Existen pruebas convincentes de que el bloqueo del sistema renina resulta útil en muchos pacientes con hipertensión. Dos recientes líneas de investigación indican que un bloqueo más completo produce mejores resultados clínicos. En una se utiliza una combinación de un inhibidor de la enzima conversora de angiotensina con un bloqueador de los receptores de angiotensina (BRA). En la segunda se utilizan dosis muy elevadas de BRA. La interacción de la renina con el sustrato es el paso limitante de la velocidad en la cascada de la renina; así pues, el reciente desarrollo de un potente inhibidor de la renina también favorece un bloqueo más completo del sistema, lo que probablemente conducirá a una mejora del tratamiento.

Powerful evidence indicates that renin system blockade is useful in many patients with hypertension. Two recent lines of investigation suggest that more complete blockade leads to improved clinical outcomes. One involves the use of a combination of an angiotensin- converting-enzyme inhibitor with an angiotensin- receptor-blocker (ARB). The second involves the use of very high dose ARB. The interaction of renin with substrate is the rate-limiting step in the renin cascade: Thus, the recent development of a powerful renin inhibitor also favors more complete blockade of the system, which likely to lead to improved treatment.
