Seven HCPs were interviewed about their experiences.</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Results:</span> Two major themes emerged from the experiences of these HCPs: “dad in the back seat” and “paternal resistance.”</p><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Discussion:</span> The theme of “dad in the back seat” captured the HCPs’ experiences and perceptions of parental roles and related stereotypes with respect to fathers’ lack of presence in the health-care setting, family roles that relegate fathers to the back seat in dealing with this issue, and the tendency of fathers to take a passive role and defer to mothers in the management of their child's weight. “Paternal resistance” reflected the perceived tendency of the father to resist the acceptance of his child's weight as a problem and to resist change and even undermine family efforts to make healthier choices.</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Conclusion:</span> HCPs’ experiences of fathers as having a minimal role in the management of their child's overweight and obesity may lead them to neglect fathers as agents of change with regard to this important issue.</p></span><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Comment</span><p id="par0015" class="elsevierStylePara elsevierViewall">Obesity and overweight are serious public health problems that adversely affect children's health<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">1</span></a>; it is thus imperative seeking their causes whilst providing potential solutions for this problematics.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">2</span></a> The prevalence of pediatric obesity remains at historically high levels.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">3</span></a> This subject has been extensively studied covering different age groups and populations. However, preventing further incidence increase, by guiding children and families already identified with the disease, is a solution still to be accomplished It is essential to perform an oriented teaching, from caregivers to parents of obese and overweight children. Mothers, as well as fathers, are crucial influences in defining their children's habits.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In this sense, the present study addresses a current and serious issue, with the objective of exploring the experience of HCPs in the outpatient setting as they work with fathers of children who are overweight and obese. In this way, fathers are highlighted in this research, since most of the literature lies in the role played by the mothers. The intended purpose naturally demands a methodology of qualitative paradigm – a phenomenological reading fits the necessary interpretation to comprehend this type of data.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Despite not having a very representative sample, this study represents an initial first step in order to define main strategies with the parents of these children. The study grasses guidelines for HCPs to deal with parents (mothers and fathers) or ideally a “whole family approach” whilst also providing lines of investigation for future research – like the understanding of rules of engagement and involvement of fathers by having a positive influence on the weight of their children. Due to HCPs it is clear that fathers should be more involved in all health questions related to their children and not dismiss their parenting role (“dad in the back seat”).</p><p id="par0030" class="elsevierStylePara elsevierViewall">This issue has been studied by a qualitative approach by others authors, by interviewing parents of obese children and exploring their views and experiences,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">4</span></a> by interviewing parents and grandparents,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">5</span></a> or even interviewing parents and their obese children.<span class="elsevierStyleSup">6</span> They all seem to be in agreement on the importance of communication between child–parents and the multidisciplinary team approach benefiting them, in order to improve effectiveness of intervention on obesity in the childhood. As referred by this study the HCPs are in a unique position to interact with families and for that matter, they are a privileged sample for scrutiny. Since most research in this area is made of quantitative studies, a literature review on the qualitative studies in this matter can be helpful in better clarifying the state-of-the-art.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Through a qualitative methodology, by using the interview methodology for understanding perception on a certain matter, it is important to comprehend those participants who are being interviewed, namely their gender, whether they are parents or not, and the years of experience dealing with children and their parents. The nature rather than the size of the sample (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>6) is relevant in this study. All participants should be interviewed face-to-face, and there must also be male professionals available (could include HCPs fathers and HCPs not parents). The sample should also comprise a varied geographical distribution since cultural habits determine important environmental factors. It will be essential having a diversified distribution regarding the different health professionals working with the obese children population.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Questioning parents directly could be a way of completing this type of study, by adding to the HCPs the opinion and perception of parents on these issues. In that sense one may better understand the other major topic of this study – the “paternal resistance”.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Although limitations in the data collection and the sample selection may be present, this study addresses a current and relevant issue, and the obtained results shall function as guidelines for professionals. Future investigations in this respect should also account for the opinions of parents and their perceptions for having a more holistic perspective and for adding value to the interpretation by the intervening health professionals.
