The increase in consultations and of hospitalisations, along with the amount of lost workdays, result in very high direct and indirect costs for society.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Flu vaccination is recommended for the specific protection of the individual but also to health care workers (HCW) for two reasons: firstly because the HCW can be a source of infection for people at risk of suffering from flu complications (especially among hospitalised patients) and because the HCW is part of the group that provides essential services to the community.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Despite the implementation and promotion of flu immunisation campaigns in health centres, vaccination coverage for HCW in Spain is very low and rarely exceeds 25%.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,3</span></a> Notably, unlike other countries, flu vaccination is always voluntary for HCW.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> One of the main reasons given by HCW for rejecting the vaccine is the fear of adverse events (AE) and the perception that the vaccine has a low effectiveness.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Available scientific evidence estimates that the effectiveness of the vaccine against the seasonal flu is over 50% for strains included in it<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> but the overall effectiveness decreases to 30% among certain risk groups.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> It has also been evidenced that the effectiveness of the vaccine increases to 86% if only the microbiologically confirmed flu cases are evaluated.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> The overall effectiveness of the vaccine provides enough protection to recommend it to risk groups.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,9,10</span></a> Inactivated flu vaccines are safe and well tolerated. Among the reported AE, local reactions are the most frequent while local pain and induration stand out as very common. The meta-analyses available indicate a relative risk (RR) of pain and induration at the injection point of 3.11 (95% CI 2.08–4.66), followed by erythema (RR 4.01; 95% CI 1.91–8.41) and myalgia (RR 31.54; 95% CI 1.12–2.11).<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Systemic reactions are much less frequent (fever, myalgia, general malaise).<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Allergic reactions and the Guillain–Barre syndrome are rare, the risk of the latter is estimated to be 1.6 cases per million doses.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> No relationship between the flu vaccination and other demyelinating diseases have been found.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The objectives of this study are to describe the coverage of the flu vaccine in HCW according to the sociodemographic characteristics of the vaccinated HCW and to characterise the reported AE after voluntary administration of vaccines in a tertiary university hospital. The interest of this study is the opportunity to evaluate a self-reporting AE system, as well as to assess the possible impact of AE on the acceptance of the flu vaccine,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> variables that will help to measure the impact of the flu vaccination campaign on the HCW beyond the vaccination coverage.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methodology</span><p id="par0025" class="elsevierStylePara elsevierViewall">Observational, cross-sectional study conducted during the 2011–2012 flu vaccination campaign (October 2011–January 2012) conducted with HCW working at the Hospital Clínic de Barcelona (HCB), a university hospital of 700 beds and 4944 registered health-care professionals during that period.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The flu vaccination campaign in the HCB included a mobile unit which was made up of doctors and nurses from the Preventive Medicine and Epidemiology and Occupational Health and Safety services (SPRL), which physically moves to different services and areas to bring vaccinations to HCW in their workplace. Following recommendations by the World Health Organisation (WHO) for the Northern Hemisphere, an inactivated vaccine that included 3 strains were used for the 2011–2012 flu vaccination campaign: A/California/7/2009 (H1N1) pdm09, A/Victoria/361/2011 (H3N2) and B/Wisconsin/1/2010 (Yamagata lineage).<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,15</span></a> The brand names of the vaccines administered to the HCW during this campaign were: <span class="elsevierStyleItalic">Inflexal</span><span class="elsevierStyleSup">®</span><span class="elsevierStyleItalic">V</span>, lote 3002110.01 (Crucell, Madrid), polyvalent influenza vaccine, <span class="elsevierStyleItalic">Leti</span><span class="elsevierStyleSup">®</span>, batches H0232-2 y H0286-2 (LETI Laboratories Barcelona) and <span class="elsevierStyleItalic">Influvac</span><span class="elsevierStyleSup">®</span>, batch X05 (Abbott, Madrid).</p><p id="par0035" class="elsevierStylePara elsevierViewall">For each flu vaccination campaign, information about the vaccinated HCW (sex, age, profession and work shift) was routinely collected. To calculate the vaccination coverage, the total amount of HCWs vaccinated during the season the study conducted was used as the numerator, and the total permanent and sub-contracted employees working at the HCB at a mid-way point during the campaign was used as the denominator.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Each year, the possible AE arising from vaccination are reported voluntarily to the Hospital's SPRL and notified using the yellow card system. All HCW medical consultations and sick-leaves are evaluated as possible AE and are monitored to identify serious AE arising from the vaccination. Allergic reactions, the Guillain–Barre syndrome, seizures and general skin reactions are considered serious AE. Despite being one of the main reasons why HCW choose not to be vaccinated, the hospital SPRL treats per year, an average of 3 AE potentially associated with the flu vaccine, none of them serious. For the reporting of an AE during the 2011–2012 campaign, a voluntary and anonymous AE notification system was established. A self-administered questionnaire that gathered information on social- demographic data (age, gender and professional category) was used, in addition to the presence or not of AE that occurred 48<span class="elsevierStyleHsp" style=""></span>hours after the vaccination was administered. Self-reported AE were classified into the following categories: local (pain, swelling, redness or functional disability) and systemic (headache, fever, fatigue, malaise, joint pain). Other less common AEs were also collected through an open question. After vaccination, the mobile unit delivered the questionnaire to the HCW, after explaining its objectives and after answering any questions. The questionnaires could also be obtained by the HCW in different HCB areas during the vaccination campaign. Once completed, they could be delivered to the SPRL mailbox or to any mobile unit employee throughout the vaccination campaign.