages (2–13%)<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1,6</span></a> choroidal neovascular membrane to a lesser extent (<1%)<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a><span class="elsevierStyleSup">,</span> anterior non-artheritic ischemic optic neuropathy and retinal vascular occlusions. However, the majority of cases remain asymptomatic and can be managed by means of regular clinic observation and supplementary explorations.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Clinic case report</span><p id="par0025" class="elsevierStylePara elsevierViewall">Female patient, 14, with hypermetropia and astigmatism history, who visits the Emergency Dept. in January 2019 referring diminished vision in the left eye (LE) with approximately 3 months evolution.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The patient exhibited best corrected visual acuity in the right eye (RE) of 20/20 (sph<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3.00<span class="elsevierStyleHsp" style=""></span>cyl<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>0.75<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>75°); in the LE 20/200 (sph<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3.75<span class="elsevierStyleHsp" style=""></span>cyl<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>1.50<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>90°), intraocular pressure of 10<span class="elsevierStyleHsp" style=""></span>mmHg in both eyes, preserved ocular movements. Ocular fundus examination in the RE revealed an elevated papilla with blurry edges and without physiological cup, macula with normal appearance, while the LE showed over-elevated papilla with blurry edges, without physiological cup, temporal juxtapapillary hemorrhage and retinal folds with macular neuroepithelium detachment compromising the fovea (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>a and b).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">Mode B echography was taken, evidencing a hyper-ecogenic dotted image in both papillas, which enabled diagnostic confirmation (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>c and d).</p><p id="par0040" class="elsevierStylePara elsevierViewall">Optical coherence tomography (OCT-SD Cirrus, Carl Zeiss Meditec Inc. Dublin, California, USA) showed over-elevated papillas and fiber layer thickening in both eyes. In addition, the LE showed the extension of the juxtapapillary neovascular membrane with neuroepithelium detachment, compromising the fovea (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>e and <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>b and c).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Autofluorescence did not show the typical hyper-autofluorescent image of drusen in papillas because they were deep and therefore not visible (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>f).</p><p id="par0050" class="elsevierStylePara elsevierViewall">Computerized visual field study (Haag Streit Interzeag Octopus 123. Schlieren, Switzerland; program G1, TOP strategy) of the RE showed preserved sensitivity (MD 2.4; LV 7.1; RF 0.0), whereas the LE evidenced a centrocecal scotoma (MD 9.8; LV 98.9; RF 0.0) (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">Due to visual acuity compromise caused by the presence of the neovascular membrane, the application of intravitreal ranibizumab (Lucentis®, Novartis Pharma, Stein AG-Stein, Switzerland) was indicated at a dosage of 0.5<span class="elsevierStyleHsp" style=""></span>mg, 3 per month, after obtaining the informed consent by the patient's legal tutors.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The checkup conducted at 3 months was positive, with visual acuity improvement to 20/20 in LE and disappearance of the neuroepithelium detachment as well as thinning of the neovascular membrane (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>d–f). The situation remained stable in subsequent checkups at 6 and 9 months.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Papillary drusen are usually asymptomatic and follow a benign course, generally enabling an expectant approach and follow-up. In the presence of a papillary edema in children, the ophthalmologist is the specialist who should manage these patients. Drusen diagnostic is difficult and could be confused with other causes, sometimes leading to unnecessary and invasive studies.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In the present case, mode B echography was essential for the diagnostic in addition to being a practical and unexpensive method.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Even though infrequent, papillary drusen could exhibit complications including visual field alterations, hemorrhages and vascular choroidal membranes, each requiring different treatments.</p><p id="par0085" class="elsevierStylePara elsevierViewall">In the absence of an established therapeutic approach, references include the description of intravitreal antiangiogenics (anti-VEFG) to treat the development of vascular choroidal membranes associated to drusen.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">5,6,8,9</span></a> Treatments described prior to the application of anti-VEFG include laser photocoagulation and photodynamic therapy among other options.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Alkin et al.,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> Delas et al.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a> and Baillif et al.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> carried out treatment with ranibizumab and between 1 and 2 doses were required, without finding recurrences in the follow-up period.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Knape et al. treated with bevacizumab, with 2 doses in combination with laser treatment.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">In the present case, 3 doses of ranibizumab were applied at 1-month intervals, observing anatomical and visual acuity improvements after treatment and the absence of complications or re-emergence of the condition in the course of the 9-month follow-up.
