in 1961 stated that LBW &#40;low birth weight infants&#41; were all newborns weighing less than 2500<span class="elsevierStyleHsp" style=""></span>g&#46; Based on the period of pregnancy&#44; low birth weight babies &#40;LBW&#41; can be distinguished from LBW according to the pregnancy period &#40;SMK&#41; and small LBW pregnancy period &#40;KMK&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> Until now&#44; babies with LBW are still a problem in the world because they are the cause of morbidity and mortality of newborns&#46;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">LBW is a major factor in increasing mortality&#44; morbidity and disability of neonates&#44; infants and children and has a long-term impact on their lives in the future&#46; The incidence in Indonesia varies greatly from one region to another&#44; which ranges from 9&#37; to 30&#37;&#46;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> The 5-year neonatal mortality rate has stagnated&#46; Nationally based on 2007 and 2012 IDHS results&#44; it is estimated that 19 per 1000 live births&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Based on reports from the District Health Office&#47;City that the number of infant deaths from year to year has increased&#46; In 2010 the number of infant deaths was 854 babies or 5&#46;8 per 1000 live births&#44; in 2011 the number of infant deaths increased to 868 babies or 5&#46;90 per 1000 live births&#44; while in 2014 there was a rapid increase in infant deaths to 1056 babies or 7&#46;23 per 1000 live births&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In Indonesia&#39;s health profile&#44; the most common cause of infant death is due to slow fetal growth&#44; fetal malnutrition&#44; premature birth and low birth weight babies &#40;LBW&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Makassar LBW incidence rate in 2012 the number of reported LBW increased to 3&#46;12&#37; of live births&#44; in 2013 the percentage of LBW decreased to 2&#46;94&#37; of live births&#44; while in 2014 the percentage of LBW again increased to 3&#46;02&#37; of live births&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Based on data obtained from the IA Fatimah RSKD Makassar City&#44; the number of LBW events in 2015 was 529 cases&#44; in 2016 there were 363 cases and in 2017 to September there were 206 cases&#46; The average occurrence of LBW per month in 2017 to September the average incidence of LBW per month is 23 cases&#46; Although every year the number of LBW events decreases&#44; but the number is still high&#44; the cases decreased&#44; where in 2015 there were 44 cases&#44; in 2016 there were 30 cases and in its occurrence&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Factors that can affect fetal growth are genetic and environmental factors&#44; among others&#44; nutrition of pregnant women&#44; the influence of drugs and stress factors experienced by mothers during pregnancy are able to affect fetal growth and development&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> From previous studies&#44; several researchers found that the average levels of bilirubin&#44; vitamins C&#44; E&#44; catalase and GPX in the AGA group &#40;38&#8211;42 weeks&#8217; gestation with a weight &#62;2500<span class="elsevierStyleHsp" style=""></span>g&#41; were significantly higher than the SGA group &#40;38 gestational age&#41; &#8211; 42 weeks with LBW infants &#40;weight &#60;2500<span class="elsevierStyleHsp" style=""></span>g&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Other studies found that levels of vitamins A and E&#44; superoxide dismutase and catalase were significantly lower in LBW while glutathione peroxidase was significantly higher in LBW than controls&#44; with the lowest levels found in infants who showed more severe growth restriction &#40;&#60;2000<span class="elsevierStyleHsp" style=""></span>g&#41;&#46; This research shows that LBW lacks a major antioxidant enzyme that acts as the first line of defense against free radicals and reactive oxygen species&#46; SOD efficiently removes superoxide radicals by converting it to H<span class="elsevierStyleInf">2</span>O<span class="elsevierStyleInf">2</span>&#44; which is then metabolized by GPx and catalase &#40;CAT&#41; into oxygen and water molecules&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Catalase enzyme as an endogenous antioxidant is a hemotetramer compound with Fe as a cofactor encoded by chromosome 11 genes&#46; This enzyme can be found in blood&#44; spinal cord&#44; mucous membranes&#44; kidneys&#44; and liver&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> Catalase enzymes are common antioxidant enzymes whose function is to catalyze the decomposition of hydrogen peroxide into water and oxygen&#46; Where the effect of gestational age and birth weight on the level of serum catalase activity as marking of antioxidants in