</p><p id="par0045" class="elsevierStylePara elsevierViewall">A descriptive analysis of the questionnaire data collected was carried out using absolute and relative frequencies for qualitative variables and by calculating the mean and standard deviation (SD) for quantitative variables. A bivariate analysis by multiple logistic regression where the <span class="elsevierStyleItalic">odds ratio</span> (OR) is presented with confidence intervals of 95% (95% CI) was carried out. The primary variable was the reporting or not of AE. To facilitate the interpretation of the regression analysis data, age was categorised in years (<35, 35 to 55 and >55) and profession was categorised into medical specialists/resident doctors, nurse/nursing assistant/medical assistant and others. The professional category “other” included: administrative, maintenance, cleaning, kitchen, security and other professional personnel and technicians. The statistical programme used was SPSS<span class="elsevierStyleSup"><span class="elsevierStyleItalic">®</span></span> v18.0. The researchers of this study, approved by the HCB Ethics Committee, respected at all times the confidentiality of the data in compliance with Law 15/1999 on the Protection of Personal Data.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Vaccination coverage in health-care professionals</span><p id="par0050" class="elsevierStylePara elsevierViewall">During the 2011–2012 flu vaccination campaign a total of 1507 HCW (30.5%) from the HCB voluntarily received one dose of the flu vaccine, 1009 were women (67% of total vaccinated) and 489 were men (33%) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Proportionally, this figure corresponds to 28.9% of the 3489 women registered as HCW at the HCB and 33.6% of the 1455 men registered. The overall average age of the HCW registered at the hospital for that period was 45.0 years (SD 11.6), and the average age of those vaccinated was 43.0 years (SD 11.7). One thousand one hundred and fifty- nine HCW (76.9%) were vaccinated in their workplace through the mobile vaccination unit, and the rest at the SPRL. The highest vaccination coverage among HCW by job category occurred in 166 of 320 resident doctors (51.9%) and in 348 of 813 medical specialists (42.8%), compared to 407 (30.0%) from a total of 1455 nurses, 222 of 815 nursing/medical assistants (27.2%) and 362 of 1541 from the category “other” (23.5%).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Self-reported adverse events</span><p id="par0055" class="elsevierStylePara elsevierViewall">Of the 1507 people vaccinated, 358 answered the AE questionnaire (23.8%). As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, 273 were women (76.3%). The highest response rate was obtained in the job category “other”, with 126 completed questionnaires (34.8%), followed by the doctors category, with 79 (22.7%).</p><p id="par0060" class="elsevierStylePara elsevierViewall">A total of 186 HCW reported some sort of AE from the 358 who responded to the questionnaire (52.0%). Of the 186, 140 reported local signs and symptoms after vaccination (75.3%), 18, systemic signs and symptoms (9.7%), and 28, both local and systemic symptoms and signs (15.1%). From the professional category, 57.9% of residents who completed the questionnaire reported some type of AE, followed by 55.8% of nursing staff. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the distribution and frequency of local and systemic symptoms among those who completed the AE questionnaire. One hundred and eighty-six of the entire 1507 HCW vaccinated reported some sort of AE through the notification system (12.3%). The analysis of factors associated with reporting some sort of AE after flu vaccination is shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> where the feminine gender and age <35 stand out as variables that do not lose statistical significance in the multivariate model.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">Flu vaccination coverage for HCB HCW was 30.5%, which exceeded the Spanish average, estimated at 24.7%<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> for the 2011–2012 season. Despite this rate being an above average HCW flu vaccination coverage percentage, it is considered low when compared to European Union and WHO recommendations.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Medical staff form a risk group in regard to flu vaccination and thus ideal coverage should be 100% in those who do not present contraindications.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> Vaccination coverage in the HCB for registered HCW aged 60 and over stands out as, despite being regarded as a higher risk group, those who had the lowest coverage by age group. This may be, at least partially, due to the administration of the vaccine to these HCW in their Primary Care centre: there is a double indication because of their occupational and age group.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The Cochrane review for injectable inactivated flu vaccines states that the oculo-respiratory and Guillain–Barre post-vaccination syndromes are extremely rare.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> However, local (redness, pain and swelling) and systemic (myalgia, fatigue) AE may be present in more than 50% and fewer than 10% of patients, respectively.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,17</span></a> AE in Spanish studies is shown to have occurred in between 25 and 31% of vaccinated HCW,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a> all of them being mild AE. In this study, despite the fact that only 25% of the vaccinated population participated in the voluntary and anonymous AE notification system, results showed that the proportion of AE is similar to that found in the above studies. This study is a pioneer in reporting the AE of flu vaccination in HCW in our environment.