Adherence of immigrant patients to group therapy" ] ] "textoCompleto" => "<p class="elsevierStylePara">Es conocido el incremento en el número de personas inmigrantes en España y de los movimientos de población en grandes zonas del mundo. El acceso de estas personas al sistema sanitario español también aumenta, y plantea cambios en los problemas de salud. No sabemos si los instrumentos terapéuticos que tenemos son igualmente efectivos en ellos.</p><p class="elsevierStylePara">En el campo de la salud mental, hay evidencias de que los pacientes inmigrantes presentan tipos distintos y prevalencias menores en diversos trastornos mentales que los pacientes nativos. Incluso éstos son distintos de los que presentan las personas de su misma raza, pero que son hijos de inmigrantes y que han nacido en el país de acogida<span class="elsevierStyleSup">1</span>. Asimismo, la raza y la religión de estas personas<span class="elsevierStyleSup">2,3</span> tendrán una influencia creciente en los tratamientos psicoterapéuticos.</p><p class="elsevierStylePara">En el caso del tratamiento del alcoholismo, se ha buscado tratamientos específicos para grupos de pacientes con características homogéneas<span class="elsevierStyleSup">4</span>. Para su éxito, es determinante el cumplimiento y la adherencia terapéutica<span class="elsevierStyleSup">5</span>. Ello es igualmente cierto en el caso de la terapia grupal (TG), que es uno de los instrumentos terapéuticos más utilizados y que, en los últimos años, empieza a demostrar su eficacia con evidencias científicas<span class="elsevierStyleSup">6</span>.</p><p class="elsevierStylePara">La adherencia a la TG permite aumentar las tasas de abstinencia<span class="elsevierStyleSup">7</span>. Desde el punto de vista del funcionamiento grupal, mejorar la adherencia de los integrantes, dirigida a la consecución de unos objetivos de abstinencia, pasa por aumentar la capacidad que éstos tienen de comunicar vivencias y sentimientos. Para ello, debe haber cohesión en el grupo<span class="elsevierStyleSup">8</span>, que se da a causa de la similitud de los problemas que tienen en común<span class="elsevierStyleSup">6</span>.</p><p class="elsevierStylePara">Pero también debe haber cierta homogeneidad entre los miembros sobre aspectos psicológicos, como las características de personalidad, aspectos orgánicos (como las enfermedades asociadas) y aspectos evolutivos (como la fase en que se encuentran en su proceso de deshabituación alcohólica)<span class="elsevierStyleSup">6</span>.</p><p class="elsevierStylePara">Otros criterios de homogeneidad históricamente potenciados para la realización de la TG han sido los sociodemográficos: el estado civil, la edad y el sexo.</p><p class="elsevierStylePara">A pesar de ello, recientemente se ha revelado errónea la habitual creencia de que las mujeres no se integran en los grupos de terapia mixtos<span class="elsevierStyleSup">7,9</span> o que la realización de grupos de terapia específicos para jóvenes mejoraba su adherencia<span class="elsevierStyleSup">10</span>. Todo ello plantea que no siempre la homogeneidad es el factor determinante del buen funcionamiento de las TG.</p><p class="elsevierStylePara">Por todo ello, el objetivo del presente estudio ha sido investigar si la procedencia etnográfica, cultural y lingüística de los pacientes alcohólicos puede afectar a su adherencia en los grupos de terapia.</p><p class="elsevierStylePara"><span class="elsevierStyleBold">Pacientes y método</span></p><p class="elsevierStylePara">Se ha incluido inicialmente las 1.856 primeras indicaciones consecutivas de TG realizadas a pacientes alcohólicos tratados en la Unidad de Alcohología del Hospital Clínic de Barcelona (UA-HCB) desde el 1 de enero de 1987 hasta el 30 de diciembre de 2000.</p><p class="elsevierStylePara">Una vez eliminados los 255 pacientes de los que no se disponía de datos completos y objetivos sobre su lugar de nacimiento, se ha seguido la cohorte de los 1.601 pacientes de los que sí se disponía de esos datos. La comparación de estos 1.601 pacientes con lugar de nacimiento conocido y los 255 restantes no reveló diferencias en ninguna variable sociodemográfica o clínica, lo que, por tanto, apoyaba la representatividad de los 1.601 pacientes respecto del total.</p><p class="elsevierStylePara">Se incluyó a los pacientes que cumplían los criterios DSM-III, DSM-III-R o DSM-IV de dependencia al alcohol y a los que sus terapeutas individuales indicaron la realización de una TG.