In Spain, depending on the geographical area, it reaches 4.7%.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> Asthma exacerbations requiring emergency room (ER) visits or hospitalisation occur in 20% of asthmatics and represent more than 80% of the direct costs associated with asthma. Triggers include external factors, such as respiratory infections or environmental contamination, as well as inadequate treatment or poor adherence.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">A variety of studies have shown an association between environmental contamination and the use of rescue medication, ER visits or admissions due to asthma exacerbations.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">3,4</span></a> Some of these studies found that an increased concentration of nitrogen dioxide (NO<span class="elsevierStyleInf">2</span>) is associated with asthma.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">3</span></a> Other studies found that frequent exposure to particles with a diameter of between 2.5 and 10<span class="elsevierStyleHsp" style=""></span>μm was also associated with asthma.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">3</span></a> However, there are also studies whose results reject an association between exacerbations of asthma and contamination.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">6,7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The association between meteorological variables and asthma has also been studied. The data supporting this argument is more controversial. Certain studies show an association between cardiovascular and respiratory mortality with abrupt temperature fluctuations.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> There are studies showing an inverse association between temperature – and positive humidity – and asthma exacerbations,<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">6,9</span></a> whereas the results of other studies do not concur.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> Other environmental factors, such as atmospheric pressure, have been less analysed, although certain studies have suggested that high atmospheric pressure could be associated with an increase in hospitalisations for asthma.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">11,12</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The purpose of this study was to explore the association between NO<span class="elsevierStyleInf">2</span>, sulphur dioxide (SO<span class="elsevierStyleInf">2</span>) and carbon monoxide (CO) and ER visits and hospitalisations due to asthma exacerbation, while analysing the impact of this possible association.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Location</span><p id="par0025" class="elsevierStylePara elsevierViewall">This was an observational and retrospective study, carried out in a hospital in Badalona, a city in Barcelona (Barcelona, Spain). It is located north of Barcelona, on the coast of the Mediterranean Sea. In 2015 the city had 216,085 inhabitants. The Germans Trias i Pujol University Hospital is a tertiary hospital with 600 beds. It is also a reference hospital for 700,000 people. In 2015 there were 28,000 admissions to this centre.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Explanation of the case</span><p id="par0030" class="elsevierStylePara elsevierViewall">The number of ER visits and hospital admissions due to asthma exacerbation was obtained through the Centre's Information Systems Unit. We consider ER visits and hospitalisations to be due to acute bronchial asthma when the discharge report contained the International Classification of Diseases (ICD-9) codes: 493.02, 493.12, 493.22, 493.92. The study period ranged from January 1, 2008 to December 31, 2016. All ages were included in the study. Other diagnoses were not considered to be either inclusion nor exclusion criteria. On this basis, the study did not analyse, for example, how many of the cohort had other respiratory comorbidities that could interfere with our results. Patients whose postal code did not correspond to the Badalona area were not included in the analysis. In order to differentiate between asthmatic events that condition an ER visit and those that condition a hospitalisation (in theory, more serious), when a patient visited the ER and was subsequently hospitalised, the entire event was considered only a hospitalisation. In the case of hospitalisations with a previous ER visit, the day of admission was considered the day the patients had visited the ER.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Obtaining environmental data</span><p id="par0035" class="elsevierStylePara elsevierViewall">Catalonia has a network of stations controlled by the Generalitat de Catalunya, Environmental Affairs department. Our data was obtained from a station located in the Badalona city centre (<a id="intr0010" class="elsevierStyleInterRef" href="http://dtes.gencat.cat/icqa/">http://dtes.gencat.cat/icqa/</a> [accessed May 13, 2017]). We obtained daily SO<span class="elsevierStyleInf">2</span>, NO<span class="elsevierStyleInf">2</span> and CO values. Because the daily data of particles with a mean aerodynamic diameter of 10<span class="elsevierStyleHsp" style=""></span>μm or less (PM<span class="elsevierStyleInf">10</span>) were unavailable, this variable was not included in the analysis.