neonates&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Seen from the previous research sites&#44; none research on enzymatic catalase levels for LBW occurrence in Indonesia&#46; While LBW incidence in Indonesia&#44; especially in Makassar increased every year&#46; Based on the background above&#44; the researchers are interested in conducting research on differences in serum levels of enzymatic antioxidant catalase in LBW placental blood and normal-weight babies at Siti Fatimah Makassar Hospital and Jumpandang Baru Health Center in Makassar&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Research location and design</span><p id="par0045" class="elsevierStylePara elsevierViewall">This research was carried out from February to April 2018&#46; Location was at the Working Area of Rumah Sakit Bersalin Ibu dan Anak Siti Fatimah Makassar and Jumpandang Baru Health Center&#46; This study deployed a cross sectional study design&#46; In this study&#44; 2 groups were used&#44; namely the LBW group and normal birth weight babies&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Population and sample</span><p id="par0050" class="elsevierStylePara elsevierViewall">The population in this study were all LBW and normal birth weight that fulfilled the inclusion criteria at the Working Area of Rumah Sakit Bersalin Ibu dan Anak Siti Fatimah Makassar and Jumpandang Baru Health Center&#46; The number of samples were 40 people with purposive sampling technique that met the inclusion criteria&#44; namely normal birth weight babies&#44; babies with LBW&#44; LBW who had perinatal asphyxia&#44; respiratory disorders&#44; infections&#44; hemolytic diseases&#44; congenital malformations&#44; and willingly mothers who as respondents&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Technique of collecting data</span><p id="par0055" class="elsevierStylePara elsevierViewall">Researchers visited Siti Fatimah Makassar Hospital and Jumpandang Baru Health Center and recorded all infants of normal birth weight and LBW until met the samples with inclusion criteria set by the researchers&#46; Measurement of serum catalase levels using the Human Catalase Elisa Kit using Elisa&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Data analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">In this study the data distribution was not so that the subsequent analysis test used the Mann&#8211;Whitney <span class="elsevierStyleItalic">U</span> test statistic&#44; the Chi-Square test&#44; the Fisher&#39;s Exact test&#44; and the Kruskal&#8211;Wallis test were then processed using SPSS for Windows 24&#46;</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Result</span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Characteristics of the sample</span><p id="par0065" class="elsevierStylePara elsevierViewall">Statistical test results in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> with the Chi-Square test&#44; Fisher&#39;s Exact test&#44; and Kruskal&#8211;Wallis test on each respondent&#39;s characteristics showed a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>0&#46;05 which means there were no significant differences in terms of age&#44; parity&#44; upper arm circumference &#40;MUAC&#41;&#44; gestational age&#44; ANC visit&#44; maternal disease&#44; and history of LBW in the LBW group and normal weight birth babies&#46; While at the level of education shows a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#46;05 which means there is a significant difference in the LBW group and babies of normal birth weight&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Distribution of differences in mean serum catalase levels in LBW and normal birth weight babies</span><p id="par0070" class="elsevierStylePara elsevierViewall">Based on <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shown that of 40 respondents &#40;100&#37;&#41; the mean serum CAT level in the LBW group was 81&#46;68<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>15&#46;22<span class="elsevierStyleHsp" style=""></span>KU&#47;L while the mean serum CAT level in the group of normal-weight infants was 92&#46;02<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>14&#46;51<span class="elsevierStyleHsp" style=""></span>KU&#47;L&#46; Statistical test results with the Mann&#8211;Whitney test showed a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;035 &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#46;05&#41;&#44; which mean there was a significant difference between serum catalase levels in LBW and normal-weight babies&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Distribution of differences in mean serum catalase levels based on age&#44; parity&#44; and upper arm circumference &#40;MUAC&#41; in the LBW group and normal birth weight babies</span><p id="par0075" class="elsevierStylePara elsevierViewall">Based on <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the mean differences in serum CAT levels