Data on the prevalence of iron deficiency anaemia of pregnancy vary from 14% to 56% depending on the country's level of development.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Due to the physiological decrease in haemoglobin (Hb) during pregnancy, the value defining anaemia in this group is lower. However, the threshold used for the definition of pregnancy anaemia varies according to the values of the population studied, depending on the calculation of the mean Hb and the statistical criteria for the determination of the lower limit of normal, which is essential for its diagnosis and prevalence analysis. The Spanish Group of RBC pathology of the Spanish Society of Haematology and Hemotherapy (SEHH) defines pregnancy anaemia as an Hb level <11 g/dL in the first and third trimesters and <10.5 g/dL in the second. These are the criteria used in our stud.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The objective of this study is to evaluate Hb levels and to know the real prevalence of anaemia in obstetric patients in our population. This will allow the development of clinical-care protocols for the treatment of pregnancy anaemia.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A retrospective study of the blood counts requested in the quarterly pregnancy controls during the year 2019 in the Pamplona Health Area (478,286 inhabitants) was conducted. Cases of congenital anaemia associated with thalassemia and/or hemoglobinopathy were excluded.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The SEHH criteria were used to identify anaemia3 The population mean Hb and its standard deviation (SD) were determined, establishing the lower limit of normal according to the conventional statistical criteria (mean Hb −2SD) and that proposed by Beutler & Waalen (mean Hb −1.65SD<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>).</p><p id="par0030" class="elsevierStylePara elsevierViewall">A study subgroup was created with those pregnant women in whom the three laboratory controls took place during the year 2019 and therefore allowed a complete progression follow-up.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Complete blood counts are performed on Abbott cell-dyn Sapphire analysers (software version 5). The data are obtained from the Laboratory Information System and from the Roche Diagnostics Art database (version 1.2.0).</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">The variables are described using mean and SD in the quantitative case and by frequency and percentage in the categorical case. The prevalence of anaemia is estimated along with its 95% confidence interval for each trimester and for the total sample. For this estimation, the three cut-off points described above will be used: SEHH criteria, conventional criteria (mean Hb −2SD) and Beutler & Waalen criteria (mean Hb −1.6SD), calculated in each analysed trimester. Mean Hb by trimester was compared using the Anova test. The analysis is performed with the IBM SPSS v.25.0 and R software v.3.4.3 (SNS-O license).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data protection</span><p id="par0045" class="elsevierStylePara elsevierViewall">Data was collected under strict patient confidentiality rules through an anonymised Excel database with restricted access by means of an individual identification code.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">A total of 9995 complete blood counts corresponding to 5507 pregnant women, with an age range of 15–51 years, were reviewed. Mean Hb values were 13.1 g/dL in the first trimester, 11.5 in the second and 12 in the third. During the period studied, anaemia was detected in 14.8% of pregnant women (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Estimation of normal values in our population</span><p id="par0055" class="elsevierStylePara elsevierViewall">The lower limit of normal for Hb calculated on the basis of our sample data differs from the criteria established by the SEHH. With our new calculated limits, the prevalence of anaemia in the first trimester would be greater than 3.2% (conventional criteria −2SD) and 5.3% (Beutler & Waalen criteria −1.65SD) vs. 2.2% (SEHH criteria) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Complete progression follow-up</span><p id="par0060" class="elsevierStylePara elsevierViewall">It corresponds to 1134 pregnant women with the three follow-up controls performed in 2019 with an age range between 16–43 years. 22.6% of these patients had anaemia during pregnancy. The percentage of anaemia was 1.8% in the first trimester, 11.8% in the second, and 13.2% in the third. Of the 20 patients in whom anaemia was detected in the first trimester, 14 continued with anaemia in successive controls (8 in the two following trimesters and 6 in one of them) and of the 98 cases diagnosed in the second, 26 continued with anaemia in the third trimester (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">Pregnancy anaemia is a major global public health problem with a risk of maternal and foetal morbidity and mortality.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> WHO estimates that 41% of pregnant women worldwide suffer from anaemia.2 Studies from North America report a prevalence between 5 and 19.9%, with a higher risk of anaemia in patients from ethnic minorities, adolescent mothers and lower socio-economic status.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Our study found 14.8% of pregnant women with anaemia, which is lower than global data and similar to North American data. There is a bias due to the fact that more first trimester CBCs are analysed, and Hb is higher in early pregnancy. This bias forces us to analyse the data from the group of patients who had the three trimester controls within the year 2019, showing a 22.6% anaemia rate, which is higher than in other developed countries and which alerts us to a margin for improvement in clinical management.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Considering the prevalence of anaemia per trimester, it is low in the first (2.2%) but experiences a significant increase in the second (11%) and continues to increase until reaching 15.2% in the third. This shows a lower rate of anaemia than other studies with similar population characteristics to ours and coincides with the increasing trend over the trimesters. A Belgian study found 4% of pregnant women with anaemia in the first trimester; rising to 21% in the third<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and a Catalan study showed a prevalence of 3.8%, 13.1% and 21.5% in each trimester, respectively.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The data in the group of patients with complete progression control are similar with a low percentage in the first trimester, which increases during pregnancy. Furthermore, the degree of anaemia is higher in the second trimester than in the third, which could indicate that second trimester anaemias are treated and therefore do not advance or may even improve (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The possible lack of detection of hematin deficiencies may be due to the underestimation of anaemia in the first trimester, as shown by the data derived from the lower limits of normal in our population. This may explain the progression to a higher prevalence of anaemia in subsequent trimesters.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Although the prevalence of anaemia in the first trimester of patients with complete progression control was 1.8%, it should be noted that 40% of these cases continued with anaemia throughout the pregnancy and 21% of new cases in the second trimester also had anaemia in the third trimester. We can conclude that 25% of pregnant women with anaemia detected in the first or second trimester end their pregnancy with anaemia, a worrying figure that leads us to the urgent need for an anaemia treatment protocol within a “<span class="elsevierStyleItalic">Patient Blood Management</span>” (PBM) programme.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The main cause of anaemia is iron deficiency, but we do not have data on iron metabolism in our population. We consider it necessary to extend this study by adding its analysis using the new complete blood count indices of the current analyzers<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> (thus avoiding the cost of the iron profile), in addition to taking into account other factors (age, parity, multiple pregnancies…).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusions</span><p id="par0095" class="elsevierStylePara elsevierViewall">The prevalence of anaemia is higher than in other Western countries and increases significantly in the second trimester. Of these, 1134 patients underwent complete follow-up in 2019. The prevalence data for anaemia were 1.8%, 11.8% and 13.2% in each trimester respectively, and the global prevalence in pregnancy was 22.6%. Regarding postpartum anaemia, its prevalence with respect to all pregnant women was 2.99%, increasing to 38.2% in those patients with complications during delivery.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">The prevalence of gestational anaemia in our population is somewhat higher than in countries like ours. Therefore, there is room for improvement in our current clinical protocols. Los datos de prevalencia de anemia fueron de un 1,8%, 11,8% y 13,2% en cada trimestre respectivamente. La prevalencia global fue de un 22,6%.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusiones</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">La prevalencia de anemia gestacional en nuestra población es algo superior a la consultada en la bibliografía y varía en función del cálculo del límite inferior de normalidad (hemoglobina media poblacional y criterios de normalidad clásico y de Beutler&Waalen).