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Obtaining weather data</span><p id="par0040" class="elsevierStylePara elsevierViewall">Daily average temperature, atmospheric pressure and relative humidity values were provided by the Catalan Meteorological Agency (<a id="intr0015" class="elsevierStyleInterRef" href="http://www.meteo.cat/">http://www.meteo.cat</a> [accessed February 13, 2016]), who sourced this data from a meteorological station located in the Badalona city centre.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">Simple Poisson regressions were used to study the effects of temperature, relative humidity, atmospheric pressure, and SO<span class="elsevierStyleInf">2</span>, NO<span class="elsevierStyleInf">2</span> and CO levels on ER visits and hospitalisations due to asthma attacks. Environmental variables and contaminants values were analysed on the same day the consultation took place, the previous day, and up to two three and four days prior to the visit (lag 0, lag 1, etc.). To correct for multiple comparisons (the study considered six independent variables <span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleSmallCaps">×</span><span class="elsevierStyleHsp" style=""></span>2 events [ER visits and hospitalisations]<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleSmallCaps">×</span><span class="elsevierStyleHsp" style=""></span>5 time lags<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>60 Poisson regression), we applied a permutation technique. More specifically, we simulated 5000 studies in which there was no association between the independent variables and the events, and in each of these simulated studies we repeated the 60 Poisson regressions and saved the maximum of the 60 <span class="elsevierStyleItalic">z</span>-values. Finally, we created the distribution of these maximum <span class="elsevierStyleItalic">z</span>-values, which indicated the probability that one or more of the 60 regressions was significant only by chance (that is, the familywise error rate). For example, the maximum <span class="elsevierStyleItalic">z</span>-values reached a value of 9.2 in only 4% of the simulated studies, so if we had used this value as a threshold, there would have only been a 4% probability that the maximum <span class="elsevierStyleItalic">z</span>-value was greater than 9.2 only by chance. In other words, there would have been only a 4% chance that one or more of the 60 regressions would be statistically significant just by chance.</p><p id="par0050" class="elsevierStylePara elsevierViewall">In order to quantify the impact of these pollutants, we estimated the attributable fraction (that is, the risk of disease exclusively due to exposure to the risk factor) as the percentage of cases that would have been avoided if the average incidence in 50% of days with more pollution would have been equal to the average incidence in 50% of days with less pollution.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The data has not been adjusted for any other variable (such as seasonality, flu activity, pollens, etc.).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Ethical aspects</span><p id="par0060" class="elsevierStylePara elsevierViewall">The study protocol was approved by the Germans Trias i Pujol University Hospital, Clinical Investigation Ethics Committee.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> describes the environmental and meteorological variables. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the evolution of the exacerbations and the variables analysed over the months. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the difference between the values of the variables on days when an event occurred (ER visit or hospitalisation) compared to days when no event occurred.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Hospitalisations for asthma</span><p id="par0070" class="elsevierStylePara elsevierViewall">There were 1961 hospitalisations during the period studied (1274 female, average age: 38.1). The results of the Poisson regressions for hospitalisations for asthma are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The number of hospitalisations associated with low temperature for lag 0 are (relative risk [RR]: 0.961), lag 1 (RR: 0.962), lag 2 (RR: 0.960), lag 3 (RR: 0.960) and lag 4 (RR: 0.958). The number of hospitalisations associated with increased levels of NO<span class="elsevierStyleInf">2</span> for lag 0 are (RR: 1.010), lag 1 (RR: 1.009), lag 2 (RR: 1.009) and lag 4 (RR: 1.008) and with atmospheric pressure increases for lag 2 (RR: 1.019) and lag 3 (RR: 1.020).