According to Republic Indonesia Health Regulation<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">2</span></a> it is stated that the recording of patient data, can be carried out in writing and electronically. Patient health records that allow for electronic access are called electronic health records (EHR).</p><p id="par0010" class="elsevierStylePara elsevierViewall">In the EHR, the data will be easier to read than data in manual medical records.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a> In addition, the use of EHR allows the availability of patient health data in real time and increases the effectiveness and efficiency.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">4</span></a> The use of EHR is also useful to reduce medical errors and improve operational performance.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In 2012, several countries showed high achievements in the implementation and use of EHR, including Switzerland 41%, Canada 56%, France 67%, USA 69%, Germany 82%, and Australia and New Zealand reached 90%.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">5</span></a> The use of EHR in Indonesia itself, has been started by several hospitals.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">6</span></a> One of the hospitals in Indonesia that has implemented the EHR system is Gadjah Mada University Hospital (UGM Hospital). The manager of the UGM Hospital's medical record states that an evaluation has never been done related to the impact or use of the EHR, while according to Chang et al.,<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">7</span></a> the evaluation of the EHR is important to be done in a timely manner. EHR evaluation is important because EHR is one of the main investments in information technology for hospitals and government.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">7</span></a> One way to evaluate the EHR is by measuring EHR user satisfaction.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">8</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">User satisfaction is used to evaluate the EHR because user satisfaction can affect the successful implementation of her.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">9,10</span></a> A study by Sharabatiet al.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">11</span></a> states that user satisfaction with the system when the system is used, affects the success of an information system. When users feel satisfied with a system, it can be interpreted that the system works well and meets user needs, and vice versa.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">12</span></a> The purpose of this article is to report how satisfied are the nurses and physicians towards the implementation of EHR at UGM Hospital.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Method</span><p id="par0025" class="elsevierStylePara elsevierViewall">This was across sectional study design. The study setting was in the outpatient service unit of UGM Hospital. The inclusion criteria were nurses and physicians who had used the EHR for at least 6 months and were willing to be involved in the study. We sent the questionnaire to all physicians and nurses in the unit (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>67). Two physicians were refused to be involved because they were on leave. The response ratio for physicians is 69.23% and for nurses is 100%. One nurse was excluded from the study because did not meet the inclusion criteria. Thus, the final number of respondents was 27 physicians and 25 nurses. Before filling out the questionnaire, respondents were given an explanation of the study and signed an informed consent.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Data collection was carried out from January 23, 2019 to March 15, 2019. The instrument in the study used the Electronic Health Record Nurse Satisfaction (EHRNS) instrument developed by Sockolow et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">10</span></a> The instrument was translated into Bahasa Indonesia using Brislin method. The instrument's content validity is 0.90 which can be interpreted as very relevant instruments. Item number 12 was dropped because it was not relevant with the EHR conditions used at UGM Hospital.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Results and discussion</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Respondents characteristics</span><p id="par0035" class="elsevierStylePara elsevierViewall">Most of the respondents were in adulthood (26–45 years old). Most nurse respondents (60%) and physicians (66.7%) have used EHR for more than 4 years and only 3% of nurse respondents who have only used EHR for less than 1 year. All nurse respondents (100%) and most physicians respondents (59.3%) used EHR with a frequency of more than 3 times a week. Most of the nurses (60%) and physicians (59.3%) had received EHR training. The remaining 36% of nurses and 40.7% of respondents said that they had never had training on the HER (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Physicians and nurses satisfaction with EHR implementation in UGM Hospital</span><p id="par0040" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows a description of nurses and physicians’ satisfaction with the implementation of EHR at UGM Hospital in the dimension of quality of information logistics. There were no differences in satisfaction between nurses and physicians in this dimension. Almost all items in this dimension get satisfactions cores from respondents, except for one item: “the patients’ health record is worth the time and effort required to use it” gets neutral score from both professions.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The table shows that some items in the dimensions of effects on quality of processes have neutral values, which can be interpreted that nurse and doctor respondents are not satisfied or are not disappointed with certain items in the EHR system. Items that are neutral include the effect of the EHR on efficiency, the effect of the EHR on the accuracy of care procedures and the role of nurses and doctors during the implementation of the EHR. Only one item in the effects on quality of processes dimension was considered satisfactory by respondents. There is no difference in satisfaction between nurses and doctors on the dimension of effect of the quality process (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">The table present the satisfaction of nurses and physicians with the implementation of EHR at UGM Hospital on the dimensions of effects on outcomes and quality of care. There were no differences in satisfaction between both professions. The effect of EHR on patient safety and the effect of EHR on interprofessional communication are two items on the dimensions that get good score (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Discussions</span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Respondent characteristics</span><p id="par0055" class="elsevierStylePara elsevierViewall">Research by Al-Azmi et al.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">13</span></a> states that age has a positive correlation with how users react to EHR. Only 14.8% of physician respondents were in the early elderly age (46–55 years) and 3.7% of the physician respondents were in the late elderly (56–65 years). The older a person (>46 years) is, the more difficult she/he is to accept the use of new technology, for example the use of computer storecord patient data.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">14</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">Most respondents nurses (60%) and physicians (66.7%) have used EHR for more than 4 years. The frequency of EHR use was more than 3 times a week in all nurse respondents (100%) and in almost all physician respondents (88.9%). Research by Holanda et al.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">15</span></a> found a positive correlation between the duration of EHR use and EHR user satisfaction. Adler et al.