Presentamos un caso de coartación aórtica abdominal en adulto joven que había estado asintomático hasta que comienza con claudicación en miembros inferiores. Se realiza historia clínica y exploración física, objetivándose cifras elevadas de presión arterial con pulsos femorales muy débiles. Se remite al paciente a urgencias de hospital, donde confirman por tomografía axial computarizada la atresia de aorta abdominal. Desde ese momento hasta la cirugía mantuvo cifras de presión arterial elevadas a pesar del tratamiento farmacológico. El tratamiento quirúrgico resolvió la claudicación de miembros inferiores y las cifras de presión arterial dentro de la normalidad con ayuda de antihipertensivos.

Planteamos la importancia de realizar un abordaje de los factores de riesgo cardiovascular en las consultas de Atención Primaria para detectar casos de hipertensión arterial secundaria que requieren un tratamiento específico.

We present a case of abdominal aortic coartation in a young adult who had been asymptomatic until the patient began with claudication in the lower limbs. A clinical history was obtained and physical examination done, observing elevated blood pressure values with very weak femoral pulses. The patient was referred to the hospital Emergency Service where ataresia of the abdominal aorta was confirmed. From this time until surgery, the patient had elevated blood pressure values in spite of drug treatment. Surgical treatment resolved the claudication of the lower limbs and the blood pressure values within normality with the help of antihypertensive drugs.

We suggest the importance of performing an approach to the cardiovascular risk factors in the primary health care consultations to detect cases of secondary high blood pressure that require a specific treatment.
