The patient consulted due to extreme fatigue and mild bilateral leg edema attributed to massive tricuspid regurgitation (TR) secondary to annular dilatation. However, the symptom that bothered him the most was due to erythromelalgia which caused intense pain in both feet preventing her from walking. This clinical condition was refractory to all types of analgesic treatment and even baths with ice water (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). After refusing cardiac surgery and given her intense fatigue, a percutaneous transcatheter repair was performed with the Triclip system (Abbott Medical) with a reduction of TR from massive to mild. The patient reported an immediate clinical improvement after the procedure with practically complete disappearance not only of fatigue but also of erythromelalgia (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Erythromelalgia is a rare disorder consisting of vasodilation of the small arteries in the feet or hands, causing burning pain, erythema, and increased temperature. It can be primary or secondary to arterial hypertension, venous insufficiency, diabetes mellitus, autoimmune, myeloproliferative, vascular, or neurological diseases.
