Resumen
Objetivo

Analizar de forma crítica las complicaciones a largo plazo y los resultados funcionales de una serie de pacientes con reservorio ileoanal para proporcionar a los candidatos a proctocolectomía total una información detallada y objetiva sobre las ventajas e inconvenientes de dicha técnica.

Pacientes y métodos

Se han estudiado los datos clínicos y patológicos de 141 pacientes a los que se realizó un reservorio ileoanal entre 1985 y 2000. Se han analizado las complicaciones inmediatas y a largo plazo, así como los resultados funcionales y el grado de satisfacción de los pacientes mediante entrevista directa o telefónica.

Resultados

La morbilidad postoperatoria fue del 36%, y requirió reintervención el 13,4% de los pacientes. Las complicaciones a largo plazo más frecuentes fueron la reservoritis (18%) y las fístulas perianales o reservoriovaginales (13%). Ambas se asocian con un nivel de satisfacción medio-bajo (p < 0,004). Los resultados funcionales, según criterios de Öresland, son buenos en la mayor parte de los pacientes.

Conclusiones

Los reservorios ileoanales se acompañan de una morbilidad moderada y buenos resultados funcionales, que los candidatos a una proctocolectomía total deben conocer para poder tomar una decisión conjunta acerca de la técnica a realizar en cada caso.

The aim of this study was to critically analyze the long-term complications and functional results in a series of patients with ileoanal reservoir in order to provide candidates for total proctocolectomy with detailed and objective information on the advantages and disadvantages of this technique.

Patients and methods

Clinical and pathological data from 141 patients undergoing ileoanal reservoir between 1985 and 2000 were studied. The immediate and long-term complications were analyzed and the functional results and degree of satisfaction were evaluated through face-to-face or telephone interview.

Results

Postoperative morbidity was 36% and 13.4% of patients required reintervention. The most frequent long-term complications were inflammation of the reservoir (18%) and perianal or reservoir-vaginal fistulas (13%). Both were associated with a lowmedium degree of satisfaction (p < 0.004). In most patients, the functional results, according to the criteria of Öresland, were good.

Conclusions

Ileoanal reservoirs show moderate morbidity and good functional results. Candidates for total proctocolectomy should be aware of these results to be able to take a joint decision on the technique to be used in each case.