</p><p class="elsevierStylePara">Se excluyó a los pacientes en los que la TG era una reindicación en la misma UA-HCB, además de los pacientes con marginación social, déficits cognitivos o sensoriales, dificultades de comunicación o comprensión, trastornos psiquiátricos graves que podían interferir en el funcionamiento de la dinámica grupal o los que tenían un consumo activo de alcohol u otras drogas, excepto tabaco<span class="elsevierStyleSup">6,7</span>.</p><p class="elsevierStylePara">Se estudió el cumplimiento y el tiempo de asistencia a la TG hasta su finalización, que puede ser: <span class="elsevierStyleItalic">a)</span> alta terapéutica, aproximadamente a los 2 años de iniciado el tratamiento; <span class="elsevierStyleItalic">b)</span> abandono de las sesiones, calculándose el tiempo de seguimiento hasta la última asistencia; <span class="elsevierStyleItalic">c)</span> baja justificada por causas justificadas y acordadas con los terapeutas, y <span class="elsevierStyleItalic">d)</span> inasistencia a ninguna sesión grupal.</p><p class="elsevierStylePara"><span class="elsevierStyleBold">Análisis estadístico</span></p><p class="elsevierStylePara">Para la descripción de los pacientes se ha utilizado frecuencias, medias y desviaciones típicas, se ha empleado la t de Student para la comparación de dos medias, el análisis de la variancia (ANOVA) en caso de comparar más de dos medias y la *<span class="elsevierStyleSup">2</span> para las comparaciones entre variables cualitativas.</p><p class="elsevierStylePara">Se ha efectuado un análisis de la supervivencia en el tratamiento de TG hasta su finalización según el método de Kaplan-Meier, considerando el abandono como el episodio analizado y los pacientes dados de baja por causas justificadas, como casos censurados. La comparación de las diferencias en el mantenimiento en tratamiento de TG según el lugar de nacimiento, ajustando por edad, se ha realizado mediante modelos de riesgos proporcionales de Cox.</p><p class="elsevierStylePara"><span class="elsevierStyleBold">Resultados</span></p><p class="elsevierStylePara">El 72,1% de los pacientes eran varones. La tabla 1 muestra las edades, los lugares de nacimiento de los pacientes y el tipo de finalización de TG. El 16,6% eran solteros; el 60,9%, casados o con pareja; el 15,2%, separados o divorciados y el 1,9%, viudos.</p><p class="elsevierStylePara"><img src="2v126n07-13085284tab01.gif"></img></p><p class="elsevierStylePara"><span class="elsevierStyleItalic">Fig. 1. Comparación del tiempo hasta el abandono de la terapia grupal (TG) en función del lugar de nacimiento de los pacientes.</span></p><p class="elsevierStylePara"><img src="2v126n07-13085284tab02.gif"></img></p><p class="elsevierStylePara">No se ha detectado diferencias estadísticamente significativas en cuanto al estado civil o los porcentajes varón/mujer según el lugar de nacimiento, pero en la tabla 1 se muestra que la edad es mayor en los pacientes nacidos en Cataluña (excepto Barcelona) y el resto de España que en los nacidos en el extranjero (media de edad en el resto de Europa, América y África en conjunto, 41,9 [9] años) o en la provincia de Barcelona (ANOVA, F = 9,2; p = 0,000). Sin embargo, también hay diferencias de edad entre los extranjeros, de los que los de origen europeo son similares a los españoles.</p><p class="elsevierStylePara">También se asigna un porcentaje superior de pacientes extranjeros y de Barcelona (24,6 y 23,7% de los pacientes, respectivamente) a grupos específicos para jóvenes que de los nacidos en el resto de Cataluña o de España (11,1 y 13,9%, respectivamente) (*<span class="elsevierStyleSup">2</span> = 24,3; p = 0,000).</p><p class="elsevierStylePara">En la tabla 1 se indica los porcentajes de cada tipo de finalización de la TG en función del lugar de nacimiento. En esta tabla se evidencia que los porcentajes de altas terapéuticas son significativamente más bajos entre los pacientes que provienen de América o África (*<span class="elsevierStyleSup">2</span> = 28; p = 0,02).</p><p class="elsevierStylePara">En la figura 1 se compara la supervivencia acumulada en función del lugar de nacimiento. En ella, se aprecia diferencias significativas (<span class="elsevierStyleItalic">log rank</span> = 17,9; p = 0,0004): los pacientes que nacen en Cataluña, a excepción de la provincia de Barcelona, son los que tienen más adherencia a la TG, y se mantienen más días de media en el grupo (media de 412 [40] días), mientras que los nacidos fuera de España son los que siguen menor número de días la TG (media de 212 [34] días). Los pacientes de la provincia de Barcelona, incluida la capital (media de 351 [10] días) y los del resto de España (media de 320 [14] días) están en una situación intermedia.