Resumen
Introducción

El seguimiento y manejo de la necrosis pancreática infectada y drenada puede hacerse de forma conservadora con el control clínico, TAC seriadas y reintervenciones programadas, o bien empleando periódicamente la endoscopia retroperitoneal translumbar

Método

Mediante un abordaje translumbar extraperitoneal del retroperitoneo se drena la necrosis pancreática infectada colocando un sistema de lavado y drenaje. Tras retirar el tubo de drenaje y a través del túnel creado introducimos, bajo visión directa, un endoscopio flexible en el espacio retroperitoneal para eliminar con lavados-arrastre y aspiración el material necrótico infectado

Resultados

En los tres pacientes estudiados hemos obtenido buenos resultados en cuanto al control y eliminación de la necrosis pancreática infectada, sin morbimortalidad en relación con la técnica

Conclusiones

La endoscopia retroperitoneal permite explorar bajo visión directa el espacio retroperitoneal en las necrosis pancreáticas, facilitando su lavado y aspiración, sin añadir morbimortalidad. Aunque hace falta un mayor número de casos, podría disminuir las reintervenciones quirúrgicas por desbridamiento incompleto, así como los estudios de imagen para el control evolutivo de estos pacientes

Introduction

The follow-up and management of drained pancreatic necrosis can be performed conservatively with clinical control, serial computed tomography and scheduled reoperations, or by periodic translumbar retroperitoneal endoscopy

Method

Infected pancreatic necrosis was drained and a lavage and drainage system was stabilized via an extraperitoneal translumbar approach to the retroperitoneum. Through the tunnel created after removing the drainage tube, a flexible endoscope was inserted into the retroperitoneal space, under direct observation, to eliminate the infected necrotic material with washouts and aspiration

Results

The results in the three patients studied were good in terms of control and elimination of the infected pancreatic necrosis and there was no technique-related morbidity or mortality

Conclusions

With retroperitoneal endoscopy, the retroperitoneal space can be explored under direct observation in patients with pancreatic necrosis, thus facilitating lavage and aspiration without increasing morbidity and mortality. Although studies with a greater number of patients are required, this technique could reduce the number of surgical reinterventions for incomplete debridement as well as the number of imaging tests required during follow-up