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,21</span></a> One of every four people vaccinated voluntarily answered the post-vaccination AE questionnaire, the most significant group being females younger than 35 years-old. 75% of self-reported AE were local. As in other studies, no severe AE (anaphylactic reactions or encephalopathies) attributed to the flu vaccine were registered, which reinforces the profile of the vaccine being safe against seasonal flu.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> The absence of severe AE can be extended to all HCW vaccinated in the hospital, since the SPRL has an exhaustive control of severe AE.</p><p id="par0075" class="elsevierStylePara elsevierViewall">With the implementation of the voluntary and anonymous AE reporting system, a greater number of notifications was achieved. The success of a voluntary AE notification system lies in ensuring HCW accept such a system, its safety and easy access, as well as the ultimate usefulness of the data collected for the HCW.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Most of the AE statements being made by women was also observed in other studies in Spain.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,20</span></a> Similarly, although with less consistency, an increased amount of AE reports was observed among the youngest vaccinated HCW group at the Hospital General Universitario de Alicante.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> Although these differences are not usually observed in clinical trials that assess the safety of the flu vaccination,<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,17</span></a> they have been found in clinical trials with adjuvant flu vaccines.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Nichol et al. associated the female gender and an age younger than 40 years with increased reactogenicity,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> which could lead to a greater tendency to notify AE. The high number of questionnaires answered that reported no AE is worth noting, especially within the team of medical specialists, the professional category that had the highest vaccine coverage. This may be a reflection of acceptance, adherence and monitoring of the HCW to the flu vaccination campaign and to the clinical quality initiatives and safety.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The study has several limitations; one of them is that for the calculation of vaccination coverage, the denominator that was used is the amount of HCW recorded for a single day, while the numerator corresponds to the amount of HCW vaccinated throughout the season. This methodology, even after verification that it does not show significant differences with other different calculation methods, could result in an overestimation of the percentage of HCW vaccinated, a fact that could be partially compensated by under-reporting by HCW who have been administered the vaccine in other health centres. Vaccination outside the hospital is not systematically notified to the SPRL, a fact that could result in an underestimation of vaccination coverage, especially in the HCW group aged over 60 years-old. Possible under-reporting, especially of mild local AE, may limit the interpretation of the study results. The study design and anonymity of questionnaires do not allow a causality between the vaccination and the AE reported to be established. The study presents a response bias because, despite the levity of the AE, the HCW affected had a greater motivation to report.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0085" class="elsevierStylePara elsevierViewall">The flu vaccine is a preventive measure which, despite not being highly effective, has a high safety profile. A communication campaign containing the HCW questionnaire results was developed for the following season's vaccination campaign (2012–2013). Its purpose was to generate evidence in the workplace environment, so that the HCW could discuss and transmit individual and collective experiences relating to one the barriers to flu vaccination that exists among their group: the fear of AE.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">The percentage of participation observed in this passive AE notification system is considerably high considering the completion of the questionnaires was voluntary. Vaccination coverage in the HCW centre did not decrease compared to previous seasons. Both events demonstrate the maturity and willingness of HCW to voluntarily report AE, which in turn would ensure the successful systematic implementation of reporting systems. High levels of vaccination coverage must be accompanied by voluntary reporting systems to ensure proper monitoring of AE, with high participation of HCW to avoid response bias. A more comprehensive assessment of the impact of reporting measures and monitoring of AE in the coverage of flu vaccination would be necessary, since they could be factors that increase HCW confidence in the vaccine, especially to demonstrate the absence of serious AE in the HCW's own health centre.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conflict of interest</span><p id="par0095" class="elsevierStylePara elsevierViewall">Researchers’ participation has been voluntary and open and there are no conflicts of interest in regard to the content of this article. The aim of this study is to describe the vaccinated population and analyse vaccination coverage and self-declared AEs after the voluntary flu vaccination in a university hospital in Barcelona.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Observational study. We used the HCW immunisation record to calculate the vaccination coverage. We collected AEs using a voluntary, anonymous, self-administered survey during the 2011–2012 flu vaccination campaign. We performed a logistic regression model to determine the associated factors to declare AEs.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The influenza vaccination coverage in HCW was 30.5% (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1507/4944). Para el cálculo de la cobertura de vacunación se utilizó el registro de vacunación de profesionales sanitarios del hospital. Los AA se recogieron mediante una encuesta voluntaria, anónima y autoadministrada durante la campaña de vacunación antigripal 2011-2012, y se analizaron mediante regresión logística. Se construyó un modelo de regresión logística para determinar los factores que predisponen a declarar AA.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">La campaña alcanzó una cobertura de vacunación antigripal del 30,5% (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.507/4.944) del PS.
