(A) Serum UA concentrations between groups; (B) frequency of hyperuricemia between groups.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Arterial hypertension (AH) is a chronic non-communicable disease classified by persistently elevated blood pressure (BP).<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">1</span></a> This illness is closely related to the development of cardiovascular diseases, such as acute myocardial infarction, heart failure, chronic kidney disease, and stroke.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">2</span></a> AH affects about 600,000 million people worldwide and, in Brazil, about 30% of individuals have the disease.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Several factors have already been described acting to influence the evolution of AH such as age, sex, ethnicity, genetics, and social and environmental conditions. In addition, the association between high UA levels and hypertension has been reported in several studies.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">UA is a by-product of the degradation of purines, which perform vital functions in the body. It can be synthesized endogenously or derived from dietary sources. When there are disturbances in its metabolism or failures in its excretion, levels of UA increase, leading to a clinical condition known as hyperuricemia, which is defined as UA levels greater than 7<span class="elsevierStyleHsp" style=""></span>mg/dL in men and 6<span class="elsevierStyleHsp" style=""></span>mg/dL in women.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">6</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Historically, serum UA was considered important in gout pathogenesis and kidney stones. However, since the 1800s some studies have shown elevations of UA in individuals with hypertension, obesity, metabolic syndrome, diabetes, chronic kidney disease, and heart disease.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">7</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">UA acts on the renin–angiotensin–aldosterone system and is capable of decreasing the synthesis of the neuronal nitric oxide synthase enzyme that produces nitric oxide (NO), a potent vasodilator, which is crucial for maintaining BP at physiological levels.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">8</span></a> In the last three decades, there has been an increase in discussions about hyperuricemia being an independent predictor of AH and whether it can, in fact, be a cause or only a marker of the pathophysiological process.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">9</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">It has been reported that the higher the UA levels, the more complex the control of BP, generating a risk of cardiovascular disease and increased mortality, especially in females.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">10</span></a> In contrast, lower serum UA concentrations could decrease the risk of prehypertension.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">4</span></a> Thus, in our study, we evaluated the levels of UA and other cardiovascular markers in prehypertensive and hypertensive adults and assess their risk for developing AH.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0035" class="elsevierStylePara elsevierViewall">The cross-sectional study was carried out in the city of Ouro Preto, Minas Gerais, Brazil, from February to November 2021. The sample size was defined using the OpenEpi program, version 3.01. Considering a prevalence of 48.4% of arterial hypertension in the city of Ouro Preto (Minas Gerais), design effect of 1, precision of 8% and confidence interval of 95%, the sample was estimated at 150 patients. We recruited 300 participants who were at the Clinical Analysis Laboratory (LAPAC) of the Federal University of Ouro Preto waiting in line to undergo laboratory tests previously requested by their doctor. We used the average of three BP measures to classify the volunteers as normotensive, prehypertensive, and hypertensive. Patients who had one or two BP measurements had no data in their medical records or were not registered in the Basic Health Units were excluded from the study. Thus, 157 participants aged between 29 and 79 years were included in the analysis, being 67 normotensive, 23 pre-hypertensive, and 67 hypertensives. No participants reported using urate-lowering therapy. The Research Ethics Committee of the Federal University of Ouro Preto (CAAE 22455119.0.0000.5150) approved the present study which was carried out in accordance with the Declaration of Helsinki and all participants signed an informed consent form.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Definition</span><p id="par0040" class="elsevierStylePara elsevierViewall">We classified participants according to their BP levels, following the most recent Brazilian guidelines.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">1</span></a> Participants who did not use any type of antihypertensive and had systolic blood pressure (SBP) lower than or equal to 129<span class="elsevierStyleHsp" style=""></span>mmHg and diastolic blood pressure (DBP) lower than or equal to 84<span class="elsevierStyleHsp" style=""></span>mmHg were considered normotensive. Prehypertension was defined as SBP greater than 130 and less than 139<span class="elsevierStyleHsp" style=""></span>mmHg and DBP between 85 and 89<span class="elsevierStyleHsp" style=""></span>mmHg in participants who were not using antihypertensive medication.