During the first 5 years after the diagnosis of SLE, up to 60% of the patients may develop kidney disease, and after 15 years, from 10 to 30% of the severe LN cases (WHO classification, class III or superior) progress to end stage renal disease (ESRD).<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Although we have improved dramatically the prognosis in those patients,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">3</span></a> LN still positions as the main predictor of mortality: 6 times more if compared to the general population and 26 times greater if left progress to ESRD.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Therefore, when we face a patient with a recent diagnosis of LN, the principal goal is to achieve disease remission (avoid progression to chronic renal failure)<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">5</span></a> and treat the metabolic side effects due to the therapy.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">6</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Many international studies correlate the diagnosis of LN with cardiovascular disease (CVD), identifying both risk and protective factors.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">7–11</span></a> Chronic renal disease (CRD) appears to be an independent risk factor for the development of CVD.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">12</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">We searched the literature and scarcely found evidence regarding the impact of inactive LN (with preserved renal function) in CVD morbimortality.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Objective</span><p id="par0030" class="elsevierStylePara elsevierViewall">To assess the prevalence of inactive LN and its influence in the cardiovascular (CV) risk profile in a cohort of SLE patients.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Patients and methods</span><p id="par0035" class="elsevierStylePara elsevierViewall">A total of 130 patients were identified and diagnosed with SLE, using the 1997 ACR criteria. Patient data were collected between May 2002 and April 2013 and registered in a database accessible through the Internet at the following web address: <a id="intr0005" class="elsevierStyleInterRef" href="http://www.registroles.es/">http://www.registroles.es</a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The patients were divided into 2 groups based on the presence of LN: Group 1 (with LN, 32 patients) and Group 2 (without LN, 98 patients).</p><p id="par0045" class="elsevierStylePara elsevierViewall">We reviewed the following variables: age, gender, duration of disease from diagnosis (months), SLE activity (SLEDAI) and chronicity (SLICC), complement C3 and C4, urea, creatinine and glomerular filtration rate (CKD-EPI), classical CVD risk factors, established CVD, family history of CVD, prevalence and age at menopause, presence of the antiphospholipid antibodies Lupus anticoagulant (LA) and anti-cardiolipin antibodies (ACA; IgG and IgM).</p><p id="par0050" class="elsevierStylePara elsevierViewall">Renal disease was considered when the glomerular filtration rate (GFR) was below 60<span class="elsevierStyleHsp" style=""></span>mL/min, and nephritis activity was defined as the presence of proteinuria and urinary casts.</p><p id="par0055" class="elsevierStylePara elsevierViewall">We examined the degree of therapeutic compliance and achievement of targets (primary prevention), early CVD events occurring in first-degree relatives (onset <55 years for men and <65 years for women), presence of hypertension (at least 2 readings with blood pressure >140/90<span class="elsevierStyleHsp" style=""></span>mmHg whilst seated or on treatment), diabetes mellitus (according to ADA guidelines; fasting glycemia >126<span class="elsevierStyleHsp" style=""></span>mg/dL or on treatment), dyslipidemia (total cholesterol >250<span class="elsevierStyleHsp" style=""></span>mg/dL, low-density lipoprotein cholesterol [LDL-C] >150<span class="elsevierStyleHsp" style=""></span>mg/dL, high-density lipoprotein cholesterol [HDL-C] <50<span class="elsevierStyleHsp" style=""></span>mg/dL in women and <40<span class="elsevierStyleHsp" style=""></span>mg/dL in men), triglycerides (>200<span class="elsevierStyleHsp" style=""></span>mg/dL or on treatment), smoking habit (considered active or former if quitted for the last 6 months), age at menopause (premature if it occurs before the age of 40), body mass index (<25<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>) and Framingham risk score (estimates risk of heart attack in 10 years).