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">ER visits</span><p id="par0075" class="elsevierStylePara elsevierViewall">There were 2431 ER visits due to asthma exacerbations during the period of time analysed (1315 female, average age: 27.9). The results of the Poisson regressions for ER visits are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. The number of ER visits associated with low temperature for lag 0 are (RR: 0.970), lag 1 (RR: 0.969), lag 2 (RR: 0.970), lag 3 (RR: 0.966) and lag 4 (RR: 0.964). The number of ER visits associated with increased levels of NO<span class="elsevierStyleInf">2</span> for lag 2 are (RR: 1.010), lag 3 (RR: 1.010) and lag 4 (RR: 1.008).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">This study found that an increase in the number of ER visits due to exacerbation of asthma was associated with a decrease in ambient temperature and with an increase in NO<span class="elsevierStyleInf">2</span> levels. Hospitalisations also increased with decreases in temperature, and with higher levels of atmospheric pressure and NO<span class="elsevierStyleInf">2</span>. It did not find an association with SO<span class="elsevierStyleInf">2</span>, CO or relative humidity levels.</p><p id="par0085" class="elsevierStylePara elsevierViewall">These findings concur with other studies, such as the one carried out by Altzibar et al., who found an increase in asthma exacerbations with an increase in NO<span class="elsevierStyleInf">2</span>, CO, flu activity and with decreases in temperature.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> A study conducted by Hajat et al. in London, found a relationship between asthma consultations and NO<span class="elsevierStyleInf">2</span>, CO and PM<span class="elsevierStyleInf">10</span> levels among the child population. This association could only be substantiated for PM<span class="elsevierStyleInf">10</span> in adults.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">4</span></a> A meta-analysis carried out by Zheng et al. reviewing 87 studies, 21 of them in adults, found that an increase of 10<span class="elsevierStyleHsp" style=""></span>μg of NO<span class="elsevierStyleInf">2</span> meant a RR of 1.018 of having an asthma exacerbation. This risk increased in children, elderly, males and during warm seasons.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">14</span></a> A subsequent meta-analysis showed a RR of 1.024 for each increase of 10<span class="elsevierStyleHsp" style=""></span>μg of NO<span class="elsevierStyleInf">2</span>.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Recently, Tian et al. observed in a large study in Beijing that asthma consultations were clearly increasing in relation to exposure to PM<span class="elsevierStyleInf">2.5</span>. Unlike other previous studies, their study included outpatient consultations, in addition to the usual ER visits and hospitalisations.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a> Other studies confirm similar data also in the area of chronic obstructive pulmonary disease.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">NO<span class="elsevierStyleInf">2</span> in cities comes mainly from the combustion of fossil fuels.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a> NO<span class="elsevierStyleInf">2</span> levels have been related to a higher incidence of asthma.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a> NO<span class="elsevierStyleInf">2</span> has been shown to increase bronchial hyperresponsiveness in individuals with asthma,<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> although this has not been confirmed by other studies,<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> and there does not seem to be a plausible biological explanation for this discrepancy. The effects of NO<span class="elsevierStyleInf">2</span> have been attributed to the fact that it facilitates the lipid peroxidation of cell membranes and the generation of several free radicals that worsen airway structures and functions.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a> NO<span class="elsevierStyleInf">2</span> can also facilitate the release of inflammatory mediators such as interleukin 8 (IL-8) or granulocyte macrophage colony-stimulating factor (GMCSF).<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">This study confirmed the relationship between temperature decrease and asthma exacerbations. It is a finding that other studies also concur with, whose results show that there are more ER visits and hospital admissions during winter.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> Other authors evaluated the direct effect of short-term temperature variations and also showed that low temperature increased asthma exacerbations.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">6,24</span></a> Furthermore, Abe et al. evaluated 6447 ER visits for asthma exacerbation over one year and found that – irrespective of contamination – as the temperature decreased, ER visits increased.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a> We found similar results, persistently over eight years, and for hospitalisations too; from the first day temperatures fell, up to four days after the decrease. A reduction in temperature coincided with autumn/winter flus.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> We know that when the temperature decreases there is often an increase in viral infections,<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> which usually peak during the same period (autumn/winter). It is said that the cooling of the nasal passages negatively affects respiratory defences.