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">1</span></a> Hypertension was defined as SBP greater than or equal to 140<span class="elsevierStyleHsp" style=""></span>mmHg and DBP greater than or equal to 90<span class="elsevierStyleHsp" style=""></span>mmHg, as well as any participant using at least one antihypertensive drug.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">We also classified participants according to their levels of UA into Q1 (≤4.3<span class="elsevierStyleHsp" style=""></span>mg/dL), Q2 (4.4–5.3<span class="elsevierStyleHsp" style=""></span>mg/dL), Q3 (5.4–6.2<span class="elsevierStyleHsp" style=""></span>mg/dL), and Q4 (≥6.3<span class="elsevierStyleHsp" style=""></span>mg/dL). Hyperuricemia was defined as UA greater than 6<span class="elsevierStyleHsp" style=""></span>mg/dL in women and 7.0<span class="elsevierStyleHsp" style=""></span>mg/dL in men.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">6</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Data collection and measurements</span><p id="par0050" class="elsevierStylePara elsevierViewall">To recruit participants, we randomly approached them in line while they waited for care and blood collection to carry out laboratory tests previously requested by their doctor. Those who were interested in participating in the project, after collecting the biological material after fasting for 8<span class="elsevierStyleHsp" style=""></span>h, were sent to a reserved room. Interviews were carried out using a questionnaire applied with the help of the Kobotoolbox application, which contained clinical history, lifestyle habits, sociodemographic characteristics and medication use. At the same time, anthropometric measurements of weight, height and waist circumference were collected using a stadiometer, bioimpedance scale (Tanita® – The Ultimate Scale Model 2204) and simple measuring tape, respectively. BP was measured once with a calibrated digital sphygmomanometer (G-TECH Home BSP11®). Subsequently, we also evaluated the records provided by the Basic Health Units to obtain the last two BP values. This allowed us to calculate the average of the three measurements for comparison and classify the groups as normotensive, pre-hypertensive and hypertensive.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Biochemical parameters</span><p id="par0055" class="elsevierStylePara elsevierViewall">Fasting glucose, triglycerides (TG), total cholesterol, high-density lipoprotein (HDL-c), low-density lipoprotein (LDL-c), urea, and creatinine were measured by UV/vis spectrophotometry using the Cobas Integra VR 400 Plus (Roche Diagnostics, Taguig City, Philippines®). An ion-selective electrode measured sodium and potassium ions with the AVL 9180 equipment (Roche Diagnostics®). We also calculated non-HDL cholesterol using the following equation: non-HDLc<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>TC (mg/dL)<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>HDL-c (mg/dL).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Cardiovascular risk</span><p id="par0060" class="elsevierStylePara elsevierViewall">Cardiovascular risk was assessed by calculating the atherogenic indices AIP (atherogenic index of plasma), VAI, and LAP. The AIP is equal to the log of the TG concentration divided by the HDL-c concentration, both in mmol/L.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">11</span></a> To calculate the VAI, we considered body mass index (BMI) (in kg/m<span class="elsevierStyleSup">2</span>), waist circumference (WC) (in cm), HDLc, and TG (both in mmol/L), with VAI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>[WC/36.58<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>(1.89<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>BMI)]<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>[TG/0.81]<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>[1.52/HDLc] (women) and VAI<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>[WC/39.68<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>(1.88<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>BMI)]<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>[TG/1.03]<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>[1.31/HDLc] (men).<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">12</span></a> The LAP was calculated based on the WC value (in cm) and the TG concentration (in mmol/L). LAP<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>[WC(cm)<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>58]<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>[TG(mmol/L)].<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">13</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Data analysis</span><p id="par0065" class="elsevierStylePara elsevierViewall">We performed an exploratory analysis of the data and obtained frequencies. Pearson's Chi-square test was used to compare categorical variables. Regarding quantitative data, we used the Kolmogorov–Smirnov test to assess normality. Normal data were expressed as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation and compared by ANOVA. Non-normal data were expressed through medians and interquartile range and compared by Kruskal–Wallis test, following Dunn's posthoc test. We also used multinomial logistic regression to determine the odds ratio (OR) for prehypertension and hypertension according to the UA quartiles. The statistical analyses were performed in SPSS version 20.0 and GraphPad Prism version 8.0.2. In all tests, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the sociodemographic and clinical characteristics of the study participants. In the normotensive group, participants were 55.