class="elsevierStylePara elsevierViewall">Autoimmune haemolytic anaemias (AIHA) are a group of diseases characterised by haemolysis due to autoantibodies directed against red blood cell surface antigens, with or without complement activation. They are classified according to the antibody reactivity temperature and by the presence or absence of associated disease. The incidence is 0.8–3/100,000 inhabitants/year, being more prevalent in adults and women. 80% of AIHA are due to warm IgG or IgG+C antibodies (complement), while those caused by cold antibodies account for 13–15%, due to IgM antibodies, with a C3d positive direct Coombs (DC) test. There is a small percentage of mixed mechanism AIHA, with warm and cold antibodies, and atypical AIHA where the antibody involved is IgA, warm IgM or negative DC, less than 5% of the total. Secondary AIHA are associated with lymphoproliferative processes, autoimmune diseases (AID), infections or drugs, while they are considered primary if the cause is not found.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The diagnosis of AIHA requires the presence of anaemia, with a positive DC test and biochemical data of haemolysis. The treatment of AIHA is aimed at reducing the production of autoantibodies and the destruction of red blood cells.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The objective of this study has been to retrospectively study the biological and clinical characteristics of patients with AIHA, as well as analyse the responses to the different treatment lines in routine clinical practice, in the absence of sufficient clinical trials that allow scientific evidence-based treatments.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0020" class="elsevierStylePara elsevierViewall">Retrospective, multicenter and observational study of 93 patients with AIHA (primary or secondary), diagnosed between 1987 and 2017 in 9 hospitals in Spain, with a median follow-up of 28 months (1–332), with the last follow-up being in June 2017.</p><p id="par0025" class="elsevierStylePara elsevierViewall">AIHA was defined at haemoglobin levels (Hb)<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>110<span class="elsevierStyleHsp" style=""></span>g/l in women and Hb<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>120<span class="elsevierStyleHsp" style=""></span>g/l in men, with positive DC and biochemical haemolysis data: elevated bilirubin and LDH levels. The LDH was expressed as quotient: sample LDH/LDH upper limit of normal of each laboratory's technique. The rest of the parameters are expressed in absolute numerical value.</p><p id="par0030" class="elsevierStylePara elsevierViewall">According to the results of the DC and the thermal characteristics of the autoantibody, they were classified as warm antibody AIHA (wAIHA): IgG or IgG+C positive DC; cold antibody AIHA (cAIHA): C-positive DC; mixed AIHA (mAIHA): coexistence of warm antibodies with cold agglutinins; and atypical AIHA (aAIHA): negative DC or warm IgA or IgM positive DC.</p><p id="par0035" class="elsevierStylePara elsevierViewall">AIHA was considered secondary to that associated with AID, lymphoproliferative disease (LPD), infectious processes and drugs. Patients with Evans syndrome were included in the secondary group associated with AID.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Evaluation of treatment and results</span><p id="par0040" class="elsevierStylePara elsevierViewall">The types of treatment received included glucocorticoids (1–2<span class="elsevierStyleHsp" style=""></span>mg/kg/day orally or intravenously), rituximab (375<span class="elsevierStyleHsp" style=""></span>mg/m<span class="elsevierStyleSup">2</span> weekly for 4 weeks), splenectomy and combinations of corticosteroids with rituximab, immunoglobulins, immunosuppressants and erythropoietin (EPO). Corticosteroid dependence was defined as the need to maintain such treatment to maintain a response, or relapse before 3 months after discontinuation.</p><p id="par0045" class="elsevierStylePara elsevierViewall">First line of treatment was defined for all drugs administered simultaneously or successively, until a response was achieved. The second and subsequent lines of treatment are those received in relapses, after having obtained a previous response.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Response to the different treatments were defined as follows: complete response (CR) when normal Hb, bilirubin and/or LDH levels were achieved without the need of transfusion; partial response (PR) when Hb levels >100<span class="elsevierStyleHsp" style=""></span>g/l or increase of 20<span class="elsevierStyleHsp" style=""></span>g/l compared to baseline Hb were reached; overall response (OR) to the sum of CR and PR; no response if it does not meet CR or PR criteria and requires a change of treatment line. Relapse if Hb <100<span class="elsevierStyleHsp" style=""></span>g/l with the need to start a new treatment.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The transfusion requirements of packed red cells were collected, with transfusion efficacy being considered if there is an increase of 10<span class="elsevierStyleHsp" style=""></span>g/l Hb for each packed red cell transfusion, stable for at least 3 days.