In the former it usually presents as an inflammatory phenotype of the oesophagus and in adults as a fibrostenotic phenotype, characterised by fibrosis and oesophageal stricture resulting in food impaction.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> To diagnose EoO, other causes of oesophageal eosinophilia must be ruled out.</p><p id="par0010" class="elsevierStylePara elsevierViewall">EoO was first described by Landres et al. in 1978<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> in a patient with vigorous achalasia, but it was not until 1993 that it was recognised as a specific pathological condition of the oesophagus.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> The prevalence of EoO is not evenly distributed around the world. It is most prevalent in northern European countries and Spain, as well as in forested areas with temperate climates in the USA, Canada and Australia, while it is less common in countries in Latin America, Asia and Africa.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> In the countries where it is most typically found, its incidence and prevalence has doubled in the last three decades, leading to it being described as an epidemic.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a> The causes of this increased incidence are unknown.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In essence, EoO is an allergic condition, mediated primarily by a Th2-type immune response involving eosinophils, mast cells and chemical mediators such as eotaxin-3, interleukin (IL)-4, IL-5 and IL-13.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Triggers for the disease are usually environmental and food allergens. Some 40%–60% of patients with EoO have a history of atopy.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> However, the reasons why people in these geographical regions used to tolerate these allergens and now react in this way are unknown. It has been proposed that improved social and economic conditions in industrialised countries have resulted in changes in the habits of the population, based on better hygienic conditions, leading to an increase in allergic conditions mediated by the Th2 immune response.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Helicobacter pylori</span> (<span class="elsevierStyleItalic">H. pylori</span>) is a Gram-negative bacterium that causes the most common chronic infection globally. Its prevalence is very uneven in different regions of the world. The extent of its prevalence is in most cases closely correlated with the socio-economic status of the population, and it is much more prevalent in underdeveloped countries.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a><span class="elsevierStyleItalic">H. pylori</span> infection is usually acquired in childhood and is associated with peptic ulcer disease and gastric adenocarcinoma.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">In recent years, several studies, mostly case-control studies, have been published describing an inverse relationship between the prevalence of EoO and <span class="elsevierStyleItalic">H. pylori</span> infection, which may represent a “protective effect” conferred by the infection. A recent meta-analysis of 11 studies reported a 37% reduction in the risk of EoO in patients with <span class="elsevierStyleItalic">H. pylori</span> infection.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> However, a prospective trial by Molina-Infante et al. found no significant inverse association between <span class="elsevierStyleItalic">H. pylori</span> and EoO, casting doubt on this relationship.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> The difference in the results of these studies may be due to methodological differences. In addition, most of the studies have been conducted in Europe, the USA and Australia, where the prevalence of <span class="elsevierStyleItalic">H. pylori</span> is low. To date, no trials have been conducted in Latin America where the prevalence of <span class="elsevierStyleItalic">H. pylori</span> is very high. In light of the above, we conducted a study to assess the type of relationship between <span class="elsevierStyleItalic">H. pylori</span> infection and EoO in Mexican patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">A prospective case-control study included patients referred to our gastroenterology department from January 2016 to December 2021. All patients gave their informed consent to take part in the study and undergo an upper gastrointestinal tract endoscopy (UGTE). The research was conducted in compliance with the Declaration of Helsinki. The protocol was approved by the Universidad Autónoma de Nuevo León Faculty of Medicine’s Ethics Committee.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Cases</span><p id="par0035" class="elsevierStylePara elsevierViewall">Adults over 18 years of age with a diagnosis of EoO without previous treatment were consecutively included. EoO was defined by symptoms of oesophageal dysfunction (regurgitation, retrosternal pain, dysphagia, etc.) and oesophageal biopsy with eosinophilic infiltrate >15 eos/HPF.