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> Such that, although no data were collected regarding viral infections, we can conclude from these studies that there is a direct relationship between both variables. However, there could also be a relationship between temperature and asthma exacerbations independent of viral infections, as we know that a decrease in temperature increases the inflammation of the airway and can cause bronchoconstriction and mucosal hypersecretion, as demonstrated in vitro in human bronchial epithelial cells; this is attributed to a mechanism mediated by the transient receptor potential melastatin 8 (TRPM8).<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">We found that NO<span class="elsevierStyleInf">2</span> and low temperature were factors associated with more exacerbations and admissions, which leads us to consider the possible interaction between pollution and weather conditions. That meteorological conditions can condition the level of contamination is well-known,<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> which could partially explain our findings. And furthermore, it has also been shown that high atmospheric pressures can worsen air quality.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">The association between high barometric pressures and hospitalisations for asthma has been demonstrated by a variety of authors.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,30</span></a> Ehara et al., studied a paediatric population who visited the ER for asthma exacerbations, and found a relationship between the number of exacerbations and a high barometric pressure and relatively low humidity.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">Our study has several strengths. Firstly, it has a long follow-up period. Data on weather and contamination variables, and the number of events (ER visits and hospitalisations) were available for every day. Secondly, the possible latency time between changes in the environmental variable and appearance of the effect have been taken into account. Thirdly, a remarkable number of events were obtained.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The study's main limitation is the fact that it is an observational study, so results should be interpreted at the population level, and should not be interpreted as causal associations. Its second one is that the definition of an event (asthma exacerbation) is based on discharge report codes. There may have been reliability issues when coding. Its third, is that it is a study conducted in a single centre, so the results should be taken with caution when extrapolating them to other geographical areas. Another limitation is that we could not evaluate ozone (O<span class="elsevierStyleInf">3</span>), PM<span class="elsevierStyleInf">10</span> and PM<span class="elsevierStyleInf">2.5</span> since these substances are collected at the station used to obtain the data, and are not collected daily. Additionally, we could not evaluate the influence of pollens on asthma exacerbations, which might have a significant confusing role. We did not collect pollen data because we could not find any measuring stations close to the geographical area analysed in our study in the metropolitan area of Barcelona. A final limitation is that it is always difficult to evaluate patients’ exposure levels in an observational study. We have excluded patients whose postal code does not correspond to the city of Badalona, but we cannot know how many hours patients were actually exposed to these substances, nor to what extent they are exposed at home to other agents such as tobacco smoke or other possible confounders not analysed in this study.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In conclusion, we found a significant association between asthma exacerbations and an increase in NO<span class="elsevierStyleInf">2</span>, decrease in temperature, and atmospheric pressure in the Badalona area. The number of daily hospital admissions and accident and emergency visits related to exacerbation of asthma between 2008 and 2016 was obtained. We used simple Poisson regressions to test the effects of daily mean temperature, atmospheric pressure, relative humidity, and NO<span class="elsevierStyleInf">2</span>, SO<span class="elsevierStyleInf">2</span> and CO levels on asthma-related emergencies and hospitalisations the same day and 1–4 days after. All <span class="elsevierStyleItalic">p</span>-values were corrected for multiple comparisons.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The number of hospitalisations was associated with low temperature (lags 0–4) and higher levels of NO<span class="elsevierStyleInf">2</span> (lags 0, 1, 2 and 4) and atmospheric pressure (lags 2 and 3). Mediante regresión de Poisson simple se estudió la asociación entre estos factores y el número de hospitalizaciones y urgencias mediante regresiones simples para el mismo día y para entre uno y 4 días más tarde (lags 0 a 4).