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">The data was collected in predesigned data templates using an Excel spreadsheet and analysed using the SPSS v. 22 (IBM, Armonk, NY, USA) statistical software. Categorical variables are described in percentages and the comparison between groups is performed using the chi-square test or Fisher's exact test for dichotomous variables. Continuous variables are expressed as mean (standard deviation) or median (interquartile range) and, depending on their characteristics of normality, the Mann–Whitney <span class="elsevierStyleItalic">U</span> test, ANOVA or Kruskal–Wallis are compared using Student's <span class="elsevierStyleItalic">t</span>-test.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Clinical and laboratory characteristics of the patients.</span><p id="par0065" class="elsevierStylePara elsevierViewall">Of the 104 initial patients, 11 with Hb figures >110<span class="elsevierStyleHsp" style=""></span>g/l and without the need for treatment were excluded, despite positive DC and biochemical haemolysis data, since they could correspond to a non-specific positive Coombs test with anaemia of another aetiology. A positive Coombs test can be detected in healthy people in a non-specific way up to 0.04% of the population<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> or associated with anticardiolipin antibodies, HIV, drugs and elevated serum immunoglobulin (IgG) levels.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">The clinical and serological data of the 93 patients are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The majority were women (62.4%), with a median age at diagnosis of 67 years. 54.8% were >65 years of age and only 6.5% were <18 years of age.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Regarding the serological type, IgG or IgG+C wAIHA were more prevalent with 85% of the cases, cAIHA was 8.6% and the mixed mechanism and atypical were 6.4%. 64.5% of the patients had primary AIHA, and 48.5% of the secondary ones were associated to LPD and 24% to AID.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The median follow-up was 28 months (1–332), with 91.4% of patients alive at the end of the study. The patient with one month of follow-up died from a metastatic lung adenocarcinoma. All deaths were related to secondary AIHA, with sepsis being the cause of death in 4 cases and metastatic malignancies in 3 cases. A patient with wAIHA died of progressive multifocal leukoencephalopathy, not having received treatment with rituximab. There were no deaths directly related to AIHA.</p><p id="par0085" class="elsevierStylePara elsevierViewall">With respect to patients under 18, 83% had IgG+C wAIHA, and the remaining 17% cAIHA; from a clinical point of view, all were primary AIHA treated in first line with corticosteroids alone or in combination with immunoglobulins; the response rate was 60% CR (40% PR).</p><p id="par0090" class="elsevierStylePara elsevierViewall">The relationship between Hb figures and the different age groups was analysed (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), observing lower Hb figures in patients <45 years of age compared to those over this age; the difference was statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.022). In addition, the relationship between age groups and serological types was analysed (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), observing a prevalence of IgG+C wAIHA in those <45 years of age while in the >45 years group there was an IgG wAIHA prevalence.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the relationship between serological AIHA types and clinical severity, with IgG+C AIHA being more severe, with significant differences (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.009) compared to the rest of the serological groups. The analysis between the IgG and IgG+C groups showed no clearly significant differences (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.065). The relationship between the biochemical parameters of haemolysis and the serological types of AIHA is shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>, without finding significant differences between the different serological types in the absolute reticulocyte, LDH and total serum bilirubin count.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">We also analysed the relationship between Hb levels at diagnosis, reticulocytes and haemolysis biochemical parameters. Although reticulocytes were higher in patients with lower Hb levels, the differences were not significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.38), due to the dispersion of the data and the existence in all Hb groups of patients with low reticulocyte counts (14 of 69 analysed), probably related to antibodies against erythroid precursors. We observed significant differences between Hb groups with LDH (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02) and with total serum bilirubin (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.007), as these were higher in those cases where anaemia was more severe.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Treatment analysis</span><p id="par0105" class="elsevierStylePara elsevierViewall">Overall treatment analysis: 86 patients (92.5%) received a first line, 47 (54.6%) a second line, 24 (27.9%) a third line and 15 (17.4%) 4–6 lines of treatment. Seven patients (3 wAIHA, 2 cAIHA, one mAIHA and one aAIHA) did not need any treatment due to having a compensated chronic haemolysis.