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.0 years old on average, while participants in the prehypertensive and hypertensive groups were 59.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.2 and 56.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.6 years old, respectively (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.34). In all groups, most participants were black, reported illiteracy or primary education, family income lower than three salaries, did not currently use cigarettes or alcohol and did not practice physical activity. We found no significant differences among the groups in sociodemographic and behavior variables (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.05).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">As we used BP levels to define groups, we found a progressive increase as we passed from the normotensive group to the hypertensive group (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The median value of SBP was 131<span class="elsevierStyleHsp" style=""></span>mmHg in the normotensive group, 144<span class="elsevierStyleHsp" style=""></span>mmHg in the prehypertensive, and 149<span class="elsevierStyleHsp" style=""></span>mmHg in the hypertensive group. The statistical analysis showed a significant difference between the normotensive and the other groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). Regarding DBP, we observed the same tendency of increase in the mean values: 81<span class="elsevierStyleHsp" style=""></span>mmHg, 86<span class="elsevierStyleHsp" style=""></span>mmHg, and 92<span class="elsevierStyleHsp" style=""></span>mmHg in the normotensive, prehypertensive, and hypertensive groups, respectively. We did not find significant differences between the normotensive and prehypertensive groups in any anthropometric and biochemical data (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0085" class="elsevierStylePara elsevierViewall">We assessed UA levels in the three study groups (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). We found progressively higher concentrations of UA (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A) and a higher frequency of hyperurucemia (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B) when analyzing the normotensive (4.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.3<span class="elsevierStyleHsp" style=""></span>mg/dL; 13.4%), prehypertensive (5.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.3; 26.1%), and hypertensive (5.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.6<span class="elsevierStyleHsp" style=""></span>mg/dL; 34.3%) groups. For both variables, statistical analysis showed differences between the normotensive and hypertensive groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05, respectively).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the OR for prehypertension and hypertension according to the quartiles of UA. Participants allocated in Q2 had an OR of 3.17 (CI 95%: 1.17–8.59; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.024) for hypertension while participants in Q4 had an OR of 4.77 (CI 95%: 1.70–13.43; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) compared with volunteers classified in Q1. Regarding prehypertension, the statistical analyses displayed no significant differences.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">To improve our understanding of the relationship between UA levels, prehypertension, and hypertension, we evaluated BP and other cardiovascular risk markers according to UA serum levels (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). Participants with higher UA levels (Q4) had significantly higher DBP than volunteers classified in Q1 (91<span class="elsevierStyleHsp" style=""></span>mmHg vs. 82<span class="elsevierStyleHsp" style=""></span>mmHg; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.009). Regarding the other cardiovascular markers, we found that volunteers allocated in Q1 had higher HDL-c (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and lower BMI (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003), WC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), VAI (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), and LAP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) than participants in Q2, Q3, and Q4. Additionally, we found that participants in Q3 (133<span class="elsevierStyleHsp" style=""></span>mg/dL; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.035) and Q4 (138<span class="elsevierStyleHsp" style=""></span>mg/dL; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) had higher levels of TG than those classified in Q1 (96<span class="elsevierStyleHsp" style=""></span>mg/dL).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">Hypertension is a multi-causal disease often associated with metabolic disorders, and functional and/or structural changes in target organs. It can be aggravated by the presence of risk factors, such as age, sex, ethnicity, family history of hypertension, frequent drinking, diabetes, overweight or obesity, and hyperuricemia.