Resumen

Se estudia el significado pronóstico de los patrones radiológicos de las lesiones no palpables de la mama. Se analiza de forma retrospectiva un grupo de 263 pacientes clínicamente asintomáticas,diagnosticadas mediante mamografía de lesión radiológicamente sospechosa. Se describen cuatro patrones radiológicos y se relacionan con el resultado de la biopsia.

La incidencia de cáncer en los patrones cálcico,nodular,nodular estrellado y distorsionante fue de 36/132 (27,3%),6/41 (14,6%),33/48 (68,7%) y 4/15 (26,7%),respectivamente. La incidencia en el grupo de mujeres premenopáusicas y posmenopáusicas de estos patrones fue la siguiente:nodular,del 7% frente al 22%; nodular estrellado,del 13% frente al 22%; cálcico,del 66% frente al 41%,y distorsionante,del 7% frente al 5%.

El patrón radiológico de peor pronóstico fue el nodular estrellado,seguido del cálcico. Tanto en las mujeres premenopáusicas como en las posmenopáusicas los patrones más frecuentes fueron el cálcico,seguido del nodular estrellado.

Abstract

We study the prognostic significance of different radiological patterns in non-palpable breast lesions.

We make a retrospective analysis of a group of 263 asymptomatic patients with suspicious radiological non-palpable breast lesions. We consider four radiological patterns, and their relationship with biopsy results.

The incidence of cancer in the microcalcifications, nodular mass, spiculated mass, and distortion patterns were: 36/132 (27.3%), 6/41 (14.6%), 33/48 (68.7%), and 4/15 (26.7%), respectively. The incidence of these patterns in the pre and post menopausal women groups were: nodular mass 7% vs 22%, spiculated mass 13% vs 22%, microcalcifications 66% vs 41%, and distortion 7% vs 5%.

The pattern with the worst prognosis was the spiculated mass followed by microcalcifications. In both pre and post menopausal women the most frequent pattern was microcalcifications followed by spiculated mass.
