survivors discharge from hospital to home may require continuous support for enteral feeding&#46; Due to this problem&#44; there has been a rapid increase in the number of survivors of ICU receiving home enteral tube feeding &#40;HETF&#41; in both the US and Europe&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> It found that poor management of enteral feeding in the community can lead to complications&#44; hospital admissions&#44; and dissatisfaction with reference care&#44; and therefore it is important that people with enteral feeding&#44; caregivers and primary care services are supported in order to manage therapy effectively&#46;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> In addition&#44; the lack of education available to community nurses requires flexible and accessible education and motivational needs to be addressed&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Consequently&#44; adequate education&#44; training&#44; and support are needed both to ensure a smooth transition between care settings and safe and effective management in primary care&#46;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> The objective of this study was to implement and evaluate the effect of the knowledge transfer programme on community nurses&#8217; knowledge and nursing skills on enteral nutrition in the care of critically ill survivors in Kuantan and Pekan&#44; Pahang&#46; The researcher used the Kirkpatrick&#39;s Training Evaluation Model&#59; Level 1 &#40;Reaction&#41; and Level 2 &#40;Learning&#41; as a pillar of this study&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Method</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Design</span><p id="par0010" class="elsevierStylePara elsevierViewall">In this study&#44; the researcher used an interventional study &#40;only an experimental group&#41; known as knowledge transfer programme&#59; a pre-test and post-test for assessment of community nurses&#8217; knowledge&#59; and a post-test for assessment of community nurses&#8217; nursing care skills&#46; The programme was organised at a one-day workshop in Kulliyyah of Nursing&#44; International Islamic University of Malaysia&#44; Kuantan&#44; Pahang&#44; Malaysia&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Population and study setting</span><p id="par0015" class="elsevierStylePara elsevierViewall">The target population for this programme was community nurses working in the Kuantan and Pekan community clinics of the Ministry of Health&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Data collection</span><p id="par0020" class="elsevierStylePara elsevierViewall">This programme began with a two-hour didactic lecture session&#46; Participants were asked to respond to the Multiple Choice Questionnaires &#40;MCQs&#41; before and after the didactic lecture session&#46; Thereafter&#44; the programme continued with a lecture demonstration on nursing care skills prior to the evaluation&#46; The researcher demonstrates all steps of the patient procedure in front of the community nurses and addresses with them the rationale and precautions for each step&#46; The researcher then assessed the nursing skills of community nurses through the Nursing Care Skills Checklist&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Study instrument</span><p id="par0025" class="elsevierStylePara elsevierViewall">The two study tools are as follows&#58;</p><p id="par0030" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Multiple Choice Questionnaires &#40;MCQs&#41;</span>&#58; Researcher has developed Multiple Choice Questionnaires &#40;MCQs&#41; to assess the knowledge of community nurses related to enteral nutrition before and after the knowledge transfer programme&#46; MCQs were validated through content validation and the Cronbach alpha coefficient was 0&#46;735&#44; indicating an acceptable range of reliability for internal consistency&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Nursing Care Skills Checklist</span>&#58; The checklists were adopted from the &#8220;Senarai Semak Prosedur Kejururawatan Kursus Diploma Kejururawatan Kementerian Kesihatan Malaysia&#8221; &#40;2008&#41; and the IIUM Nursing Skills Manual &#40;2013&#41;&#46;</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Data analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">IBM Statistical Package for Social Sciences &#40;SPSS&#41; Statistic Version 20 was used to analyse the knowledge of community nurses and nursing care skills on enteral nutrition&#46; A descriptive analysis was used to analyse the pre-test and post-test result on the knowledge of community nurses&#44; and analyse the community nurses&#8217; nursing care skills on nasogastric tube insertion&#44; nasogastric tube feeding and enteral nutrition health education after intervention&#46; Spearman Rank Correlation is used to analyse the relationship between the knowledge of community nurses and the nurses care skills in enteral nutrition&#46;</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Ethical consideration</span><p id="par0045" class="elsevierStylePara elsevierViewall">Data collection began after approval by Kulliyyah of the Nursing Postgraduate and Research Committee &#40;KPGRC&#41;&#44; the National Medical Research Register &#40;NMRR&#41; and Pahang State Health Department&#46;</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">The demographic data between community nurses is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#46; As a result&#44; 43&#46;9&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>36&#41; of community nurses&#8217; age ranges from 30 to 39 years of age&#46; The majority of them are Malay and graduated in nursing education&#44; 95&#46;1&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>78&#41; and 96&#46;3&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>79&#41; respectively&#46; In addition&#44; 42&#46;7&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>35&#41; of community nurses have 11&#8211;15 years of experience as registered nurses&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#44; there is a clear trend for an increasing number of participants who respond correctly to the Multiple Choice Questionnaires &#40;MCQs&#41; following the intervention&#58; &#8220;Complication and Risk&#8221; &#91;4&#46;9&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>4&#41; to 67&#46;1&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>55&#41;&#93;&#44; &#8220;Definition&#8221; &#91;79&#46;3&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>65&#41; to 95&#46;1&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>78&#41;&#93;&#44; &#8220;Advantage and Disadvantage&#8221; and &#8220;Nursing Management&#8221; &#91;31&#46;7&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>26&#41; to 41&#46;5&#37; &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>34&#41;&#93;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> presented that community nurses conducting a tube location assessment via&#58; auscultation &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>73&#44; 89&#37;&#41;&#44; pH paper testing &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>64&#44; 