<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">10,13,14</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">We analyzed demographic, clinical and serological data, as well as outcome using SPSS v15.0 software. Continuous variables were described as a mean with standard deviation [SD] and categorical variables were described as percentages. Comparisons of means were made using <span class="elsevierStyleItalic">T</span>-Student test and Chi-square test or Fisher's exact two-tailed test were used for categorical variables when it was appropriate. We used the logistic regression analysis with statistical significant variables obtained in the univariate analysis. We considered to be statistical significant a <span class="elsevierStyleItalic">p</span>-value <0.05.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">The mean age (<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>130) at diagnosis was 37.8 years [SD 12.7]. The majority of patients were women (90%, 117 patients). The average follow-up was 92.3 months [SD 97.7] and the SLEDAI and SLICC score were 6.3 [SD 4.9] and 1.2 [SD 1.1], respectively. The C3 was 95.4<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 35.4] and C4 was 17.1<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 12.1]. Urea was 32.3<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 9.8], creatinine 0.77<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 0.17] and GFR was 100.1<span class="elsevierStyleHsp" style=""></span>mL/min [SD 20.4].</p><p id="par0070" class="elsevierStylePara elsevierViewall">The most common traditional CV risk factors were sedentary lifestyle, overweight/obesity and dyslipidemia, followed by smoking and hypertension, in comparison to diabetes mellitus, which was seldom reported (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). We did not observe differences in the percentage of patients with 3 or more vascular risk factors (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). The average Framingham risk score was 5.7 [SD 7] and in 21.5% of them was above 10.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Considering female patients, 23% reached the menopause (mean 43.6 years [SD 5.3]), being premature menopause in more than a fourth (26%). Regarding to the presence of antiphospholipid antibodies, LA was observed in 31.25% of the patients, ACA IgG in 19.6% and ACA IgM in 13.3%. 13.7% of the patients reported CV events and in 10.8% there was a family history of early CVD.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding to the management, 86% were receiving SLE treatment, with more pulses of cyclophosphamide (11.5%) than corticosteroids (7.8%). More than a half (53.1%) of the patients were taking CV medication (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), with higher prevalence of antihypertensive use in the group of patients with lupus nephritis. Nevertheless, a high percentage did not reach a therapeutic target value, particularly regarding obesity (11.5%) and cholesterol levels (LDL-C 15.7%) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Stratifying by age (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>; group A <35 years, B 35–44, and C >45), it is noteworthy that those with 45 years or older (group C) had higher systolic blood pressure, waist circumference, urea, homocysteine, hemoglobin, total cholesterol (higher LDL-C than group B), sedentary lifestyle, as well as, lower GFR and SLICC punctuation. We also noted a lower percentage of anti-DNA antibody (A 47.1%, B 28.6%, C 19.4%; <span class="elsevierStyleItalic">p</span> 0.045) and corticosteroid pulses (A 20%, B 4.8%, C 3.2%; <span class="elsevierStyleItalic">p</span> 0.016). Stratifying by the time of evolution of SLE (A-ev <5 years, B-ev 5–15 years, C-ev >15 years), we found the latter (group C-ev) higher Framingham risk score, total cholesterol, LDL-C, SLICC, and lower presence of anti-DNA antibody (A-ev 44%, B-ev 18.4%, C-ev 18.5%) and GFR. The presence of hypertension was higher than group B-ev, but similar to A-ev (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">As summary, we found more CV risk factors, chronicity (SLICC), lower GFR and time of evolution of SLE in elder patients, moreover, the presence of anti-DNA antibody and higher SLEDAI is more common in younger patients with less time of evolution.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Stratifying by the presence of LN, we found in this group (24.6%, 32 patients) a statistically significant higher prevalence of dyslipidemia, smoking habit, LVH, family history of early CVD, corticotherapy and use of cyclophosphamide, proteinuria, lower HDL-C and C4. In the group without LN (98 patients) the prevalence of sedentarism, ACA IgM and LA were higher, although we did not reach statistically significance (48.1% in the LN group vs 48.9 without LN, <span class="elsevierStyleItalic">p</span> 0.95).