Historically it has been described as a complication of anticoagulant treatment.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">1–3</span></a> However, there are also cases in non-anticoagulated patients (NAC) with risk factors such as neoplasms, vasculitis, coagulopathy, dialysis or rupture (spontaneous or iatrogenic) of blood vessels.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">4,5</span></a> The SRH incidence might be increasing due to increased use of anticoagulants in the treatment of atrial fibrillation, increased age or suffering from chronic diseases in general population.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">6</span></a> However, few series have analyzed this complication deeply and most of them are on anticoagulated patients (AC). We have studied the clinical profile of SRH patients, related or not with anticoagulant treatment, in our hospital for 11 years (2003–2013). In addition, we compared this series to another series published by our group<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">1</span></a> to detect changes in the SRH treatment.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0010" class="elsevierStylePara elsevierViewall">We retrospectively reviewed the medical records of patients older than 18 years treated at the Hospital Universitario Marqués de Valdecilla (Santander) from 2003 to 2013 with SRH diagnosis. We define as “spontaneous” the SRH unrelated to trauma, invasive procedure or bleeding due to rupture of an aortic aneurysm. Clinical data were obtained from the medical records. The clinical variables collected included age, sex, use of anticoagulants, bleeding risk factors and clinical manifestations. The data collected were blood determinations (blood count and coagulation studies), imaging tests (computed tomography, ultrasound and arteriography), treatment completed (fluid therapy, vitamin K, blood transfusion, ICU admission or surgery) and clinical outcome (reintroduction of anticoagulant therapy, ICU admission and mortality). We defined as “conservative” treatment the use of intravenous fluids, vitamin K supplements or blood transfusions (red cells, platelets and plasma). “SRH-related mortality” was defined as mortality caused by bleeding in the first 30 days after diagnosis. We compared this series with a series previously published by us with 23 SRHs in our hospital from 1991 to 2001.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">1</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0015" class="elsevierStylePara elsevierViewall">Results are expressed as mean (SD) and percentage. Student <span class="elsevierStyleItalic">t</span> test or the Mann–Whitney test were used for comparing quantitative variables and the Chi-square test or Fisher's exact test was used for qualitative variables. A <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 is considered as statistically significant. This analysis was performed using SPSS<span class="elsevierStyleSup">®</span> statistical software.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Current case series</span><p id="par0020" class="elsevierStylePara elsevierViewall">We identified 34 SRH patients (64% women) with a mean age of 65 years (23–88 years). Fifteen cases (44%) were AC, 13 with acenocumarol. Only 2 patients were on low molecular weight heparin (LMWH, enoxaparin). No case with the novel oral anticoagulants were found, drugs more recently introduced for the treatment of venous thromboembolism and stroke prevention in atrial fibrillation.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">7</span></a> However, we believe that these results should be interpreted with caution. The indication for anticoagulant therapy was heart disease in 81% (atrial fibrillation in 9, valve disease in 2 and cardiomyopathy in one) and venous thromboembolism in 19%. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the clinical characteristics of the AC and NAC SRH. Mean age was 13 years higher in AC patients (72 [13] vs 59 [18] years; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02). As expected, atrial fibrillation was more prevalent in AC (60 vs. 16%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.02). 62% had hypertension (73% AC and 53% NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.19), and one third, renal failure (33% AC and 28% NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.55). In half of the 19 NAC SRHs there were risk factors for bleeding: in 4, cancer, in 2 kidney transplants, 1 chronic liver disease, 1 acquired hemophilia A, 1 microscopic polyarteritis nodosa (PAN) and a woman with an ectopic pregnancy. The remaining subjects did not show clear cause of bleeding. Abdominal pain was the most frequent symptom (80% AC and 89% NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.57), followed by hypovolemic shock (55% AC and 56% NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.62). The presence of a palpable abdominal mass was more frequent in AC (33 vs 0% NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.009). Less frequent clinical manifestations were fever and pain. Two patients had a femoral neuropathy, one was AC with enoxaparin (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Mean hemoglobin at the time of diagnosis was 8.4<span class="elsevierStyleHsp" style=""></span>g/dl (5.1–14.2<span class="elsevierStyleHsp" style=""></span>g/dl), similar in both groups. There were no differences in creatinine levels (2.4 [1.0]<span class="elsevierStyleHsp" style=""></span>mg/dl AC and 1.7 [1.2]<span class="elsevierStyleHsp" style=""></span>mg/dl NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.11). Mean INR in AC was 4.1 (3.5), 40% had an INR<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>3 at admission. The diagnostic test most commonly used was computed tomography (CT) in 82%, followed by abdominal ultrasound, in 62%. Six patients underwent angiography (17%) and 6 required laparotomy (17%). The maximum diameter of the hematoma was 15 (6) cm in AC and 12 (6) cm in NAC (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.41). Most SRHs were treated conservatively (80%); only 20% required surgery. In no case drainage or intra-arterial embolization was performed. 68% received transfusions of red blood cell concentrates, 40% plasma and 5% platelets. In 60% of AC cases it was necessary to reverse anticoagulation with vitamin K and protamine. One third of patients were admitted to the ICU, similar percentage in both groups (33% AC and 31% NAC; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>ns). 66% AC cases restarted anticoagulant therapy after the acute episode (5 with acenocumarol, 2 with LMWH and one with unfractionated heparin), with an average 19 days (2–90 days), without showing further complications.