in LBW and normal birth weight babies &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#46;05&#41;&#44; i&#46;e&#46; only in the high risk MUOS group &#40;&#60;23&#46;5<span class="elsevierStyleHsp" style=""></span>cm&#41; with <span class="elsevierStyleItalic">p</span> values<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;034 while the mean differences in serum CAT levels on LBW and normal BB babies who were not significant &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#46;05&#41; in the high risk maternal age group &#40;&#60;20 or &#62;35 years&#41; with <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;077 and risk &#40;20&#8211;35 years&#41; with <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;084&#44; group high risk parity &#40;&#60;2 times and &#8805;5 deliveries&#41; with <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;345 and risk &#40;2&#8211;4 deliveries&#41; with <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;065&#44; and LILA risk &#40;&#8805;23&#46;5&#41; with <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;234&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">This study found that differences in serum CAT levels in the LBW group and normal birth weight babies showed a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>0&#46;05 &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;035&#41;&#44; meaning there were significant differences in the two groups&#46; Differences in serum CAT levels were lower in LBW compared to babies of normal birth weight&#46; This supported the hypothesis in this study that a decreasing in serum CAT levels caused the free radicals formed was not completely captured&#44; so that H<span class="elsevierStyleInf">2</span>O<span class="elsevierStyleInf">2</span> was formed more then converted to hydroxyl radicals that enabled DNA and cell damage&#46; If the body is unable to fight free radicals&#44; it initiated oxidative stress which impacted on extensive tissue damage&#44; caused abnormalities in the anatomy of the placenta&#46;</p><p id="par0085" class="elsevierStylePara elsevierViewall">In these abnormalities&#44; the diameter of the spiral arteries was smaller so that resistance to blood flow decreases and ultimately leads to insufficiency and ischemia&#46; As a result of this process&#44; there was a decrease in the function of the placenta that results in nutritional deficiencies and oxygen supply that caused LBW&#46; In the control group &#40;normal birth weight babies&#41;&#44; existing serum CAT levels enabled to capture the formed free radicals&#44; so that excessive oxidative stress did not occur during pregnancy so it did not affect fetal growth and development&#46; Thus researchers assumed that there were the differences between serum levels of enzymatic catalytic antioxidants in LBW and normal-weight babies&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">Previous studies found that catalase levels were significantly lower in LBW compared to controls &#40;normal birth weight babies&#41; with the lowest levels found in infants that showed more severe growth restriction &#40;&#60;2000<span class="elsevierStyleHsp" style=""></span>g&#41;&#46; This research shown that LBW lacks a major antioxidant enzyme that acts as the first line of defense against free radicals and reactive oxygen species&#46; SOD efficiently removed superoxide radicals by converting it to H<span class="elsevierStyleInf">2</span>O<span class="elsevierStyleInf">2</span>&#44; which then metabolized by GPx and catalase &#40;CAT&#41; into oxygen and water molecules&#46; With reduced antioxidant enzyme activity&#44; the body was unable to remove superoxide radicals and H<span class="elsevierStyleInf">2</span>O<span class="elsevierStyleInf">2</span> that able to attack all biological molecules with wider tissue damage&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The results based on age&#44; mean serum catalase levels &#40;CAT&#41; showed no significant difference in the age of high risk &#40;&#60;20 or &#62;35 years&#41; and age at risk &#40;20&#8211;35 years&#41; in the LBW group and normal-weight babies&#46; The results of this study differ from existing theories that mothers with age &#60;20 years tend to give birth to smaller babies because they were still in a period of growth and development&#44; so mothers and babies competed with one another for nutrition&#46; Nutrition is very important to determine the maternal antioxidant micronutrient requirements needed to improve fetal survival in malnourished populations or in populations at high risk of micronutrient deficiencies and birth weight&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">Even though they have more than 35 years of age&#44; their body condition and health had started to decline so that it was able to affect intra-uterine fetal growth&#46; At this age&#44; it was very susceptible to free radicals so