</p><p id="par0100" class="elsevierStylePara elsevierViewall">There were no significant differences between age, gender, previous CVD, hypertension and diabetes. Even though, a trend toward higher prevalence for CVD in the LN group could be observed (25.8% vs 15.3%) (<a class="elsevierStyleCrossRefs" href="#tbl0025">Tables 5 and 6</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><elsevierMultimedia ident="tbl0030"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Stratifying by age, we found that group A (<30 years) with LN, had received more pulses of corticosteroid and cyclophosphamide, higher prevalence of hypertension, HVI, proteinuria, waist circumference and more hospitalizations. Whereas group B (31–44 years) we found lower prevalence of sedentarism, and in C (>45 years), higher prevalence of dyslipidemia (85%) and levels of triglycerides (<a class="elsevierStyleCrossRef" href="#tbl0035">Table 7</a>). No other differences were found related to the presence or not of LN.</p><elsevierMultimedia ident="tbl0035"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">It is noteworthy, when comparing the prevalence of certain CV risk factors according to age and the presence of LN, most of them show a V-line due to its predominance in the younger group (A, <30 years), which is not detected by the Framingham risk index (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">Analyzing the presence of LN according to the time of evolution of SLE, compared to B-ev, we found in C-ev a higher levels of total cholesterol (167<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 38] vs 214.6<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 42.7], <span class="elsevierStyleItalic">p</span> 0.05), LDL-C (91.6<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 18] vs 133.3<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 28.1], <span class="elsevierStyleItalic">p</span> 0.001) and platelets (173<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/L [SD 54] vs 258<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span>/L [SD 58], <span class="elsevierStyleItalic">p</span> 0.016). Moreover, the C3 levels were lower in A-ev compared to B-ev (83<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 23.8] vs 105<span class="elsevierStyleHsp" style=""></span>mg/dL [SD 19.6], <span class="elsevierStyleItalic">p</span> 0.045).</p><p id="par0120" class="elsevierStylePara elsevierViewall">Considering the group without LN, we found in C-ev (>15 years) higher prevalence of hypertension (3.2% vs 35%, <span class="elsevierStyleItalic">p</span> 0.004), systolic blood pressure (SBP) (118<span class="elsevierStyleHsp" style=""></span>mmHg [SD 16] vs 127<span class="elsevierStyleHsp" style=""></span>mmHg [SD 15.3], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05), SLICC (1.17 [SD 0.8] vs 2.8 [SD 1.3], <span class="elsevierStyleItalic">p</span> 0.05) and Framingham risk index (4.1 [SD 4] vs 11.7 [SD 9], <span class="elsevierStyleItalic">p</span> 0.001) than in B-ev (5–14 years). GFR was lower in C-ev (88.8<span class="elsevierStyleHsp" style=""></span>mL/min [SD 19.4]) than A-ev and B-ev (101.7<span class="elsevierStyleHsp" style=""></span>mL/min [SD 19] and 100.8<span class="elsevierStyleHsp" style=""></span>mL/min [SD 16], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05). Group A-ev (<5 years), had higher SLEDAI than C-ev (7.44 [SD 4.9] vs 3.56 [SD 3.4], <span class="elsevierStyleItalic">p</span> 0.001) and less smoking habit than B-ev (17.8% vs 41.9%, <span class="elsevierStyleItalic">p</span> 0.035).</p><p id="par0125" class="elsevierStylePara elsevierViewall">After performing a logistic regression analysis, none of the studied variables (hypertension, diabetes, sedentarism, smoking, Framingham risk score – recoded as <5<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0 or >5<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1, dyslipidemia and LN) showed a predictive association with the previous CV events.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">Classical modifiable CV risk factors do not explain the tendencies for developing atherosclerotic disease in SLE. These patients had more CV morbimortality due to the conjunction of traditional risk factors, mainly dyslipidemia and hypertension, in association with risk factors associated with SLE (inflammatory cytokines, antibodies against lipoprotein lipase, anti-ox LDL antibodies, corticosteroids, etc.) that can accelerate atherosclerosis.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">15,16</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Some studies have found that SLE patients have higher Carotid intima-media thickness (CIMT) and atheromatous plaques.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">17–19</span></a> However, in patients with LN, it is found more often the presence of plaques and endothelial dysfunction due to FMV (flow mediated vasodilation), rarely seeing differences in CIMT.