</p><p class="elsevierStylePara">Aunque la edad sea un factor que influye en la adherencia a la TG, cuando se repite el análisis de supervivencia ajustando por la edad, que se introduce como covariante según el método de Cox, la ecuación obtenida muestra que el riesgo relativo instantáneo de abandonar la TG sigue siendo muy superior en los pacientes extranjeros (razón de probabilidad = 1,81; intervalo de confianza del 95%, 1,33-2,45; p = 0,000).</p><p class="elsevierStylePara"><span class="elsevierStyleBold">Discusión</span></p><p class="elsevierStylePara">Los datos muestran con claridad que los pacientes alcohólicos originarios de otros países tienen muy pocas posibilidades de vincularse correctamente a una TG. Abandonan más y de forma más rápida la TG y sólo en contados casos son capaces de lograr el alta terapéutica.</p><p class="elsevierStylePara">A pesar de que los abandonos son mucho más frecuentes en todos los pacientes nacidos fuera de España, los europeos se adaptan mucho mejor que los originarios de Sudamérica y África. No disponemos de información concluyente acerca de si ello puede deberse a factores raciales, idiomáticos o socioculturales.</p><p class="elsevierStylePara">Los miembros de los grupos de terapia intentan tener una actitud especialmente acogedora y comprensiva con los inmigrantes que se incluyen en el grupo. Esta actitud es potenciada por los terapeutas. Sin embargo, la desconfianza y la hostilidad con frecuencia parece aparecer precisamente entre las personas extranjeras.</p><p class="elsevierStylePara">Podemos especular acerca de si tienen la sensación de ser más observados, o si culturalmente no aceptan hablar de problemas personales en situaciones grupales, o si hay dificultades idiomáticas de comprensión, pero todas estas posibilidades habían sido evaluadas y descartadas previa y específicamente, existiendo además una conformidad y compromiso explícitos del interesado.</p><p class="elsevierStylePara">Otra posibilidad sería que, por la mayor inestabilidad laboral y residencial, las personas extranjeras tiendan más bien a buscar soluciones urgentes y puntuales a sus problemas, más que a incluirse en programas de seguimiento a largo plazo. Esta opción podría investigarse estudiando si los extranjeros acuden con una frecuencia distinta de la de los nativos a los servicios de urgencia o a los servicios ambulatorios: más en los primeros y menos en los segundos.</p><p class="elsevierStylePara">De cualquier modo, los datos presentes sugieren que es más importante la integración social que el idioma o la cultura, ya que los pacientes sudamericanos hispanohablantes cumplen mucho menos que los centroeuropeos o nórdicos y, en cambio, su comprensión lingüística nunca ha sido un problema en los grupos de terapia.</p><p class="elsevierStylePara">Finalmente, está el hecho de que los inmigrantes son más jóvenes que los nativos y que en estudios realizados previamente se indica que la menor edad interfiere en el cumplimiento de la TG<span class="elsevierStyleSup">10</span>. Esta mayor juventud podría explicar parte de sus dificultades de adherencia grupal y un menor tiempo para integrarse. Sin embargo, cuando se elimina el efecto de la edad, se mantiene el menor número de días de seguimiento.</p><p class="elsevierStylePara">Por otra parte, los nacidos en Barcelona tienen una edad similar a la de los extranjeros en conjunto y no presentan estos problemas. Y, si distinguimos a los inmigrantes entre europeos y no europeos, vemos que los primeros tienen una edad similar a los nacionales y, en cambio, abandonan más a menudo la TG. Los nacidos fuera de Europa sí que son mucho más jóvenes, y su cumplimiento de la TG acostumbra a ser bajísimo.</p><p class="elsevierStylePara">Todo lo anterior indica que el origen de las dificultades de adaptación está más en los propios individuos que en la composición, la actitud, la estructura y la dinámica de los grupos de terapia.</p><p class="elsevierStylePara">Se podría plantear la realización de grupos específicos para pacientes inmigrantes, pero la experiencia con los grupos para jóvenes sugiere la poca efectividad del esfuerzo<span class="elsevierStyleSup">10</span>, además de que estaríamos potenciando la exclusión y disminuyendo las posibilidades de integración. Es difícil hallar soluciones para mejorar el cumplimiento de la TG en pacientes alcohólicos extranjeros, cuando los propios beneficiarios están poco interesados en ellas y tienen poco interés en integrarse en los programas de tratamiento. Sus intereses y preocupaciones probablemente van en otras direcciones, como el trabajo, la familia y la vivienda, lo que, unido a la habitual falta de conciencia de su enfermedad, favorece el abandono terapéutico.