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">2,14</span></a> Although some studies have reported an association between UA and hypertension, there is still a lack of evidence regarding prehypertension and data in specific populations, such as Brazilian adults.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">15</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">In this study, we found that hypertensive volunteers had lower HDL-c and higher BMI, WC, TG, LAP, and VAI than normotensive and prehypertensive subjects. Excessive fat favors the synthesis of cytokines, reactive oxygen species, and generates inflammation.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">16</span></a> Additionally, high TG levels generate an inflammatory response in the endothelium, transforming it into LDL-c.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">17,18</span></a> Similarly, the increase in FBG levels triggered by insulin resistance causes endothelial dysfunction of the artery causing a decrease in the synthesis of nitric oxide favoring the elevation of BP. The endothelium produces endothelin and as the synthesis of NO is compromised, vasoconstrictor actions stand out.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">19</span></a> This set of alterations contributes to the elevation of free radicals, generating inflammation, vascular remodeling, unregulated cell proliferation, and increased arterial stiffness, promoting AH.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">17,18</span></a> Several studies have also described deteriorating metabolic parameters in hypertensive patients.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">20–23</span></a> Regarding LAP and VAI, it was expected to have higher values in hypertensive participants, since the values of BMI, WC, and TG were also higher, and HDL-c was lower in those individuals.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Studies have shown that uncontrolled level of UA is associated with AH and other cardiovascular disorders due to its ability to promote endothelial dysfunction and vascular remodeling, thereby increasing blood vessel resistance.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">22</span></a> In our study, we evaluated the levels of UA and other cardiovascular markers in normotensive, prehypertensive, and hypertensive individuals aiming to know the influence of such markers on AH.</p><p id="par0115" class="elsevierStylePara elsevierViewall">We found that the levels of UA and the frequency of hyperuricemia increased linearly in the normotensive, prehypertensive, and hypertensive groups. Additionally, we found that individuals classified in the highest quartile of UA (≥6.3<span class="elsevierStyleHsp" style=""></span>mg/dL) had an OR of 4.77 to develop hypertension compared with individuals in the first quartile (≤4.3<span class="elsevierStyleHsp" style=""></span>mg/dL). Studies have shown that UA plays an important role in increasing BP, thereby increasing the frequency of hypertension.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">15,24</span></a> The mechanisms involved in this process include oxidative stress, the proliferation of vascular smooth muscle cells, endothelial dysfunction, decreases in the availability of NO, and activation of the renin–angiotensin system.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">9,25</span></a> A recent systematic review and meta-analysis showed that uric acid-lowering therapies may have a beneficial effect on BP, which strengthens the evidence that UA may influence hypertension pathogenesis and progression.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">26</span></a> Other studies have also shown an association between high UA levels and hypertension in adults.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">4,27</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Like the hypertensive group, prehypertensive individuals in this study also had higher levels of UA and a higher frequency of hyperuricemia compared with the normotensive group. Moreover, participants with serum concentrations of UA greater than or equal to 6.3<span class="elsevierStyleHsp" style=""></span>mg/dL had an OR of 1.95 to have prehypertension. However, statistical analysis did not show significant differences between the prehypertensive and the other groups. This may have occurred due to the reduced sample size in the prehypertensive group. We chose to classify the participants by the average of three BP measurements collected on different days. As a result, we were able to obtain more accurate results. However, this also meant that we were not able to obtain three measurements from all participants initially recruited, which resulted in a smaller sample size. Other studies had a higher sample size.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">28</span></a> In contrast, they only measured BP once, which may have influenced the results. Although the statistical analysis has not shown significant differences between groups, our results are interesting, since UA levels in prehypertensive individuals may represent an important target to prevent the development and progression of hypertension.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Aiming for a more complete understanding of the association between UA, hypertension, and cardiovascular disease, we evaluated BP and other cardiovascular markers according to the quartiles of UA. We calculated VAI and LAP since they are simple and low-cost indexes, which have been used to predict metabolic syndrome and cardiovascular risk in a wide range of populations, including hypertensive patients.