elsevierViewall">A 42-year-old man from Mali attended the emergency department for evaluation. Recently arrived in Spain to work in the orange campaign, he reported abdominal pain, diarrhea, and skin lesions of 3–4 months’ evolution. It consists of erythematous plaques, some with superficial erosion and ulceration and others with a more cracked appearance and located on the neck and shoulders (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). They were painful especially with light exposure during his work. No other skin lesions or other systemic symptoms were noted. An analysis was performed that reported decreased levels of niacin and treatment with niacin at a dose of 500<span class="elsevierStyleHsp" style=""></span>mg/day was established until resolution of the skin lesions. Due to his return to his country of origin, the patient did not reappearance to the consultation.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Casal's collar is a clinical sign described by the Spanish doctor Gaspar Casal Julián in 1735 (and recorded in the <span class="elsevierStyleItalic">Natural and Medical History of the Principality of Asturias</span>). It represents the affectation of the cervical skin in a circumferential cervical way in pellagra, which may or may not be accompanied by other cutaneous or systemic symptoms (typically diarrhea and dementia).