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Controls</span><p id="par0040" class="elsevierStylePara elsevierViewall">Adult patients over 18 years of age, seen in the outpatient clinic, with oesophageal symptoms requiring UGTE, were evaluated and considered as probable controls. Individuals with no previous diagnosis of <span class="elsevierStyleItalic">H. pylori</span> infection who had <15 eos/HPF in oesophageal biopsies were included in this study. Four age-matched and gender-matched controls were assigned to each case.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Endoscopic procedures</span><p id="par0045" class="elsevierStylePara elsevierViewall">All UGTE were performed by board-certified gastroenterologists. The procedures were performed under intravenous propofol sedation. Patients taking proton pump inhibitors (PPIs) were discontinued for at least two weeks prior to UGTE. During the endoscopic procedure, a minimum of six biopsies were obtained from the mucosa of the distal and proximal oesophagus for investigation of eosinophilic infiltrate. In addition, five gastric mucosal biopsies (two from the fundus and antrum and one from the angular incisure) were taken according to the Sydney protocol to rule out eosinophilic gastroenteritis and to investigate the presence of <span class="elsevierStyleItalic">H. pylori</span>. The samples were sent to the hospital’s pathology department for evaluation.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Histological evaluation</span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Eosinophilic oesophagitis</span><p id="par0050" class="elsevierStylePara elsevierViewall">Oesophageal samples were processed with formalin and stained with haematoxylin and eosin for histological evaluation. Eosinophilic infiltrate in the oesophageal mucosa was determined in the area of highest cell density and counted in this region. A count greater than 15 eos/HPF in at least one of the fields was defined as EoO. Subepithelial fibrosis, microabscesses and eosinophilic degranulation were also reported.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105"><span class="elsevierStyleItalic">H. pylori</span> infection</span><p id="par0055" class="elsevierStylePara elsevierViewall">All patients were required to have no previous history of <span class="elsevierStyleItalic">H. pylori</span> infection. Stomach samples were processed with formalin and stained with haematoxylin and eosin for histological examination. <span class="elsevierStyleItalic">H. pylori</span> infection was defined as the finding of bacillary structures compatible with the germ in tissue samples. Identification of an inflammatory process suggestive of chronic active gastritis was also reported.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Data collection</span><p id="par0060" class="elsevierStylePara elsevierViewall">On a basis specially designed for the study, the demographic data of cases and controls (age and gender); the presence of atopy, which was established by assessment by an allergist and defined by the presence of asthma, allergic rhinitis, atopic dermatitis, drug and food allergy, oesophageal symptoms (retrosternal pain, regurgitation, dysphagia and food impaction), symptom duration; endoscopic findings (exudates, oedema, longitudinal furrows, oesophageal rings, strictures, hiatal hernia, erosive oesophagitis) were also recorded. Endoscopic activity of EoO was assessed with the Endoscopic Reference Score (EREFS) classification, which uses five types of endoscopic abnormality: oedema, rings, exudates, longitudinal furrows and strictures to assess disease. This score was proposed by Hirano et al. and subsequently validated.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Statistical analysis</span><p id="par0065" class="elsevierStylePara elsevierViewall">Parametric and non-parametric variables were expressed as means, standard deviations and ranges, respectively. Comparisons of the categorical variables of the different groups were performed using non-parametric tests such as the <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup">2</span> test with Yates correction where necessary, while continuous variables were analysed using Student’s <span class="elsevierStyleItalic">t</span>-test.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Multiple regression analysis was performed with the variables that were significant in the univariate analysis. Analysis of the association between EoO and <span class="elsevierStyleItalic">H. pylori</span> infection was estimated by calculating the odds ratio (OR) with its corresponding 95% confidence interval (CI). A p-value <0.05 was considered statistically significant. The statistical analysis system SPSS 22.0 was used for data evaluation.</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">A total of 241 potentially eligible patients were screened, of whom 38 had EoO (cases) and 152 did not (controls), making a total of 190 subjects. Some 51 people were excluded due to refusal to participate in the study (13 patients) or incomplete clinical information (38 patients).