</p><p id="par0110" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows the treatments administered and their response rates according to treatment lines.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">The corticosteroids were used first line alone in 60 patients, and combined with other drugs due to lack of response or corticosteroid-dependence in 23 patients. Initial 1–2<span class="elsevierStyleHsp" style=""></span>mg/kg/day dose. Predominantly oral administration route (87.3%). Average duration of treatment: 25 months (0.5–80). Response rates: 83% OR alone (CR: 58.3%), with 32.5% corticosteroid dependence. The duration of the response was 45 months (1–79); and according to corticosteroids in monotherapy and in combination, of 58 and 43 months, respectively. When used in subsequent lines of treatment, steroid CRs in monotherapy are 40–55%, with ORs close to 100%.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Rituximab was administered to 34 patients: Eighteen in monotherapy in different lines and 16 associated with steroids (1–2 dose mg/kg/day). Dose used: 375<span class="elsevierStyleHsp" style=""></span>mg/m<span class="elsevierStyleSup">2</span>/week<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>4 doses Three patients with primary cAIHA were treated in first line with rituximab alone, achieving clinical response in all cases, with 66.7% CR. In patients with wAIHA, in which it was used alone in 2nd or 3rd line, the OR was 60–90%, with CR of 20–45%. Steroid-associated rituximab was used in 16 patients with 100% OR rates in 1st and 2nd line treatment and 75% in 3rd line. The duration of the response was 20 months (1–72) with rituximab alone and 30 months (1–96) when used in association with steroids.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In 15 patients, with a mean age of 71 years (47–89), immunosuppressive therapy (dolquine, cyclophosphamide, azathioprine, mycophenolate) combined with corticosteroid therapy was used. Three of them had underlying AID. The OR rate was 71.4% (42.8% CR) in 1st line and 50% in 2nd line; in subsequent lines only PRs were reached.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Intravenous immunoglobulins were used in 20 patients, always associated with steroids, achieving OR rates close to 90% with 70% CR when used in 1st and 2nd line treatment. They were used as first line in 8 patients, 6 of whom had immunodeficiency.</p><p id="par0135" class="elsevierStylePara elsevierViewall">EPO combined with steroids was used in 3 patients; 2 with creatinine clearance <40<span class="elsevierStyleHsp" style=""></span>ml/min.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Splenectomy was performed on 6 patients from 2nd line of treatment, with OR of 100%, and a duration of 50 months (10–120). All were adults except a girl diagnosed with wAIHA at 7 years of age, with a good response to glucocorticoids and immunoglobulins, but with 2 relapses, achieving CR maintained until today.</p><p id="par0145" class="elsevierStylePara elsevierViewall">With respect to supportive treatment, 37 patients (39.8%) received transfusions with transfusion efficacy in 22 of them (59.4%). The majority of cases without transfusion efficacy were severe onset AIHA (Hb<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>50<span class="elsevierStyleHsp" style=""></span>g/l).</p><p id="par0150" class="elsevierStylePara elsevierViewall">The cases associated with an infectious trigger (babesiosis and toxoplasmosis), corresponded to IgG+C wAIHA. The first one received first-line corticosteroids alone, achieving PR. The patient with toxoplasmosis received corticosteroids in association with immunoglobulins, achieving a CR maintained for 20 months and a 2nd line of treatment with corticosteroids and rituximab, achieving a CR without relapses until the last follow-up (18 months).</p><p id="par0155" class="elsevierStylePara elsevierViewall">The case of AIHA secondary to the intake of <span class="elsevierStyleItalic">Coutarea latiflora</span> (<span class="elsevierStyleItalic">C. latiflora</span>), an hypoglycaemic agent of vegetable origin referred to in the medical literature as colpachi or copalchi, was related to an 84-year-old diabetic woman that presented with severe anaemia (Hb: 57<span class="elsevierStyleHsp" style=""></span>g/l) and received steroids, achieving a maintained CR. A patient with severe postmenopausal osteoporosis treated with denosumab developed an IgG+C wAIHA, remitting spontaneously upon discontinuation. A case of IgG wAIHA, secondary to penicillin intake after a complicated cholecystectomy, received corticosteroids, achieving CR.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Treatment complications</span><p id="par0160" class="elsevierStylePara elsevierViewall">A 5.8% of infections were recorded, in all cases associated with corticosteroid treatment: a urinary infection due to <span class="elsevierStyleItalic">Staphylococcus epidermidis</span> added to central venous catheter candidiasis, a bacteraemia due to <span class="elsevierStyleItalic">Salmonella</span>, 2 severe pneumonia that required admission to the ICU and a case of toxoplasmosis.