that it had impact on the damage to fat molecules&#44; proteins&#44; and DNA&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a> Due to the low CAT antioxidant levels which was unable to catalyze H<span class="elsevierStyleInf">2</span>O<span class="elsevierStyleInf">2</span> into oxygen and water molecules so that caused oxidative stress impaired growth in pregnancy&#46; This was likely to cause IUGR which results in LBW infants&#46; Naturally the body was able to produce its own antioxidants&#44; but this ability has its limits&#46; The body&#39;s ability to produce natural antioxidants diminish with age&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">9</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Based on parity&#44; there was no mean difference in high risk &#40;&#60;2 births and &#8805;5 deliveries&#41; and risk &#40;2&#8211;4 deliveries&#41; in the LBW group and normal birth weight babies&#46; The different from the existing theory&#44; where the parity factor is often associated with LBW&#46; Mothers with low parity &#40;&#60;2 births&#41; tend to experience complications&#44; namely infants born immature or complications as the first experience of the reproductive capacity of the mother&#46; Whereas high parity &#40;&#8805;5 deliveries&#41;&#44; LBW occurs because the mother&#39;s reproductive system has experienced depletion due to frequent childbirth&#46; High parity status increase the risk of LBW and stillbirth&#44; due to the higher parity status&#44; the ability of the uterus to provide nutrition for the next pregnancy decreased so that the distribution of nutrients between the mother and fetus was disrupted which ultimately lead to LBW&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">Based on LILA&#44; mothers showed a high risk &#40;&#60;23&#46;5&#41;&#46; There were significant differences in the LBW group and normal-weight babies while those at risk &#40;&#8805;23&#46;5&#41; had no significant differences in the two groups&#46; From the results of this study it was found that poor nutritional status cause decreased levels of the antioxidant enzyme catalase in pregnancy which risk on LBW&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">According to researchers&#44; maternal nutritional status as measured through LILA reflects nutrient reserves and maternal nutritional status&#46; If during pregnancy&#44; the mother has KEK&#44; it will cause anemia&#46; If anemia occurs during pregnancy&#44; iron &#40;Fe&#41; in the body reduced the formation of hemoglobin&#44; while Fe is a cofactor encoded by the enzyme catalase&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> In a study proving that the mechanism of reducing hemoglobin has reduced catalase activity&#46; Whereas&#44; the enzyme catalase is a metaloenzim whose activity is highly dependent on the presence of metal ions namely Fe &#40;iron&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">9</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Adequate nutritional status to provide the physiological needs of pregnancy&#44; namely hormonal changes and increased blood volume for fetal growth&#44; if the supply of nutrients to the fetus decreases as a result of fetal growth and development hampered and born with a low weight&#46; Nutritional deficiencies cause reactive oxygen species &#40;ROS&#41; to be produced continuously in cells&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a> So if catalase levels in pregnant women are low&#44; it causes LBW because the body is unable to fight free radicals&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">Statistical test results using the Chi-Square test&#44; Fisher&#39;s Exact test&#44; and Kruskal&#8211;Wallis test on each respondent&#39;s characteristics showed homogeneous data on maternal age and parity characteristics in the LBW group and normal birth weight babies&#46; This is in line with the study of Grujic that the group of pregnant women included in their study is homogeneous in terms of age&#44; weight&#44; height and gestational age&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a></p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusion</span><p id="par0130" class="elsevierStylePara elsevierViewall">Based on the results of the study found that there are significant differences in serum catalase levels in LBW and normal birth weight babies&#46; With reference to this research&#44; it is expected that in the future a larger scale sample research will be carried out for a diagnostic test research&#44; further research is still needed because in this study due to it is still rarely done especially in Indonesia&#44; it is expected to enrich the characteristics of respondents not only as signs for LBW occurrence but can be known risk factors for LBW&#44; and hopefully it can be a sign for LBW occurrence so that prevention of LBW can be done such as giving antioxidant supplements &#40;Fe&#41;