</p><p id="par0080" class="elsevierStylePara elsevierViewall">Cases and controls were similar in terms of gender and age, frequency of oesophageal symptoms such as regurgitation and retrosternal pain, as well as some endoscopic findings such as erosive oesophagitis and Barrett’s oesophagus. However, patients with EoO had significantly higher frequency of atopy (allergic rhinitis and food allergy), dysphagia and episodes of food impaction, peripheral eosinophilia and endoscopic abnormalities such as longitudinal furrows, oesophageal rings and strictures. Controls had a higher recurrence of hiatal hernia.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The EREFS score in the cases was 3.5 ± 2.0, with a mean of 4 (range 0–8), reflecting moderate disease activity. Eosinophils in the oesophageal mucosa were 37 ± 18.8 per HPF, with a mean of 34.5 (range 16–90). Subepithelial fibrosis was observed in 10% of cases (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125"><span class="elsevierStyleItalic">H. pylori</span> infection and EoO</span><p id="par0090" class="elsevierStylePara elsevierViewall">Out of a total of 190 patients, 121 were diagnosed with <span class="elsevierStyleItalic">H. pylori</span> infection (63.6%), of whom 66 (54.5%) were male and 55 (45.5%) were female (p > 0.05). Infection was more common in patients 40–49 years of age, with the distribution of infection by age group shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. There were significant differences in the prevalence of <span class="elsevierStyleItalic">H. pylori</span> between cases and controls. It was identified in 14/38 (36.8%) of cases, while in the controls it was observed in 107/152 (70.4%), odds ratio (OR) 0.21; 95% CI, 0.08−0.69, p = 0.001 (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130"><span class="elsevierStyleItalic">H. pylori</span> infection and atopy</span><p id="par0095" class="elsevierStylePara elsevierViewall">A sub-analysis of the prevalence of <span class="elsevierStyleItalic">H. pylori</span> infection in patients with and without atopy in general, regardless of whether they were cases or controls, showed an inverse relationship between the two groups: prevalence was lower in atopic patients compared to non-atopic patients: 13.1% vs. 50.5%, respectively, OR 0.20; 95% CI, 0.06–0.69, p < 0.001). Regarding types of atopic disease, the inverse correlation was found only in patients with allergic rhinitis (rhinitis 4.2% vs. no rhinitis 59.4%) OR 0.13; 95% CI, 0.2–0.60, p < 0.001) and food allergy (4.2% vs. 59.4%), OR 0.13; 95% CI, 0.2–0.60, p < 0.001 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). No inverse relationship was found with asthma, atopic dermatitis or drug allergy.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">This is the only study to date assessing the relationship between <span class="elsevierStyleItalic">H. pylori</span> infection and EoO conducted in Latin America, a region where the prevalence of <span class="elsevierStyleItalic">H. pylori</span> in the general population is one of the highest in the world<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> and where EoO is a rare disease<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>. Our study found an inverse association. This type of relationship was also observed with atopic diseases, particularly allergic rhinitis and food allergy. It was not linked to asthma or atopic dermatitis.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The results of our study are similar to those published in case-control studies between 2011 and 2017 (positive studies).<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–21</span></a> They reported wide differences in the prevalence of <span class="elsevierStyleItalic">H. pylori</span> infection between cases and controls (from 1.7% to 33.4%).<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a> In our analysis the difference was higher (33.6%). In contrast, our results differ from those recently published by Molina-Infante et al. in 2018 (negative study),<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> in which a similar prevalence of <span class="elsevierStyleItalic">H. pylori</span> infection was observed in cases and controls (37% vs. 40%, respectively, OR 0.97; 95% CI, 0.73–1.30, p > 0.05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">The positive studies were criticised because they were considered to have some methodological shortcomings: most were retrospective and did not describe the protocols used to obtain oesophageal and stomach biopsies,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–21</span></a> did not report whether PPI use was discontinued prior to UGTE, nor did they provide data on previous <span class="elsevierStyleItalic">H. pylori</span> eradication and treatment of EoO prior to inclusion in the protocol.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–21</span></a> In addition, some studies defined <span class="elsevierStyleItalic">H. pylori</span> infection as the presence of serum antibodies.