</p><p id="par0165" class="elsevierStylePara elsevierViewall">A patient with AIHA secondary to systemic lupus erythematosus (SLE) who had received corticosteroids and immunosuppressants, developed a progressive multifocal leukoencephalopathy due to JC virus, and died after this complication, being in CR at that time; no cases of patients treated with rituximab were recorded.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Ischaemic complications were observed in 5.5% of cases, they were mostly arterial (2 cerebral ischaemic accidents and 2 cardiac ischaemic events) and 2 venous episodes (a deep venous thrombosis and a pulmonary embolism) diagnosed with Doppler ultrasound and CT angiography. All cases corresponded to primary IgG wAIHA. These episodes were associated with a severe onset of AIHA with an average Hb figure of 52<span class="elsevierStyleHsp" style=""></span>g/l and need of 2nd line treatment in 80% of cases.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0175" class="elsevierStylePara elsevierViewall">We retrospectively analysed 93 patients with an average age of 67 years, a figure slightly higher than what was recorded in the medical literature, which could be explained by the inclusion of both primary and secondary cAIHA and AIHA.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">They mostly correspond to primary AIHA, with chronic lymphocytic leukaemia being the most common disease in secondary AIHA, similar to the group of Roumier et al., where most secondary ones were related to LPD and SLE.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a> The association of AIHA with AID in our series is 24.2%, with most relating to Evans syndrome and SLE, similar to the study by Dierickx et al., with 21% of AIHA being secondary to autoimmune disorders.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">The association of AIHA and bacterial infections is known, but medical literature is scarce regarding parasitic and viral infections. We have only found one case of AIHA associated with toxoplasmosis.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> Somewhat more common are the described cases of AIHA associated with babesiosis. The largest series published is that of Woolley et al.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">9</span></a> included 86 patients with babesiosis, of which 6 developed an IgG+C AIHA, with all patients being asplenic; 4 of them requiring treatment with immunosuppressants in order to achieve a response. Two cases of severe wAIHA secondary to babesiosis are described in series by the Narurkar et al.,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a> with good response to corticosteroids in one of them and no response in the other. In our case it was a patient with IgG+C wAIHA without asplenia, which required a 2nd immunosuppressive treatment line.</p><p id="par0190" class="elsevierStylePara elsevierViewall">In relation to phytotherapy products, a patient presented AIHA associated with the intake of <span class="elsevierStyleItalic">C. latiflora</span>. We do not know of any cases published in the medical literature of this association, although there are cases described associated with hepatitis.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">11,12</span></a> Regarding the association between AIHA and drugs, the connection with penicillin is well documented,<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">13</span></a> as well as that of novel immunomodulatory drugs being introduced in the field of haematology–oncology, such as nivolumab.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">14</span></a> In our series we have diagnosed a case of AIHA in relation to denosumab, anti-RANKL monoclonal antibody, without knowledge of any previously published cases.</p><p id="par0195" class="elsevierStylePara elsevierViewall">From a clinical point of view there is a greater severity of anaemia in patients <45 years and in serological types IgG+C, similar to the study by Barcellini et al.,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> where Hb was lower in mAIHA, aAIHA and in IgG+C3 wAIHA.</p><p id="par0200" class="elsevierStylePara elsevierViewall">AIHA is a rare disorder <18 years and most respond well to steroids regardless of the type of antibody.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a> In our study, 6.5% of patients <18 years were evaluated, with a median Hb figure<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>58.5<span class="elsevierStyleHsp" style=""></span>g/l, the lowest by age groups, with no records of infectious complications or deaths, unlike other studies<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a><span class="elsevierStyleSup">.</span><a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a> where most were secondary AIHA, with Hb levels<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>58–76<span class="elsevierStyleHsp" style=""></span>g/l, mostly treated with steroids with good overall response rates and infectious complications being recorded in 25.7% of cases and even one death from cytomegalovirus.