The number of adolescent boys was 13.07%, and the number of adolescent girls was 12.56%.<a class="elsevierStyleCrossRef" href="#bib0005">1</a> The number of teenagers in Yogyakarta in 2012 was 15.15% of the population.<a class="elsevierStyleCrossRef" href="#bib0010">2</a> Adolescents experience biological and psychological maturity which are characterized by attraction toward the opposite sex and begin experimenting about sexual activity.<a class="elsevierStyleCrossRefs" href="#bib0015">3,4</a> Young marriages are influenced by gender inequality, can create a health problem, but the level of acceptance and practice of young marriage is strongly influenced by geography, culture and religion.<a class="elsevierStyleCrossRef" href="#bib0025">5</a> Gender ideology involves beliefs that see differences between males and females, in terms of social justice, and nature that is based on gender particularly regarding rights and obligations, freedoms and restrictions, boundaries and possibilities, as well as strength and subordination. Culture often places men higher than women.<a class="elsevierStyleCrossRef" href="#bib0030">6</a> Research has shown that some gender inequality can be overcome with education.<a class="elsevierStyleCrossRef" href="#bib0035">7</a>Reproductive education based on gender equality can develop motivation and efficacy of self-control, change attitudes and beliefs of traditional gender roles, reduce the impact of sexuality, and increase youth empowerment.<a class="elsevierStyleCrossRef" href="#bib0040">8</a> In a previous study conducted by Parmawati,<a class="elsevierStyleCrossRef" href="#bib0045">9</a> it was found that health education based on gender equality had an effect on improving the knowledge and attitudes of reproductive health in adolescent girls. The purpose of this study was to determine the effect of gender equality-based reproductive health education on improving knowledge, attitudes, and self-efficacy in adolescent girls and boys.Material and methodsThe research was a quasi-experimental study using a nonequivalent pretest and posttest with a control group design. This research was conducted in from August 2018 until April 2019. Schools were determined based on preliminary studies with predetermined criteria, School A as the intervention group and School B as the control group was done without randomization. The purposive sampling method was used to select 135 students. The two groups performed pretest measurements, then received interventions of health education based on gender equality and routine reproductive health education, after which posttests were conducted. The research site was in a junior high school class in Yogyakarta. Inclusion criteria were: (1) female and male students of class XI; (2) willing to become respondents and (3) permitted by parent. Exclusion criteria were: (1) students who did not attend school when collecting data and did not participate in a health education activities. Health education was given through small group discussion, lectures with booklet and comic media. Based on the sample calculation the minimum number of samples was 33 people for each group, total respondents 135 students. The instruments of this study were the reproductive health knowledge, the reproduction attitude and the sexual self-efficacy questionnaire. All questionnaires had previously passed the content validity test by 3 experts, and the construct validity test with 124 respondents declared the questionnaire can be considered valid and reliable. Independent t-tests were used to compare differences in mean scores between control and intervention groups with normal data distribution. Ethical approval certificate number: Ref: KE/FK/0865/EC/2018.Findings<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows that the intervention and the control groups at the beginning of the study had homogeneous characteristics, only one characteristic of respondents showed differences in the experience of getting reproductive health education.<elsevierMultimedia ident="tbl0005"></elsevierMultimedia>DiscussionBased on <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, for the male respondents, the value of knowledge, attitudes and self-efficacy in the intervention group tended to decrease, as well as in the control group, except the value of self-efficacy was slightly increased, there were no significant differences between pre and post-test. For the female respondents, in the intervention group, only the knowledge showed a significant increase whereas the attitude and self-efficacy tended to decrease. Meanwhile in the control group the value tended to increase but the increase in score was not significant between the pre and post-test.<elsevierMultimedia ident="tbl0010"></elsevierMultimedia><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows, health education affects the knowledge level only in female respondents. Based on a systematic review conducted by Kagesten et al.<a class="elsevierStyleCrossRef" href="#bib0050">10</a> it was found that adolescents from various cultures have a tendency of expressing stereotypical or gender injustice behavior. Teenage girls are frequently treated badly by boys. This injustice will then have an impact on adolescent reproductive health. Adolescents need to be given a correct and appropriate understanding of gender justice in the context of reproductive health.