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,20</span></a> This test cannot discern between an active infection and a previous inactive infection.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Although our trial and the negative Molina-Infante study have a similar methodological design, and the aforementioned shortcomings in the retrospective analyses were corrected, the results are different. The differences between the two studies are difficult to explain. The Molina-Infante study was multicentre, conducted in Europe and comprised a significant number of patients, while our study was conducted in a single centre and with a smaller number of patients, due to the fact that EoO is a rare disease in Mexico. However, our study used four controls for each case, which improves the efficiency of the statistical analysis in the comparison. On the other hand, in our study histology was used to define <span class="elsevierStyleItalic">H. pylori</span> infection in all patients, whereas in the Molina-Infante trial it was used in only 70% of patients. In this context, histology is considered to have a high yield for the diagnosis of <span class="elsevierStyleItalic">H. pylori</span> infection, with a sensitivity of approximately 91% and a specificity of 100% in adults,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> which can be improved when samples are obtained following the Sydney protocol during endoscopy.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> In our opinion, other factors attributed to the biological characteristics of the bacteria or to environmental factors should also be taken into consideration to explain the difference in results. In this context, two <span class="elsevierStyleItalic">H. pylori</span> antigens (gamma glutamyl transpeptidase and vacuolating toxin [VacA]) have been reported to induce regulatory T-cell activity (Tregs) in the gastric mucosa of experimental animals, resulting in tolerance development and decreased allergic response, especially in allergen-induced asthma.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Half of the world’s population is infected with <span class="elsevierStyleItalic">H. pylori</span>. However, prevalence varies depending on the region studied (<35% in the USA, Canada, Northern Europe and Australia; 35%–50% in Southern and Eastern European countries; 50%–70% in Mexico, India and China; and lastly >70% in Central and South America and Africa).<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Prevalence is directly associated with socio-economic development<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> and socio-cultural habits that increase the risk of transmitting this infection.</p><p id="par0125" class="elsevierStylePara elsevierViewall">To explain a “protective” effect of <span class="elsevierStyleItalic">H. pylori</span> infection in EoO and other allergic conditions, it has been proposed that infection in childhood may induce a redirection in the Th1 and Th2 inflammatory response. In the Th1 inflammatory reaction, helper lymphocytes produce inflammatory cytokines such as IL-2, interferon-γ and tumour necrosis factor (TNF)-α, which play a role in cell-mediated immunity. T lymphocytes, in contrast, act in Th2 inflammation, generating IL-4, IL-5 and IL-13, which contribute to the production of immunoglobulin E (IgE) and stimulate the eosinophil-mediated allergic response.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">In this context, it has been shown that the prevalence of asthma, allergic rhinitis and atopic dermatitis has increased over the last thirty years in developed countries, as has the prevalence of EoO.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> This has coincided with improving socio-economic conditions in countries, but has also been linked to the implementation of <span class="elsevierStyleItalic">H. pylori</span> eradication strategies, with the aim of reducing the incidence of peptic ulcer disease and gastric cancer.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> Some meta-analyses have found an inverse relationship between <span class="elsevierStyleItalic">H. pylori</span> and atopy,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">29,30</span></a> while others have not confirmed it.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> These differences may be explained by inconsistencies in the definition of atopic diseases and <span class="elsevierStyleItalic">H. pylori</span> infection. While our study found a significant inverse association between <span class="elsevierStyleItalic">H. pylori</span> and atopic diseases, the Molina-Infante study found only a marginal association.</p><p id="par0135" class="elsevierStylePara elsevierViewall">All the arguments discussed above reflect the complex relationship between <span class="elsevierStyleItalic">H. pylori</span>, EoO and allergic conditions. The putative role of <span class="elsevierStyleItalic">H. pylori</span> in the pathophysiology of EoO in experimental studies has not yet been determined in humans. Moreover, to date no study assessing the association between EoO and <span class="elsevierStyleItalic">H. pylori</span> has considered exposure to other risk factors (such as socio-economic status, ethnicity, transvaginal delivery, breastfeeding and non-lethal infections in childhood).