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">18–20</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Gustafsson et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">17</span></a> found a significant higher prevalence of CVD in SLE patients (12% vs 1%) compared to the control group, regarding the presence of atheromatous plaques, were more frequently found in LN patients (23% vs 11%), who were also younger (45 years vs 49). Hermansen et al.,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">21</span></a> in a Danish cohort, found in LN patients higher incidence of CV death and myocardial infarction, although not for stroke.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Therefore, the risk for CVD in SLE increases with age, as expected, but relatively higher in the group with LN.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">22,23</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">In our cohort, we found a significant higher prevalence of middle-aged female patients, with moderate-activity, low-chronicity SLE and a remarkably high prevalence of hypertension, with levels of target achievement for this and other CV risk factors inadequate in more than a half of the patients, even though they had a correct medication adherence.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The V-line distribution of some CV risk factors (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>), illustrates a higher risk in younger patients (<30 years) with LN, failed to be detected with classical scores such as the Framingham risk index, which results in overlooking the risk in this younger subgroup. The group without LN, as expected, there is an increase in CVD associated with aging.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Thus, our results, consistent with other international studies,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">24</span></a> confirm that LN patients have an increased risk of CVD, which could partially attributed to the unsatisfactory and more difficult CV risk control achievement. Moreover, the risk increases with the time of evolution of SLE, for instance, the prevalence of dyslipidemia is estimated to be between 36% (time of diagnosis) and 60% (3 years after diagnosis).<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a> Hypertension is significantly prevalent in patients with LN, at least 53.1% of the patients who had suffered a LN episode remain hypertensive even after restoration of renal function.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">25</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">A strategy of treating to target can be considered, according to the presence of established CVD, the number of CV risk factors, and the need to avoid the abuse of high doses of corticosteroids, due to its metabolic side effects. Moreover, this strategy must take into account the dynamic and evolving CV risk profile, not only the pharmacologic prevention and treatment, but also the lifestyle changes and smoking cessation.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusion</span><p id="par0170" class="elsevierStylePara elsevierViewall">The prevalence of CV risk factors in LN patients is considerably high and its control still inadequate. They would require a closer therapeutic management, in order to elude ESRD progression and CV morbidity and mortality.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Although a trend could be observed, we did not find differences in CV events between the two groups (with or without LN), most likely due to an insufficient follow-up period. The early recognition and remission of flares, while trying to avoid the metabolic adverse effects of medication, must be mandatory.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Aim</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">The aim of our study was to assess the cardiovascular (CV) risk profile in a cohort of lupus patients with preserved kidney function after a nephritis episode, compared to patients without a nephritis flare.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">130 patients diagnosed of SLE (32 with previous nephritis flare and 98 without) were studied in order to evaluate the CV risk profile, despite the preserved kidney function.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">The most prevalent risk factors were sedentary lifestyle (57.6%), overweight/obesity (38.3%) and dyslipidemia (36%), followed by smoking (32%) and hypertension (16%). Though more than a half (53.1%) was taking CV medication, a high percentage did not reach a therapeutic target value, especially regarding obesity (11.5%) and cholesterol levels (LDL-C of 16%). The prevalence of dyslipidemia (53.1% vs 30.6%), smoking (46.6% vs 27.5%), left ventricular hypertrophy (LVH) (21.4% vs 6.4%) and lower HDL-C (48.6<span class="elsevierStyleHsp" style=""></span>mg/dL vs 55.4<span class="elsevierStyleHsp" style=""></span>mg/dL) were significantly different in the group with previous nephritis flare.
