Abstract

Evaluation and staging of liver disease is essential in the clinical decision-making process of liver tumors. The severity of portal hypertension (PH) is the main prognostic factor in advanced liver disease. Performing an accurate hepatic venous pressure gradient (HVPG) measurement is not always possible, especially when veno-venous communications are present. In those complex cases, a refinement in HVPG measurement with a thorough evaluation of each of the components of PH is mandatory. We aimed at describing how some technical modifications and complementary procedures may contribute to an accurate and complete clinical evaluation to improve therapeutic decisions.

La evaluación y la estadificación de la enfermedad hepática son fundamentales en el proceso de toma de decisiones clínicas de los tumores hepáticos. La gravedad de la hipertensión portal (HP) es el principal factor pronóstico en la enfermedad hepática avanzada. No siempre es posible realizar una medición precisa del gradiente de presión venoso hepático (hepatic venous pressure gradient [HVPG]), especialmente cuando existen fístulas veno-venosas. En esos casos complejos es obligatorio un refinamiento en el cálculo del HVPG con una evaluación exhaustiva de cada uno de los componentes de la HP. Nuestro objetivo es describir cómo algunas modificaciones técnicas y procedimientos complementarios pueden contribuir a una evaluación clínica precisa y completa para mejorar las decisiones terapéuticas.
