¿Debe ser la hernioplastia la técnica de elección en el tratamiento de la hernia umbilical del adulto?

Introducción

Los excelentes resultados de la utilización de materiales protésicos en la enfermedad herniaria de la región inguinal nos obligan a valorar su empleo en la hernia umbilical.

Material y métodos

Revisamos 213 hernioplastias umbilicales operadas en la Unidad de Cirugía sin Ingreso del Hospital de Elche entre junio de 1992 y enero de 1998. Fueron criterios de exclusión la obesidad mórbida, las hernias gigantes y los pacientes con criterios ASA IV. Como técnica quirúrgica se realizó al 100% de los pacientes una hernioplastia con plug si el anillo herniario era inferior a 3 cm, o con malla si era igual o superior a 3 cm. El seguimiento medio ha sido de 64 meses.

Resultados

Características de la población: edad media 57,1 años; 123 mujeres y 90 varones; 188 ASA I-II; anillo herniario superior a 3 cm en 70 pacientes, e inferior a 3 cm en 143. Técnica anestésica: local más sedación en el 91,5%. Complicaciones: 12 seromas (5,6%), 5 hematomas (2,3%), 3 infecciones de la herida (1,4%), 2 rechazos a la prótesis (0,95%) que provocaron una recidiva herniaria.

Conclusiones

La hernioplastia umbilical ofrece índices de morbimortalidad muy bajos y tasas de recidivas menores a las técnicas herniorráficas clásicas, por lo que debe ser considerada como técnica de elección.

Introduction

Because of the outstanding results of prosthetic repair in the treatment of inguinal hernia, we evaluated the use of this technique in umbilical hernia.

Material and methods

We reviewed 213 patients who underwent surgery for umbilical hernia at the Day Surgery Unit of the Hospital del Elche between June 1992 and January 1998. Exclusion criteria included morbid obesity, large umbilical defects and ASA IV status. A polypropylene plug was used in defects < 3 cm and polypropylene mesh was used in defects ≥ 3 cm. Mean follow-up was 64 months.

Results

Mean age was 57.1 years. There were 123 women and 90 men. One hundred eighty-eight patients were classified as ASA stage I-II. Hernia defect was < 3 cm in 143 patients and > 3 cm in 70. Local anesthesia plus sedation were used in 91.5% of the patients. Complications included 12 seromas (5.6%), 5 hematomas (2.3%) and 3 wound infections (1.4%).Two patients (0.5%) showed intolerance to prosthesis, causing recurrence.

Conclusions

Prosthetic umbilical repair produces very low morbidity and mortality. This technique produces fewer recurrences than the classical approach is used. Consequently, umbilical hernioplasty should be the technique of choice.
