An 83-year-old male patient with no history of interest consulted due to recent-onset proptosis and diplopia in the left eye. A cranial tomography scan (Fig. 1) and magnetic resonance imaging (Fig. 2) showed a heterogeneous mass in the left lacrimal gland with bone invasion of the superior lateral orbital rim. A biopsy was taken from the lacrimal lesion, which showed signs compatible with a metastatic lesion of intestinal origin. Given the findings, an extension study was performed, which detected advanced superior rectal adenocarcinoma (T3N2bM1c), in association with liver and bone metastases. Due to the advanced nature of the disease, the patient was treated with palliative orbital radiotherapy and palliative chemotherapy.