78&#37;&#41; and a bubble test &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>57&#44; 69&#46;5&#37;&#41;&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows that 18&#46;3&#37; do not record the amount and the type of formula given in the intake and output chart&#46;</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows that community nurses have performed well in providing health information to the patient&#59; unclog nasogastric tube with syringe 50<span class="elsevierStyleHsp" style=""></span>ml &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>50&#44; 61&#37;&#41;&#44; administer 10<span class="elsevierStyleHsp" style=""></span>ml of warm water &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>50&#44; 61&#37;&#41;&#44; never combine medication with tube feeding &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>59&#44; 72&#37;&#41; and crush medication with fine powder and disperse in 5<span class="elsevierStyleHsp" style=""></span>ml of warm water &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>66&#44; 80&#46;5&#37;&#41;&#46;</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>&#44; <span class="elsevierStyleItalic">p</span>-value &#62;0&#46;05&#44; therefore&#44; do not reject Ho&#46; Knowledge and nursing care skills on enteral nutrition are not directly and strongly correlated&#46;</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Discussion</span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Community nurses&#8217; knowledge in enteral nutrition</span><p id="par0080" class="elsevierStylePara elsevierViewall">The result of the Multiple Choice Questionnaire &#40;MCQ&#41; pre-test shows that there is a clear deficiency in the knowledge of enteral nutrition of community nurses&#46; Specific knowledge deficiencies identified included in the following&#58; 21&#37; did not understand the definition of enteral nutrition&#59; 57&#37; did not know the advantage and disadvantage of enteral nutrition&#59; 92&#37; did not know the indication and contraindication of enteral nutrition&#59; 95&#37; did not know the complication and risk of enteral nutrition&#59; 68&#37; did not understand the nursing management regarding enteral nutrition&#46;</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Community nurses&#8217; nursing care skills in enteral nutrition</span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Nasogastric tube insertion</span><p id="par0085" class="elsevierStylePara elsevierViewall">The main component of nasogastric tube insertion skills is tube placement confirmation to prevent life-threatening complications&#46; More than half of the community nurses use the air bubbling technique that is performed by detecting the air exchange in the distal part of the tube to confirm its position&#46; This technique is unreliable in terms of its accuracy and ineffective in guiding professionals to the location of the tube&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> Besides&#44; community nurses most often use the auscultatory method&#46; However&#44; there is no evidence to show the effectiveness at the same weight of radiographic and pH confirmation methods&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> Consequently&#44; the auscultatory method &#40;also known as air insufflations test&#41; should not be used as the sole tool for determining the location of the feeding tube&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> Previous studies have verified that radiography is the most effective and favoured technique &#40;Gold-standard&#41; given the risk of exposure to radiation&#44; if not available&#44; the analysis of aspirates can be applied by pH colour test&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> The rationale of pH colour testing is the combination of pH and visual characteristics can help to differentiate between the position of the respiratory and gastrointestinal tubes&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a></p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Nasogastric tube feeding</span><p id="par0090" class="elsevierStylePara elsevierViewall">The finding shows that 18&#46;3&#37; of community nurses do not document the amount&#44; type of enteral formula given in the intake and output chart following the nasogastric tube feeding procedure&#46; It is recommended that people receiving enteral tube feeding in the community should have their nutritional status monitored every 3&#8211;6 months or more often if there is any change in their clinical condition&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> From the intake and output chart&#44; we are able to monitor the patient&#39;s weight&#44; body mass index &#40;BMI&#41;&#44; nutritional and fluid intake&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a></p></span></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Health education on enteral nutrition</span><p id="par0095" class="elsevierStylePara elsevierViewall">Mechanical complications are quite common in home enteral nutrition &#40;HEN&#41; and include dislodgement and obstruction of the tubes&#46; Common routine in the prevention of nasogastric tube feeding has been clogged by flushing the tube before and after feeding as three-quarters of community nurses performing routine&#46; Routine action similar to the ESPEN guideline as water flushing after feeding can prevent the occlusion of the tube&#44; and if the tube is clogged&#44; simple water flushing can help to restore the patency&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a> The majority of community nurses also administer warm water to de-clog tubes&#44; as warm water is the most effective irrigant for the de-clogging of blocked feed tubes&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a> Three-quarters of community nurses have not pointed out that &#8220;advise against using anything else&#44; such as carbonated beverages or meat tenderisers&#44; to unclog your tube because clinical studies have shown that they are not effective&#8221; in health education on enteral nutrition&#46; It is important to advise patients or caregivers at home not to use carbonated beverages as it is not recommended because the sugar content of sodas increases the risk of contamination of the tubes by bacteria&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a></p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Association between community nurses&#8217; knowledge and nursing care skill in enteral nutrition</span><p id="par0100" class="elsevierStylePara elsevierViewall">The present study reported that there is no correlation between the knowledge of community nurses and nursing care skills&#59; nasogastric tube insertion&#44; nasogastric tube feeding&#44; and enteral nutrition health education&#46; The findings of the current study are consistent with the study that found that there was no significant correlation between overall nursing knowledge and NG tube feeding administration practice&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a></p></span></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusion</span><p id="par0105" class="elsevierStylePara elsevierViewall">In conclusion&#44;