<a class="elsevierStyleCrossRef" href="#bib0055">11</a> found there was a significant change in the value of sexual autonomy while for sexual knowledge in adolescents there was no significant increase, the intervention was given for 10 months. Kernsmith and Jozefowics<a class="elsevierStyleCrossRef" href="#bib0060">12</a> research also provided gender-sensitive interventions with peer education programs. The main results indicated that gender-sensitive peer educators can encourage boys to reduce violence against girls in school. Gender specific programs could reduce sexual misconduct and harassment. This study also used the method of intervention for an extended time and followed up after 3 months.<elsevierMultimedia ident="tbl0015"></elsevierMultimedia>According to the research by Chi et al.<a class="elsevierStyleCrossRef" href="#bib0065">13</a> on the effects of comprehensive reproductive health education, there was also a significant increase in sexual health knowledge and a positive attitude after being given health education with as many as 6 sessions for 9 weeks. Longer time in providing interventions can certainly increase the knowledge and understanding in adolescents, because the material can be given repeatedly. In this study the intervention was only given as an additional extracurricular activity for students and short time, because reproductive health education has not yet been included in the learning curriculum.<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the male group values knowledge, attitudes and self-efficacy were not significant. In the group of women, only the value of knowledge had a significant difference that indicated an increase in knowledge. At the beginning of the study, the knowledge, attitudes and self-efficacy of adolescent girls showed a higher score compared to adolescent boys. This can be influenced by the levels of interest of adolescent girls which are higher compared to adolescent boys. The interest of young women was also shown by their paying more attention and active asking question when health education was given. Mudingayi et al.<a class="elsevierStyleCrossRef" href="#bib0070">14</a> found adolescent knowledge about HIV and sexual risk behavior was higher among boys with 54.3% of the male group compared with 45.7% for the female group. The results about premarital sexual behavior and condom use behavior in both groups showed the same behavior that were not good.This study provided gender equality-based reproductive health education, according to Grose et al.<a class="elsevierStyleCrossRef" href="#bib0040">8</a> Provision of sexuality education interventions that are integrated with gender and basic human rights has also been reported in the It's All One program by Rogow et al.<a class="elsevierStyleCrossRef" href="#bib0075">15</a> The It's All One is a program teachers were involved which had a better value for program success and sustainability. In this study the researchers directly provided health education interventions to students, while not involving teachers in providing health education. The differences in receiving information in the groups of adolescent girls and boys in this study are influenced by several factors including: (1) interventions provided in a relatively short time; (2) respondents are less serious in receiving health education; (3) topics and methods of health education may be less attractive to them and (4) lack of follow-up to read modules and comics.In the results of this study, adolescent boys were less enthusiastic and less seriously involved in the interventions, while the adolescent girls were more interested and eriously. Differences in respondent responses were also conveyed in the study by Grose et al.<a class="elsevierStyleCrossRef" href="#bib0040">8</a> that provided a school-based sexual education intervention program, which included topics about gender ideology. The results of that research on adolescent girls showed more increased progressive attitudes, where there was an increase in knowledge and there was a low agreement stating that the position of men was higher than women. Kagesten et al.<a class="elsevierStyleCrossRef" href="#bib0050">10</a> stated the existence of stereotypical attitudes and gender injustice is influenced by demographic characteristics factors, such as: gender, ethnicity, immigration, social class and age, in addition to interpersonal factors, namely: family and peers, and the use of media.ConclusionsGender-based reproductive health education was not been able to increase the value of knowledge, attitudes and self-efficacy in adolescent boys.Reproductive health education based on gender equality could only increase the value of knowledge among adolescent girls.Suggestion<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005">1.For schools, the reproductive health education based on gender equality can be one alternative method to provide reproductive health education.</li><li class="elsevierStyleListItem" id="lsti0010">2.For further researchers, using better methods in providing reproductive health education is recommended by optimizing the follow-up, choosing the right time to conduct interventions so as not to interfere with adolescents’ concentration, and considering additional time allocation in an effort to provide more effective gender equality-based reproductive health education.</li></ul>FundingDepartment of Nursing, Faculty of Medicine UGM.Conflict of interestThe authors declare no conflict of interest." For the adolescent girls the comparison of the scores of the intervention group actually tended to decrease at the posttest while the control group tended to increase, except in the aspect of knowledge with Knowledge value 1.06 vs −0.34, p=0.006, attitude value −6.78 vs 0.91, p=0.000, and self-efficacy value −5.06 vs 1.94, p=0.015.