<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> Furthermore, the role of oesophageal and gut microbiota alterations in the pathophysiology of EoO has not yet been determined.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a></p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Conclusions</span><p id="par0140" class="elsevierStylePara elsevierViewall">In conclusion, our study identified an inverse association between <span class="elsevierStyleItalic">H. pylori</span> infection and EoO, as well as with atopic diseases in general, particularly with allergic rhinitis and food allergy in Mexican patients. However, due to the study design it is not possible to define whether <span class="elsevierStyleItalic">H. pylori</span> has a “protective” factor against EoO or is just an epiphenomenon reflecting other undetermined factors. Experimental studies of the mechanisms of action of <span class="elsevierStyleItalic">H. pylori</span> on Th2-mediated immunity may be helpful in clarifying this relationship. Further prospective studies, with larger numbers of patients, are warranted. In addition, factors other than <span class="elsevierStyleItalic">H. pylori</span>, such as socio-economic status, ethnicity, environmental factors and changes in the microbiota, need to be considered in future trials. Retrospective studies have found an inverse association with <span class="elsevierStyleItalic">Helicobacter pylori</span> infection (<span class="elsevierStyleItalic">H. pylori</span>). A recent prospective study has questioned this relationship. We aimed to evaluate this relationship in Mexican patients.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Patients and methods</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">We evaluated adult patients without prior eradication of <span class="elsevierStyleItalic">H. pylori</span>. Cases were defined by the presence of esophageal symptoms and >15 eosinophils/high power field (HPF) in the esophageal biopsy. Controls were defined by the presence of <15 eosinophils/HPF in esophageal biopsy. <span class="elsevierStyleItalic">H. pylori</span> infection was defined by histology. Patients were matched by age and gender assigning four controls per case.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">We included 190 patients: 38 cases and 152 controls. Cases had higher frequency of atopy, dysphagia, food impaction, peripheral eosinophilia, and endoscopic EoE abnormalities. The overall prevalence of <span class="elsevierStyleItalic">H. pylori</span> was 63.6%. Cases had significantly lower prevalence of <span class="elsevierStyleItalic">H. pylori</span> than controls (36.8% vs. 70.4%, OR 0.21 95% CI 0.08–0.69, <span class="elsevierStyleItalic">p</span> = 0.001). Atopic patients had lower prevalence of <span class="elsevierStyleItalic">H. pylori</span> than non-atopic: 13.1% vs. 50.5% (OR 0.20, 95% CI 0.06–0.69, <span class="elsevierStyleItalic">p</span> < 0.001), particularly allergic rhinitis and food allergy.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">We observed an inverse relationship between <span class="elsevierStyleItalic">H. pylori</span> and EoE as well as atopy. Por lo que buscamos evaluar esta relación en pacientes mexicanos.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Pacientes y métodos</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">Evaluamos pacientes adultos sin erradicación previa de la infección por <span class="elsevierStyleItalic">H. pylori</span>. Los casos se definieron por la presencia de síntomas esofágicos y > 15 eosinófilos/campo de alto poder (CAP) en biopsias de esófago. Los controles, por la presencia de < 15 eosinófilos/CAP. La infección por <span class="elsevierStyleItalic">H. pylori</span> se estableció por histología. Los pacientes fueron pareados por edad y género, asignando cuatro controles por caso.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">Se incluyeron 190 pacientes: 38 casos y 152 controles. Los casos tuvieron mayor frecuencia de atopia, disfagia, impactación alimentaria, eosinofilia periférica y anormalidades endoscópicas de EEo. La prevalencia de la infección por <span class="elsevierStyleItalic">H. pylori</span> fue de 63,6%. Los casos tuvieron prevalencia significativamente menor que los controles (36,8 vs. 70,4%, odds ratio (OR) 0,21, intervalo de confianza (IC) 95% 0,08-0,69, p = 0,001). Los pacientes atópicos tuvieron menor prevalencia en comparación con aquellos sin atopia: 13,1 vs. 50,5% (OR 0,20, IC 95% 0,06-0,69, p < 0,001), particularmente con rinitis alérgica y alergia alimentaria.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusiones</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Observamos una relación inversa entre la infección por <span class="elsevierStyleItalic">H. pylori</span> y EEo así como con atopia.
