A 75-year-old female patient consulting for post-prandial epigastric pain associated with 5kg loss of weight in a year. An angio-tomography was performed, which showed proximal stenosis compatible with compression of the celiac trunk, suggestive of a median arcuate ligament syndrome (Fig. 1). An exploratory laparoscopy was performed with dissection of the vascular elements of the celiac trunk. The hepatic, splenic and left gastric arteries were identified and released until there onset (Fig. 2A and B). We recognized the celiac trunk of 2cm length. Release of the arcuate ligament and decompression of the celiac trunk was observed (Fig. 2C and D). The patient evolved favorably, with adequate oral intake and pain management.
