One of them is the complication of diabetic foot with characteristics of dry, blackened foot, easily thickened, pale, and easily injured. The number of people with DM in the world reached 382 million people in 2013 with the largest population in poor and developing countries.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">1</span></a> Reducing the rate of DM disease and preventing complications earlier in the diabetic foot have become challenges.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Changes of circulation in diabetic foot play an important role in damage to the integrity of the skin and tissue. It can affect the wound healing process and the process of further treatment. Changes of circulation in diabetic foot include changes in microvascular and macrovascular conditions that will have an impact on patients’ comfort.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">2</span></a> The cause of circulation changes in diabetic foot is associated with poor glucose control.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">3</span></a> Microvascular changes in diabetic foot include the presence of sensory, motor, and autonomic neuropathy, which will affect cause the skin to be less sensitive, impaired, and dry. Macrovascular changes are characterized by a decrease in the quality of peripheral pulses due to blockages in the blood vessels in the legs, which can contribute to the gradual blockage of circulation in the legs. The prevalence of diabetic foot syndrome in the range of 4–15% contributes to the time of day care. The results showed that there was a significant relationship between the duration of diagnosis of diabetes mellitus, patient age, and compliance in controlling diabetes mellitus and the occurrence of complications of diabetic neuropathy.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">4</span></a> Meanwhile, research on the relationship between changes in the ankle-brachial index (ABI) score and the healing process in diabetic foot ulcers is still rare, especially research that uses the digital image approach for measuring granulation tissue, necrosis, fibrin, and wound area.</p><p id="par0015" class="elsevierStylePara elsevierViewall">One of the procedures for examining blockage in the leg arteries is by using an ABI score useful for looking at the degree of ischemia.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">5</span></a> The condition of ischemia contributes to damage to the integrity of the skin and tissues in the diabetic foot. Thus, further studies need to be done. Damage to skin and tissue integrity can be measured by digital image methods with a mobile wound analyzer (MowA) to see changes in granulation, necrosis, fibrin, and wound area. Therefore, a preliminary study is needed regarding the relationship between ABI scores and the degree of damage to the skin and tissue integrity in diabetic foot with a wound image analysis approach. This study aimed to analyze the relationship between ABI scores and damage to skin and tissue integrity in type 2 diabetes foot ulcers with parameters of granulation, necrosis, fibrin, and wound area. This research becomes the basis for nurses in making predictions from the results of circulation studies on diabetic foot related to changes in wound tissue based on wound image analysis. This will help nurses plan more appropriate management on diabetic foot, which is preventive, promotive, and curative collaboration. Long-term effects are useful in the development of interventions based on the improvement of peripheral circulation.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Method</span><p id="par0020" class="elsevierStylePara elsevierViewall">This study used a quantitative research approach with a descriptive design using a cross-sectional method. A total of 76 patients with diabetic foot ulcers were recruited by purposive sampling technique at 3 hospitals in Malang, Indonesia. The data collection includes patient characteristics data; ABI scores measurement with a foot Doppler device; measurements of granulation, necrosis, fibrin, and wound area with the application of MowA.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">6</span></a> This research has obtained approval from the Health Research Ethics Commission of the Faculty of Medicine Universitas Brawijaya, Malang, Indonesia (No. 188/EC/KEPK/08/2018).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">The results of the analysis showed that the average age of the patients was 59.97 years (95% CI: 58.66–61.29) with a standard deviation of 5.74 years; the average duration of diabetes mellitus was 4.74 years (95% CI: 4.27–5.20) with a standard deviation of 2.03 years; the average body mass index (BMI) of diabetic foot ulcers patients was 24.52<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> (95% CI: 23.85–25.19) with a standard deviation of 2.94<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>; the average blood sugar level of diabetic foot ulcers patients was 216.66<span class="elsevierStyleHsp" style=""></span>mg/dl (95% CI: 198.95–234.36) with a standard deviation of 77.47<span class="elsevierStyleHsp" style=""></span>mg/dl (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Diabetic foot ulcers patients with male sex were 32 individuals (42.1%) and women were 44 individuals (57.9%). Meanwhile, patients with smoking history were 30 individuals (39.5%), and nonsmokers were 46 people (60.5%) (see <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">The statistical test results showed that ABI scores were significantly associated with granulation (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.015), necrosis (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.000), fibrin (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.036), and wound area (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.025). From all diabetic foot ulcer profiles, necrosis tissue has a strong relationship with ABI scores with negative direction, which means that increasing ABI scores will reduce necrosis tissue in diabetic foot ulcers (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.516). Granulation tissue has a moderate relationship with the ABI score in a positive direction, which means that increasing the ABI score will increase the granulation tissue (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.278). Fibrin and wound areas have a weak relationship with the ABI score (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.240; <span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.258) (see <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">Based on the results of the analysis of the patients’ characteristics, most DM patients who have complications of diabetic foot ulcers are women and nonsmokers in the elderly category. Their BMI values and blood sugar levels are within the upper normal limits with a long average of 4.74 years with diabetes mellitus. Increasing age and duration of diabetes are risk factors for diabetic foot ulcers.