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a></p><p id="par0205" class="elsevierStylePara elsevierViewall">The main clinical presentation (64.5%) of wAIHA was severe with Hb levels <80<span class="elsevierStyleHsp" style=""></span>g/l, as opposed to cAIHA in which 87.5% of patients started with Hb >80<span class="elsevierStyleHsp" style=""></span>g/l, data similar to the study by Barcellini et al. where Hb values were lower in IgG+C and in mixed forms.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> No statistically significant differences were found between the serological type of AIHA and the biochemical haemolysis data, similar to Roumier et al.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">The treatment of AIHA is not standardised. Case series studies have been published in the medical literature, mostly retrospective, that analyse the response rates to the therapies used.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">17–21</span></a></p><p id="par0215" class="elsevierStylePara elsevierViewall">The first-line treatment in wAIHA was corticosteroids alone, 16% of patients needing to associate another drug due to lack of response or corticosteroid dependence; the majority (60%) were IgG+C wAIHA secondary to LPD and with clinical presentation of severe anaemia (Hb<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>33–70<span class="elsevierStyleHsp" style=""></span>g/l). Similar results are reported in the published series, with OR rates of 75% and 87% respectively.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0220" class="elsevierStylePara elsevierViewall">Rituximab monotherapy achieves an OR rate of 80–90% (CR: 45–65%) when administered as first line in patients with wAIHA. Since only 3 patients with cAIHA received treatment with rituximab, it is not possible to draw conclusions. In 3 recent studies the overall response rates of rituximab were 70.6% (CR: 26.5%),<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">19</span></a> 75<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> and 80%,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">5,6</span></a> all similar to our series. A meta-analysis carried out by Reynaud et al.,<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">21</span></a> which analyses the efficacy and safety of rituximab in 21 studies, a OR rate of 73% is found in 1st line (CR: 37%): 70% (CR: 42%) for wAIHA and 57% (CR: 21%) for cAIHA, similar to our results.</p><p id="par0225" class="elsevierStylePara elsevierViewall">The combination of corticosteroids and rituximab in 1st and 2nd line obtains a OR of 100% (CR: 50–60%), figures higher than corticosteroids in monotherapy. The study by Birgens et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a> compares the efficacy of glucocorticoids alone to the combination with rituximab in 1st line, achieving a 6 and 12 month CR rate of 63% and 75% respectively in the combined treatment compared to a 39% CR rate at 6 months and 36% at 12 months in the group treated with corticosteroids alone. The response at different follow-up intervals has not been evaluated in our study, but it is interesting to note that, in the long run, the immunomodulatory effect of rituximab could increase the responses, so it would be especially useful in the more serious cases of the serological type IgG+C.</p><p id="par0230" class="elsevierStylePara elsevierViewall">With regard to safety, rituximab was generally well tolerated, not observing immediate reactions during infusion, or during subsequent follow-up. There were no infectious complications and none of the 34 patients died during the follow-up. For all the above, treatment with rituximab in 1st line in association with glucocorticoids in cases of special clinical severity at diagnosis should be considered.</p><p id="par0235" class="elsevierStylePara elsevierViewall">The combined treatment of corticosteroids and immunosuppressants obtained OR rates of 70% in first line, most of them partial; the OR rates described with immunosuppression are 56%<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a> at 60–70<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> and they are also mostly partial.</p><p id="par0240" class="elsevierStylePara elsevierViewall">The combination of immunoglobulins with corticosteroids achieves CR rates of 70%. The small number of patients and the different administered regimens do not allow definitive conclusions to be drawn, but it does seem that they could have a role in treatment, especially in corticosteroid-refractory cases. There are published cases where immunoglobulins are used in combination with steroids, in first line, when there is no response to steroids alone, with good results.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">6,22</span></a></p><p id="par0245" class="elsevierStylePara elsevierViewall">The 3 patients who received the combination of corticosteroids with EPO, all with reticulocytopenia, and 2 with renal failure, achieved OR. Fourteen patients were treated with EPO in the study by Barcellini et al.,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> most of them severe forms under a polytherapeutic approach, in combination with other treatments, with an OR in 13 of them. The study by Arbach et al.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a> analysed the role of EPO in 2 cases of AIHA with reticulocytopenia with 1st line immunosuppressive treatment failure, achieving response in both cases. According to these authors, EPO can improve anaemia through 2 mechanisms: the stimulation of red blood cell production and the decrease in the total amount of autoantibodies present in each red blood cell.