Reference values: girl, 6-year-old and born in January.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The attention deficit/hyperactivity disorder (ADHD) is the most prevalent mental health problem in childhood.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">1</span></a> The diagnosis requires both a clinical evaluation, including the presence of different symptoms, as well as the opinions of parents and teachers about the child's behaviour.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">2</span></a> The proportion of children diagnosed with ADHD has grown significantly in developed countries over the past 2 decades,<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">3–6</span></a> and an increasing number of them receive drug treatment for this condition. Despite the existence of recommendations and guidelines that attempt to homogenize treatment,<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">2,7,8</span></a> reports show wide differences between countries<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">9,10</span></a> and no clear correlation between the increase in the number of treatments and its prevalence.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">11</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In the case of Spain, a recent meta-analysis estimated an overall prevalence of ADHD of 6.8%, even with logical heterogeneity between studies with very different populations, sampling methods and diagnostic criteria.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">12</span></a> Regarding the prevalence of those treated pharmacologically, in the Basque Country, a study reported figures around 1.6% for the group between 6 and 13 years<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">13</span></a> during 2012, consistent with the figures of another study conducted in the Castilla-Leon region.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">14</span></a> The study in the Basque Country (2002–2012 data), as well as the one in Castilla-Leon (1992–2009) and a previous one which analyzed the period 1992–2001 with global data for Spain<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">15</span></a> show a fast treatment expansion in the last decade (from about 0.1 defined daily doses per 1000 people of all ages and day [DDD] in 2002 to more than 2.5 DDD in 2012 in the case of the Basque Country<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">13</span></a>), coinciding with the introduction of sustained release methylphenidate and atomoxetine.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Most countries with compulsory schooling use a cut-off date to determine when students should join their first academic year. In the case of Spain, compulsory primary education should begin with children who are 6 years old before 31st December of the corresponding calendar year. This implies that in September children with a significant relative difference in age join the same class (from those whose 6th birthday was in January to those who will be close to 7 in the first quarter of that school year, to those born in December who will not be six years of age until well into this first quarter). Several studies have shown that differences in relative age of the children sharing the same class in these schooling periods represent a maturity disadvantage for the younger ones, a handicap that can have a negative impact on school performance<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">16,17</span></a> and mental health.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">18</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Studies in the United States,<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">19,20</span></a> Canada,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">21</span></a> Iceland<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">22</span></a> and Sweden<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">23</span></a> have shown that children born in the last months of the year (younger at the time of the start of each school year) are at increased risk of being diagnosed and/or treated for ADHD than classmates born in the early months of the year. In general, and with the exception of Sweden, these studies were performed in countries with a high prevalence of treatment, 2–4 times higher than the Spanish. A study in Denmark, however, showed that there was an association between age and ADHD treatment,<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">24</span></a> although the prevalence of those treated was much lower than reported in most of the studies that showed this association, and also lower than the Spanish. In our country, and with the exception of some data published in the Basque Country,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">13</span></a> no studies have specifically examined this association.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The aim of this study is to evaluate whether there is a different prevalence of ADHD treatments in our environment depending on the relative age of the children at the time of school enrolment. As 1st January defines the start of each academic cohort, our hypothesis is that if there is an association, a gradient must be evident from low to high prevalence of children treated, as the chronological year's months progress. On the contrary, the absence of this association will mean the lack of a gradient.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Population and method</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0030" class="elsevierStylePara elsevierViewall">Population, observational, cross-sectional study of drug use in a health department, using a prevalence cut-off of children treated in November 2013.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Scope</span><p id="par0035" class="elsevierStylePara elsevierViewall">Departamento de Salud de La Plana (De la Plana Health Department), Castellón Province, Autonomous Region of Valencia. The Department of Health, the territorial and organizational health service structure dependent on the Valencian Government, has a 258-bed university hospital and various health centres and local offices spread across 9 basic health areas, providing health care to a population of 180,503 inhabitants.