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Mortality</span><p id="par0025" class="elsevierStylePara elsevierViewall">The SRH-related mortality was 21% (7 cases), similar in AC and NAC (20 and 21%, respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.64). The mean survival was 5 days (1–15 days). The average age of the deceased was similar to that of non deaths (66 [17] and 64 [17] years; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.78). However, the presence of a palpable abdominal mass was more frequent in those who died (42 vs 7%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04) as well as the need for ICU admission (71 vs 22%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.024).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Comparing both series</span><p id="par0030" class="elsevierStylePara elsevierViewall">In the first series we recorded 23 SRH patients from 1991 to 2001 (15 AC and 8 NAC). In the current series, with the same number of years (2003–2013), we found 11 more cases, i.e. 34 cases (15 AC and 19 NAC) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). This 47% increase is due entirely to NAC patients. Comparing the AC SRHs in both series (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) we found more cases with acenocumarol and fewer cases with heparin. No major changes in the clinical presentation, but the use of CT is significantly increased as a diagnostic test (from 47 to 93%). Regarding treatment, the number of patients requiring surgery decreases from 6 to 3 cases. Mortality is similar in both series (13 and 20%).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">SRH is a complication of anticoagulant therapy,<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">8–11</span></a> although cases have also been reported in NAC subjects.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">1,2</span></a> Few series have analyzed this entity deeply. We reported 34 cases and found that 56% were not anticoagulated, although they had other reasons for bleeding. SRHs have been reported after kidney transplant caused by coagulation disorders,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">12</span></a> in the PAN caused by the rupture of a renal artery aneurysm<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">13,14</span></a> or after ectopic pregnancy.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">15</span></a> However, it is noteworthy that over half of the patients in our study had no clear cause of hemorrhage. Historically, SRH symptoms and signs include abdominal pain and hypovolemic shock.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">2</span></a> Femoral neuropathy is also a SRH manifestation due to structure compression and may require surgery or percutaneous drainage.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">16</span></a> We found it in 2 patients, and both were resolved with conservative treatment. Imaging techniques, specifically CT, play an important role in diagnosis.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">17</span></a> In a recent study of 27 SRHs, CT identified 100% cases, and the source of bleeding in 40%.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">18</span></a> In our series, it was the diagnostic technique most widely used and its use was clearly increased. Moreover, there is no consensus on what is the best treatment for SRH patients because the evidence is based on small series or isolated cases. Conservative treatment is performed, if the patient is stable, with fluidotherapy, correction of coagulation and transfusions of blood products.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">19</span></a> Intra-arterial embolization and surgery may be necessary in eligible cases.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">20,21</span></a> In our series SRH-related mortality was 21%, similar to that described in other studies (5–13%).<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">2,22</span></a> Once past the acute episode, and being the patient stabilized, we faced the dilemma of whether or not to reintroduce anticoagulation. Only a few studies have analyzed this fact. In the first series we described the reintroduction of anticoagulation therapy in 4 subjects (4–25 days after diagnosis), and they showed no complication.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">1</span></a> In the current series, 66% AC SRHs restarted anticoagulant therapy without recurrence of bleeding. Based on these results we believe that this practice can be considered relatively safe. When comparing both series, increased NAC SRHs is noteworthy in relatively young people with diseases that contribute to bleeding. We consider it an interesting fact that makes us think in this entity as not only related to anticoagulant therapy.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In conclusion, the clinical profile of SRH patients is similar, whether or not anticoagulated. Reintroducing anticoagulant therapy after hematoma resolution appears to be a relatively safe practice. The number of NAC SRH cases is increasing. One-third of cases had chronic renal insufficiency. Abdominal pain was the most common symptom both in anticoagulated and non-anticoagulated patients (80% in anticoagulated and 89% in non-anticoagulated patients). About one half of the patients developed shock. A CT scan was the most commonly performed diagnostic test, followed by abdominal ultrasound. Most cases were managed conservatively (80%). More than half of the patients (66%) restarted anticoagulation therapy after the acute event with a mean delay of 19 days (range 2–90 days). None of them suffered a new bleeding episode.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Restarting the anticoagulation treatment after hematoma resolution seems to be a safe practice. Comparamos esta serie con la descrita previamente en nuestro hospital.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Identificamos 34 HRE (64% mujeres) con una media de edad de 65 años (23-88 años). El 44% estaban anticoagulados por enfermedad cardiaca (81%). Un tercio tenía insuficiencia renal, y el 15%, cáncer. El dolor abdominal se describe en el 85%. La TC se realiza en el 82%. El 32% ingresa en UCI y solo el 20% requiere cirugía. La mortalidad relacionada fue del 21% (media de supervivencia de 5 días; 1-15 días). El 66% reinicia la anticoagulación a los 19 días (2-90 días), sin complicaciones posteriores. Se incrementan los casos no anticoagulados en un 47%.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusión</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">En los casos anticoagulados, la reintroducción del tratamiento, tras la resolución del hematoma, parece una práctica relativamente segura.