</p><p class="elsevierStylePara">Es posible que si estos grupos de población van asentándose de una forma más estable, logren mejorar el aprovechamiento de los tratamientos a largo plazo, tal y como lo están haciendo los pacientes de origen europeo. El hecho de que los europeos de edad similar que los españoles tengan un cumplimiento más parecido y que los nacidos en América o África sean más jóvenes y abandonen muy rápido el TG apoya esta idea.</p><p class="elsevierStylePara">La conclusión es que, actualmente, en el tratamiento del alcoholismo, el tipo de recursos terapéuticos que parecen demandar los pacientes extranjeros, especialmente los más jóvenes, es el destinado a soluciones rápidas, como las primeras visitas, desintoxicaciones, tratamiento de las intoxicaciones en urgencias o el contacto rápido con recursos sociales, pero no su vinculación con programas de deshabituación continuada.</p><p class="elsevierStylePara">Es muy posible que las características de urgencia puntual de las demandas y de escaso cumplimiento de los tratamientos a medio o largo plazo por parte de los inmigrantes se repitan en otros ámbitos sanitarios. La homogeneidad es un factor importante para esta cohesión. En este estudio se valora si la TG tiene la misma eficacia en los pacientes inmigrantes que en los nativos. Pacientes y método: Las 1.856 primeras indicaciones consecutivas de TG para pacientes alcohólicos se han seguido hasta el alta o el abandono prematuro del tratamiento. Se compara el tiempo en tratamiento y tipo de abandono en función del lugar de nacimiento. Resultados: Finalizan la TG el 31% de los pacientes nacidos en Barcelona, el 39% de los nacidos en Cataluña, el 27% de España y el 14,5% de los nacidos en el extranjero (20% en Europa, 4% en América y ninguno de los nacidos en África [p = 0,02]). La probabilidad media de mantenerse en la TG es, respectivamente, de 351, 412, 320 y 212 días (p = 0,000). Discusión: Los pacientes nacidos fuera de España tienen pocas posibilidades de vincularse a una TG, y la abandonan con mayor frecuencia y más rápido que los otros pacientes. Increasing numbers of foreigners are accessing alcohol treatment facilities, but efficacy of TG in those patients has not been studied. This work focuses on the influence of ethnicity and language in the compliance of GT. Patients and method: 1856 consecutive patients who were referred to GT for alcohol dependence were followed until discharge or dropout. Time on treatment and final endpoint were compared according to place of birth. Results: 31% of patients born in Barcelona, 39% born in Catalonia, 27% from the rest of Spain and 14.5% of foreigners (20% from Europe, 4% from America and none born in Africa (p = 0.02) completed successfully GT. The probability to stay in GT was 351, 412, 320 and 212 days (p = 0.000), respectively Discussion: Patients born abroad have lower chances to complete GT successfully, presenting with higher dropout rates and shorter treatments. Fundamento y objetivo: El número de extranjeros aumenta en España y con ello el acceso a los servicios sanitarios y los tratamientos por el alcohol. La terapia grupal (TG) es un elemento básico en el tratamiento de la dependencia alcohólica en todo el mundo, que, para su buen funcionamiento, requiere cohesión grupal. La homogeneidad es un factor importante para esta cohesión. En este estudio se valora si la TG tiene la misma eficacia en los pacientes inmigrantes que en los nativos. Pacientes y método: Las 1.856 primeras indicaciones consecutivas de TG para pacientes alcohólicos se han seguido hasta el alta o el abandono prematuro del tratamiento. Se compara el tiempo en tratamiento y tipo de abandono en función del lugar de nacimiento. Resultados: Finalizan la TG el 31% de los pacientes nacidos en Barcelona, el 39% de los nacidos en Cataluña, el 27% de España y el 14,5% de los nacidos en el extranjero (20% en Europa, 4% en América y ninguno de los nacidos en África [p = 0,02]). La probabilidad media de mantenerse en la TG es, respectivamente, de 351, 412, 320 y 212 días (p = 0,000). Discusión: Los pacientes nacidos fuera de España tienen pocas posibilidades de vincularse a una TG, y la abandonan con mayor frecuencia y más rápido que los otros pacientes. La mala adherencia de los pacientes procedentes de Sudamérica descarta que la causa sea idiomática. La mejor adherencia de los europeos y su mayor edad sugiere que la explicación del abandono de la TG tiene que ver con la baja integración social y los objetivos personales a corto plazo.