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Study population</span><p id="par0040" class="elsevierStylePara elsevierViewall">6–12-Year-olds’ population (the age group in which the “relative age” effect may have more impact), resident within the Department's area of influence and with pharmaceutical services covered by the Health Department. Children covered by healthcare services provided to civil servants were excluded (due to the limitations relative to identifying their prescriptions) and temporary residents (presuming they are not attending school in the Department). Population data were obtained from the Sistema de Información Poblacional (SIP) (Population Information System) of the Health Department, identifying a total of 20,237 children (10,367 boys and 9870 girls) who met the inclusion criteria.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Information sources</span><p id="par0045" class="elsevierStylePara elsevierViewall">All data were obtained from the electronic information systems of the Health Department. Date of birth and sex of children was obtained from the SIP. Prescriptions and dispensations of drugs with approved indication for the treatment of ADHD in Spain (methylphenidate and atomoxetine in any formulation) were obtained from the GAIA pharmaceutical services management system, linked to the ABUCASIS electronic medical records. The presence of a ADHD diagnosis was not investigated, assuming that all those under treatment had it (GAIA requires a diagnosis compatible with the drug to allow the issuance of a prescription) and a great heterogeneity in the diagnostic logging in untreated cases.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Study variables</span><p id="par0050" class="elsevierStylePara elsevierViewall">Birth month (January–December), age (6–12 years, in full years), sex, and treatment with methylphenidate and/or atomoxetine.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Ethical aspects</span><p id="par0055" class="elsevierStylePara elsevierViewall">The study protocol was approved by the Clinical Research Ethics Committee of the La Plana of Vila-real Hospital (Favourable opinion of 6th October 2014), and classified by the Spanish Agency of Medicines and Medical Devices as “Postmarketing Study with designs different to the prospective follow-up” (reference: TDH-MYA-2014-01). Databases built for statistical analysis did not include any information that would allow the identification of patients or their combination with other databases or information systems.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">First, the proportion of children receiving drug treatment for ADHD by age and month of treatment was estimated, and then the bivariate associations between these factors and the prevalence of treatment was analyzed. The analysis was stratified by sex and the Cochran-Armitage trend test<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">25,26</span></a> was used to assess the existence of a trend between age and prevalence of treatment, and month of birth and prevalence of treatment. Then a generalized linear multivariable model with logarithmic transformation and binomial family was used to estimate the strength of the association between the variables (age, sex, month of birth) and the probability of receiving treatment in November 2013. This model allows to estimate prevalence ratios<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">27</span></a> associated with different values of the explanatory variables with respect to the reference group (6-year-old girls born in January). Analyses were performed using STATA statistical software v. 13 (Stata Corp., College Station, TX, USA) and R (Free Software Foundation's GNU General Public License, Boston, MA, USA).</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">A total of 349 children (1.73%) were in treatment for ADHD in October 2013, with almost 4 times more prevalence among boys than girls (2.70 vs. 0.71%; OR: 3.89; 95% CI: 2.98–5.13). 90.8% of those treated (boys: 92.5%; girls: 84.1%) were receiving methylphenidate, 8.6%, atomoxetine (boys: 6.8%; girls: 15.9%), and 0.6%, all boys, received both drugs.</p><p id="par0070" class="elsevierStylePara elsevierViewall">In the bivariate analysis, the prevalence of treated (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>) increases with age from the age of 6 (0.26% treated) to 12 (2.50%), with a rapid growth in the first years and a slowdown from the age of 8 in girls and 9 in boys. The prevalence of treated also increases with the birth month, from 0.79% for those born in January, to 2.24% for those born in December (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001 in the trend test), and children born in December were almost 3 times more likely to be in treatment for ADHD than those born in January (OR: 2.81; 95% CI: 1.53–5.16). By gender, the gradient of treatment relative to the month of birth was significant in boys (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) but not in girls (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.059).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">In the multivariate analysis (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), and after adjusting for the effect of age and sex, those born from August to December show a treatment increase of between 2.5 and 3 times compared to those born in January. Between February and July, and although adjusted prevalence estimates are higher than in January, only those born in the months of March and May had a significantly higher treatment prevalence ratio (about 2 times higher) compared to those born in January.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">The results of our study show, first, that also in our environment there is an association between the relative age of the children and the prevalence of ADHD treatment, with a significant higher proportion of treated among the most immature children (born between August and December, which start school in September of their respective school year) when compared to the more mature (born in January), and with intermediate treatment prevalence – not always significant – in those born from February to July. This association remained after controlling for the effects of age and sex using a multivariable model. Second, the effect of relative age seems more pronounced in boys than in girls, in which the trend test was close – but did not reach – a conventional statistical significance, although this result could be due to a lack of statistical power for the lower prevalence of treatment in girls. A third aspect of interest is that the central estimators of the adjusted prevalence ratio do not seem to show neither a gradual increase as the months of birth advance (more immature children), nor 2 blocks configured by those born before and after July, although the wide confidence intervals do not allow to confirm this observation.