</p><p id="par0250" class="elsevierStylePara elsevierViewall">Splenectomy plays an increasingly less important role in the treatment of AIHA, being currently displaced to late treatment lines. It was only implemented after the 2nd line in our series, and despite being polytherapy patients, the responses are good (initial OR of 100% and maintained of 85%); data similar to the study by Roumier et al.,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a> in which 9 out of 60 patients were splenectomised in late treatment lines, with an initial OR of 100% and maintained in the long term at 67%.</p><p id="par0255" class="elsevierStylePara elsevierViewall">The median follow-up in our series was 28 months (1–332), with 8.6% of patients dying, all secondary AIHA, with anaemia not being the cause of death. Death rates similar to the study by Roumier et al.,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a> while higher rates are observed in other studies, between 10 and 20%.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">6,24</span></a> A possible explanation for these differences could be the increased use of corticosteroid-sparing therapies based on rituximab, immunoglobulins and EPO and the current low use of immunosuppressants, which translates into a more immunologically competent patient, able to better cope with the responsible infectious processes of the vast majority of deaths in this pathology.</p><p id="par0260" class="elsevierStylePara elsevierViewall">Our study recorded 5.8% of infections attributable to steroid treatment. The rate of infectious complications recorded in the medical literature varies from 2.8<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> to 8.4%,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> mostly pneumonia, as in our series.</p><p id="par0265" class="elsevierStylePara elsevierViewall">5.5% of ischaemic events were recorded, mostly arterial and coinciding with very low Hb levels, associating the severity of anaemia with arterial ischaemic symptoms, especially cardiac episodes. Ischaemic events accounted for 6% of cases in the study by Birgens et al.,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a> mostly arterial, without differences in the treatments received. The rate of ischaemic complications varies from 5–6% to 20% in the medical literature, collected in the study by Roumier et al.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">5</span></a></p><p id="par0270" class="elsevierStylePara elsevierViewall">This study has retrospectively analysed the cases of 93 patients diagnosed with AIHA in the last 30 years in 9 Spanish centres, providing a global vision of the disease and showing the variable management and progression of said disease. The initial management of wAIHA is still steroids, but in cases with severe anaemia, and especially in those caused by IgG+C, combined treatment with rituximab could be considered, given the higher and faster response rate.</p><p id="par0275" class="elsevierStylePara elsevierViewall">Finally, the increasingly common use of immunomodulatory drugs (nivolumab, denosumab, daratumumab etc.) for the treatment of numerous diseases, many of which can cause autoimmune mechanism-related anaemia, as well as herbal therapy products used by patients and whose mechanism of action is not well known, must be taken into account. The lowest haemoglobin values at diagnosis related to patients under 45 years of age and serological type IgG+C. Of the patients, 92% received first line treatment, 54% second line, and 27% third line. The warm AHAI were treated in first line with steroids, with overall responses of 83% and complete of 58%. Rituximab in monotherapy or in association with steroids was administered to 34 patients with overall responses close to 100% (complete responses 40–60%), relegating splenectomy to the third line. The immunosuppressive treatment was administered in patients with autoimmune diseases or in corticoid-dependent patients.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Discussion</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">We found high rates of response to steroids, with very prolonged treatments that cause side effects and corticoid dependence in a third of patients. The combination of steroids with rituximab in the first line, could be indicated in patients with low levels of haemoglobin and serological type IgG+C. El rituximab en monoterapia o asociado a esteroides se administró a 34 pacientes con respuestas globales cercanas al 100% (respuestas completas 40-60%), relegando la esplenectomía a tercera línea. El tratamiento inmunosupresor se administró en pacientes con enfermedades autoinmunes o en dependientes de corticoides.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discusión</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Encontramos altas tasas de respuesta a esteroides, con tratamientos muy prolongados que provocan efectos secundarios y corticodependencia en un tercio de los pacientes. La asociación de esteroides con rituximab en primera línea podría estar indicada en pacientes con bajos niveles de hemoglobina y tipo serológico IgG+C.