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">7,8</span></a> This shows that elderly women with DM have a risk of complications of diabetic foot ulcers despite controlled BMI and blood sugar conditions. This condition has an impact on the quality of life of women with complications of diabetic foot lower than men, especially in terms of body function and pain.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">9</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Necrosis tissue has a strong relationship with ABI scores with negative direction, which means that increasing ABI scores will reduce necrosis tissue in diabetic foot ulcers. This concept aligns with the optimal circulation process in diabetic foot, which can reduce the formation of necrosis tissue so that the tissue regeneration process will run better. The ABI score describes the condition of the macrocirculation, which is thought to affect tissue profiles at the base of diabetic foot ulcers as measured by digital images. The results study are beneficial for wound practitioners in predicting circulatory conditions by analyzing tissue necrosis in diabetic foot ulcers. It could be used as preliminary data for consideration of circulatory examinations. The results of the study showed that circulation factors play an important role in healing diabetic foot ulcers.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">10</span></a> This is in accordance with the results of study indicating that inadequacy in monitoring ABI can encourage necrosis tissue formation.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">11</span></a> The condition of ischemia in diabetic foot marked by a decreased ABI score will be followed by the formation of widespread necrosis tissue that is exacerbated by the occurrence of hyperglycemia in persons with DM.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">12</span></a> The results of this research are useful for the preparation of the wound base so that the healing process can run more optimally in accordance with the conditions of the macrocirculation and microcirculation so that necrosis tissue can be controlled.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">13</span></a> The analysis of MowA, necrosis tissue is captured by digital images as blackish-colored tissue.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Granulation tissue and fibrin are often found on the base of the wound with irregular shapes.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">14</span></a> The results of this study indicate that granulation tissue has a moderate relationship with the ABI score in a positive direction, which means that increasing the ABI score will increase the granulation network. Moderate relationships can be caused by the condition of diabetes stenosis of the blood vessels so that it will affect the results of ABI measurement.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">15</span></a> Therefore, it is necessary to compare it with other examination methods for predicting granulation changes. ABI measurement is often used because it is noninvasive, inexpensive, and easily applied. Poor circulation will affect the process of granulation so that revascularization management is needed for the management of diabetic foot.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">16</span></a> Increased blood flow to the wound area can increase cellular factor changes on the wound base, such as vascular endothelial growth factor (VEGF), EGF, platelet-derived growth factor, and angiotensin two, which have an impact on granulation processes.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">17</span></a> In the analysis by the MowA method, granulation tissue was captured as a reddish-pink tissue indicating an extensive measurement of granulation tissue.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The formation of fibrin at the base of diabetic foot ulcers is very weak, shown by ABI scores. The process of fibrin formation begins with the occurrence of necrosis in diabetic foot ulcers indicated by the change from yellow to blackish brown. This shows that tissue injury occurs avascularly, which affects the outer surface of the fibrin tissue to appear blackish brown. In the analysis procedure using the digital image method, this color was captured as a necrosis tissue. The weakness of the MowA method in analyzing fibrin conditions is that it does not reflect the condition of the tissue under the wound surface, which is actually an avascularization process. Therefore, it is necessary to conduct further research that strengthens the process of fibrin formation associated with changes in circulation to monitor existing changes using digital image analysis methods in diabetic foot wounds. The results of previous studies indicate that digital image analysis is an accurate method for double-blind and multicentric trials to measure the percentage of fibrin in wounds as a consideration for wound debridement.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">18,19</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The wound area has a weak relationship with the ABI score. The area of the wound reflects changes in process in the stages of healing diabetic foot ulcers. The process of healing wounds in diabetic foot is influenced by several factors, such as the duration of diabetes, the presence of vascular disease and the type of ulcers.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">20</span></a> Further research related to the analysis of digital images of diabetic wound area needs to be further linked to other factors so that it can help strengthen the prediction of existing changes. This condition can be influenced by the degree of complications caused by the course of DM disease.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Conclusion</span><p id="par0060" class="elsevierStylePara elsevierViewall">It can be concluded that the method of digital image analysis employed to analyze diabetic foot ulcer profiles in terms of granulation, necrosis, fibrin, and wound area produced good data for the wound assessment. This method can help nurses analyze injuries more accurately to support wound care services.