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Our study is consistent with other studies that found an association between the relative immaturity of the school children and treatment for ADHD,<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">19–22</span></a> although the prevalence of those treated in the La Plana Department (1.7%) is much lower than those reported in these studies (US, Canada, Iceland), ranging between 3.7 and 6.3%. They are also consistent with the results of another study conducted in Sweden<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">23</span></a> and the recent reports -still without a statistical analysis- for the Basque Country,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">13</span></a> both with treatment prevalence figures closer to those of this study. In Denmark, however, another recent analysis found that the relative age of the children in class had no effect on the prevalence of ADHD treatment.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">13</span></a> A plausible explanation for this discrepancy is the very low proportion of children treated in that country (<1%) and greater discretion for parents on the date of incorporation of their children to school (in the Danish study, half of the boys and just over a third of girls born in the last quarter of the year delayed their entry to school to the next school year). It should be noted that in the United States, where different states use different months to determine when students join the first school year, the relative age effect is associated to the month of birth that the most immature children contribute to the said first school year.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">19,20</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Differences in relative age seem to entail a systematic inequality among children born shortly before the cut-off date for compulsory schooling and their more mature class mates, with potential negative consequences for the former, which have been evident in other areas beyond diagnosis or treatment of ADHD and, among other effects, they have been associated with worst academic results,<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">16,17,22,28</span></a> more likely to be labelled as a student with special educational needs,<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">29</span></a> less likely to be chosen in the selection process for individual or team sports<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">30,31</span></a> and also a higher proportion of mental health problems other than ADHD.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">18,32,33</span></a></p><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Study limitations</span><p id="par0095" class="elsevierStylePara elsevierViewall">Among the study's limitations, the cross-sectional design itself must be mentioned first. Although, in this case, it is not a problem to establish the timeline between cause and effect (the date of birth is obviously prior to any treatment for ADHD and, in fact, it could be seen as a natural experiment with randomization of children to each of the 12 months), information on children who started treatment but left it before October 2013 is lost, an aspect that could cause bias if dropouts were differential between children born in different months. In this regard, it should be noted that the studies published in North America and the Nordic countries use cohort designs that do not have this problem and are consistent with our results. A second limitation to consider is that children diagnosed with ADHD but without drug treatment are not included. In this regard, studies that have also evaluated this aspect show the importance of the age effect on the diagnosis. Children with health coverage linked to civil servant's health insurances (in the Valencian Community, it is estimated that about 4–5% of the population is covered by these public insurers with their own prescription system) or those who are served by the private health sector and get their prescriptions outside the National Health System were not included. Finally, the study was conducted in a single health department, and given the great variability in the diagnosis and treatment of ADHD, its results can not necessarily be extrapolated to other territories, which could have differential characteristics, both in prevalence of treatments as well as in school issues (for example, a greater or lesser use of pre-schooling before 6 years of age).</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusions and implications for clinical practice</span><p id="par0100" class="elsevierStylePara elsevierViewall">Children with a lower relative age than their classmates are more likely to be treated pharmacologically with methylphenidate and/or atomoxetine. This effect could be more pronounced in boys than in girls, an aspect of great interest, which would require confirmation studies with more statistical power. The most widely used causal argument to explain this effect is that the most immature children compared to the average age in the class face school requirements that are too high in the early years of schooling, which could contribute to the development of behaviours close to ADHD and/or that teachers, by comparing them with their older class mates, interpret the behaviours of these more immature children as typical of ADHD.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">19,20</span></a> The most important implication of this situation -besides more flexible strategies regarding compulsory schooling should probably be explored together with the role of children's schooling prior to compulsory schooling – is that clinicians should be very careful with labelling and treatment of children 6–9 years of age who were born in the last months of the calendar year to avoid ADHD overdiagnosis and the overuse of drugs with potential significant adverse La prevalencia de tratados se incrementó con la edad, en varones y en los más jóvenes del curso (nacidos en los últimos meses de cada año).