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">16</span></a> found that the longer use of EHR can increase satisfaction. This can be said because along with the increasing use of the EHR, users will be more experienced and accustomed to using the EHR so that satisfaction will increase.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">17</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Based on the results in this study, the majority of respondents were nurses (60%) and physicians (59.3%) had received EHR training and the rest had never received training on EHR. EHR training can be one of the strong factors that influence satisfaction.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">9,18</span></a> This is because after the training, the EHR users will experience increase in knowledge, abilities and skills in using her.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">13</span></a></p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Physicians and nurses satisfaction</span><p id="par0070" class="elsevierStylePara elsevierViewall">There was no difference in satisfaction between nurses and physicians. Nurses and physicians generally have the same satisfaction with the implemented EHR.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Nurses and physicians in the study were satisfied with the role of EHR in patient care outcomes. According to the study, the use of EHR can lead to better patient care outcomes by increasing patient safety through features available in the EHR, for example Clinical Decision Support System<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">18</span></a> features. The study also found that nurses and physicians at UGM Hospital were satisfied with the role of EHR in patients safety. The American Medical Association<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">19</span></a> mentions several things that can interfere with patient safety in hospitals, for example medication errors, diagnostic errors and administrative errors. Research respondents stated that hand writing on the paper medical records is often difficult to read. Sometimes it causes misdiagnoses and medication errors. In addition, the use of paper medical records often leads to duplication of patient data.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The study of Tanner et al.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">20</span></a> found that the use of EHR can prevent errors in drug orders, the use of EHR allows a warning when making wrong drug orders. The use of EHR could prevent diagnostic errors, increase the effectiveness of communication between care team members.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">21</span></a> This was confirmed by the study of Staggers et al.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">22</span></a> which found that EHR contributed less than 1% to the total errors that occurred in health care settings. The impact of EHR use on patient safety is important to be investigated in the next study.</p><p id="par0085" class="elsevierStylePara elsevierViewall">This study found that respondents were satisfied with the effect of EHR on patient comfort. According to study respondents, patients feel comfortable with nurses and physicians who use EHR, as in the Bahadori et al.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">23</span></a> and Rao et al.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">24</span></a> studies. The longer the clinician pays attention to the patient's explanation and continues to interact with the patient while using the EHR, the patient will remain satisfied, comfortable and will not feel disturbed.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">25</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Respondents were satisfied with the security system. Previous research by Alharthi et al.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">26</span></a> also found that respondents were satisfied with the safety of patient data on the EHR. This finding could be due to the respondents perceptions that with EHR patient health data is stored safely and not mis used outside the hospital.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">27</span></a> Even so, physicians still worry about the confidentiality of patient data on the EHR at UGM Hospital, because there are no restrictions on accessing patient data. This can be an input for EHR developer, that it should be limited to physicians or clinicians who are responsible for patients care. They are the one should get complete access to patient data other than that there should be restrictions. This concern arose could be because the participants did not have enough information about access regulation which has been established at the setting. However it shows that the clinicians were aware of this rules related to confidentiality and data safety.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Some items also get scores that can be interpreted as dissatisfaction. One item that got dissatisfied score is the training. Although most respondents had attended training, this dissatisfaction could be caused by the lack of training carried out, either at the time of the introduction of the EHR or when the system was implemented.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">27</span></a> Previous research suggested that the training should be more structured, more frequent and carried out regularly.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">28</span></a> Similar phenomenon found in a study conducted by Sockolow et al.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">27</span></a> and Devkota et al.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">28</span></a> The type of training conducted also influences user satisfaction. The study found that training conducted one-on-one had a different impact on satisfaction compared to training conducted in groups.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">29</span></a> In addition, according to research by Hill et al.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">30</span></a> found that training carried out at the beginning before the implementation, was not enough to make respondents understand the EHR system. Respondents expressed difficulty in practicing what they learned during the training.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Some items also get scores that can be interpreted as neutral, which was the respondent was not satisfied but also did not feel disappointed with the item. Items that were neutral were the effect of EHR on the clinical process. These results are slightly different from the results of previous studies conducted by Sockolow et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">10</span></a> The existence of these differences, can be caused by certain factors, as in the study of Ghattas et al.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">31</span></a> One influential factor is environmental conditions, including the availability of equipment and expertise of health workers.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">31</span></a> The neutral value on this item could be due to lack of expertise and ability to use EHR on respondents at UGM Hospital. In a study by Sockolow et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">10</span></a> respondents can be said to have slightly higher expertise because respondents were satisfied with the training conducted. This can be interpreted that the training conducted in research by Sockolow et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">10</span></a> was sufficient to improve the skills and abilities of respondents in using the EHR. This is in contrast to the training conducted at UGM Hospital.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0105" class="elsevierStylePara elsevierViewall">Based on the results of the study, there were no differences in satisfaction between nurses and doctors at UGM Hospital. Nurses and doctors at UGM Hospital have the same perception of satisfaction in all items in the EHR. Overall, nurses and doctors were satisfied with some functions in the EHR implemented at UGM Hospital.
