Paciente de 28 años con antecedentes de hipertensión arterial que ingresa por la aparición de una tumoración mastoidea derecha. Durante la realización de una punción-aspiración de dicha masa tuvo una crisis hipertensiva. El estudio posterior dirigido mostró elevación del ácido vanilmandélico en orina y la aparición de una tumoración retroperitoneal. El diagnóstico final fue feocromocitoma extraadrenal maligno con metástasis hepáticas y óseas. Fue tratado con quimioterapia y posteriormente con 131I-metayodobenzilguanidina a dosis terapéuticas con mala evolución.
A 28-year-old male with a history of arterial hypertension, who was admitted to hospital due to the appearance of a tumor in the right mastoid. While undergoing an aspiration puncture of the above-mentioned mass, he had a hypertensive crisis. A later indicated study showed elevated levels of vanil mandelic acid in his urine and the appearance of a retroperitoneal tumor. The final diagnosis was malignant extra-adrenal pheochromocytoma with hepatic and bone metastases. It was treated with chemotherapy, and later with therapeutic doses of 131I-methyl-iodobenzyl- guanidine with poor evolution.