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">12,13,29</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Our results showed that subjects with higher UA serum concentrations had significantly lower HDL-c and higher BP, BMI, WC, TG, VAI, and LAP than individuals with lower UA levels. UA can affect adipocytes and reduce the production of adiponectin, contributing to accelerated lipogenesis and chronic low-grade inflammation. Moreover, the oxidative stress generated by high levels of UA is also associated with dyslipidemia, which justifies our results.<a class="elsevierStyleCrossRefs" href="#bib0335"><span class="elsevierStyleSup">30,31</span></a> Our data confirm findings that showed the relationship between serum UA and increased BMI, hypertension, and dyslipidemia in adults.<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">32,33</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Taken together, our results support the evidence that relates UA to cardiovascular disease, showing that UA is not only associated with increased BP but also with other anthropometric and biochemical parameters that represent risk factors for hypertension and cardiovascular disease. Another study evaluating Brazilian adults also showed that higher UA levels were positively associated with BMI and TG.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">34</span></a> In addition, another recent work showed that high levels of UA increase the risk of unfavorable changes in WC, BP, and HDL, as well as hyperinsulinemia, insulin resistance, and metabolic syndrome in Brazilian children and adolescents.<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">35</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Despite the evidence showing the association between UA and hypertension, it is important to recognize that not all subjects with hypertension have an elevated UA. Likewise, there are individuals with hyperuricemia who do not present alterations in BP.<a class="elsevierStyleCrossRef" href="#bib0365"><span class="elsevierStyleSup">36</span></a> This observation corroborates the definition of hypertension as a multifactorial disease.<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">37</span></a> Thus, it is important to evaluate other genetic and environmental factors that may protect subjects from the AH and negative effects of UA.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Our study has some limitations. We had a small sample size and collected data from a specific Brazilian population of adults living in Ouro Preto, MG, which may not be representative of the Brazilian population. We did not perform a confounding factor adjustment, and the wide confidence intervals, along with the lack of significance in the analysis of the risk of hypertension in individuals in Q3, demand cautious interpretation of the data. Additionally, the study design allows us to make associations, but not a causal relationship between variables. Thus, further longitudinal studies must establish if UA is a cause or a consequence of AH, whether UA should be measured in all subjects at cardiovascular risk, as well as if lowering UA agents might represent an alternative to prevent or help in AH management.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusion</span><p id="par0150" class="elsevierStylePara elsevierViewall">In general, participants with UA levels lower than 5.3<span class="elsevierStyleHsp" style=""></span>mg/dL had lower BP and better anthropometric and biochemical parameters than individuals with higher concentrations. Considering the importance of BP and other parameters in hypertension and cardiovascular disease, further studies may review the UA cut-off to determine whether lower values might be appropriate for both the female and male populations aiming to reduce the risk of cardiovascular disease and its complications.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Authors’ contributions</span><p id="par0155" class="elsevierStylePara elsevierViewall">All of the authors discussed the results and approved the final manuscript. Therefore, the objective of this study was to determine the levels of UA and other cardiovascular markers among prehypertensive and hypertensive patients and assess their risk for developing arterial hypertension.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">157 individuals were recruited: 67 normotensive, 23 pre-hypertensive and 67 hypertensive. Blood samples were collected to measure biochemical parameters and anthropometric measurements and blood pressure were evaluated. We calculated the product of lipid accumulation and the visceral adiposity index to assess cardiovascular risk.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Our data showed an increase in UA levels in normotensives (4.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.3<span class="elsevierStyleHsp" style=""></span>mg/dL), prehypertensives (5.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.3<span class="elsevierStyleHsp" style=""></span>mg/dL) and hypertensives (5.9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.6<span class="elsevierStyleHsp" style=""></span>mg/dL) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004). We found a higher frequency of hyperuricemia in the hypertensive group (34.3%) than in the normotensive group (13.4%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05).
