We included admitted patients over 18 years of age that presented diarrhoea, defined as the emission of 3 or more unformed stools per day for at least 2 days, and who had a positive microbiological detection of <span class="elsevierStyleItalic">C. difficile's</span> A or B toxin. Until 15th September 2014, the microbiological diagnosis was performed by direct cytotoxicity assay in stools (MRC-5 cell lines) and simultaneous toxigenic culture (cyccloserina-cefoxitin-fructose agar, performing a cytotoxicity assay if <span class="elsevierStyleItalic">C. difficile</span> growth was obtained). Identification of the isolates was performed using standard biochemical methods. From 16th September 2014, the diagnosis was made by immunochromatographic test for the simultaneous detection of glutamate dehydrogenase (GDH) antigen and A and B toxins (C. Diff Quik Chek Complete<span class="elsevierStyleSup">®</span>, Techlab Inc., Blacksburg, USA). In cases where the results were GDH positive/negative toxins or vice versa, a toxigenic culture was performed using specific multiplex PCR assay for genus and species and A and B toxin genes.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">5</span></a> Pseudomembranous colitis cases confirmed through a colonoscopy or histopathologically were also included.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Following the established criteria,<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">6</span></a> a hospital acquisition was considered when symptoms occurred after 3 days from admission. Community acquisition was considered if symptoms appeared in the community or in the first 48<span class="elsevierStyleHsp" style=""></span>h after admission and without having had a previous admission in the last 12 weeks. Clinical variables such as age, sex, admissions, Charlson comorbidity index (absence of comorbidity: 0–1 points, low comorbidity: 2 points and high ≥3 points) were collected,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">7</span></a> together with risk factors such as previous use of antibiotics, steroids or proton-pump inhibitors (PPI) as well as hospitalisation, ICU stay or surgical intervention in the 3 months prior to infection. Clinical and diagnostic tests (radiological and colonoscopy) were collected. Complications, the treatment received (type of antibiotic and duration), recurrence (defined as a new episode in the first 8 weeks after resolution of the process) and related mortality (attributable to infection in less than 30 days) were recorded.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0020" class="elsevierStylePara elsevierViewall">The distribution of quantitative variables was analysed with the Kolmogorov–Smirnov test. Data are expressed as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation (SD). Student's <span class="elsevierStyleItalic">t</span> test was used to detect differences between 2 quantitative variables, or the Mann–Whitney test, and the <span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span> or Fisher's test for 2 qualitative variables. SPSS (Statistical Package for Social Sciences, Chicago, IL, USA) was used.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0025" class="elsevierStylePara elsevierViewall">244 patients with <span class="elsevierStyleItalic">C. difficile-associated</span> diarrhoea were identified with an average age of 66 (SD 15) years (19–94 years); 53% were male. 80% was hospital-acquired in 20% community. The service with the largest number of cases was Internal Medicine (28%), followed by Digestive Diseases (23%) and Haematology (15%). The hospital stay was 26 (SD 20) days. 84% of patients had a high rate of comorbidity. 91% used antibiotics; piperacillin/tazobactam was the most used (41%), followed by ciprofloxacin (36%) and cephalosporins ([27%], cefazolin, ceftriaxone, cefuroxime, cefotaxime, ceftazidime, cefepime, cefixime and cefoxitin). Other factors associated with infection were the use of PPIs (78%), corticosteroids (19%) and chemotherapy (15%). Prior admission was recorded in half of the cases (53%), ICU admission in 32% and abdominal surgery in 14%. As for the clinical manifestations, all patients had diarrhoea (28% with pathological products), followed by abdominal pain (69%) and fever (54%). 11% developed septic shock and there were only 2 cases with toxic megacolon (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) and one with intestinal perforation. 81% of cases had at least one colitis severity criterion (albumin <3<span class="elsevierStyleHsp" style=""></span>g/dl or creatinine >1.5<span class="elsevierStyleHsp" style=""></span>mg/dl or leukocytes >15,000<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleSup">3</span>). As for diagnostic tests, an X-ray was performed in 1/3 (normal in 19% and distended loops in 15%), and an abdominal CT in 2/3 (normal in 15% and distended loops in 12%). Only 68 cases (28%) had performed a colonoscopy (28% with erythema, 26% with pseudomembranes and 26% with nonspecific colitis) and 47 of them a colon biopsy. Pseudomembranous colitis was the most frequent finding (37%), followed by a nonspecific colitis (31%). The related antibiotic was removed only in half of the cases and isolation measures were implemented in 2/3 (60%). As for treatment, 96% received antibiotics: oral metronidazole was the most used (81%), followed by oral vancomycin (17%). Oral fidaxomicin was used only in 2 cases due to a poor clinical outcome. 7% of patients required ICU admission. Related mortality was 3% with a median survival of 13 (SD 11) days. The relapse rate of the disease was 11%.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">When comparing the clinical profile of patients by the infection acquisition site, we found that hospital cases involved older patients, 67 years of age (SD 15) compared to community acquisition 63 years of age (SD 19) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01), predominantly male (57 vs. 37%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01) and high comorbidity (86 vs. 75%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01). Antibiotic use was also higher (95 vs. 76%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), namely piperacillin/tazobactam (46 vs. 21%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), cephalosporins (40 vs. 12%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and levofloxacin (24 vs. 8%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01), PPI use (87 vs. 48%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), the percentage of QT (17 vs. 6%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05) and dialysis (8 vs. 0%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.04). Clinical manifestations were similar in both groups. Interestingly, only the presence of diarrhoea with pathological products was more frequent in community cases (49 vs. 22%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The percentage of endoscopies performed was also higher (43% community and 24% hospital; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03). There were no differences in associated mortality (3 and 4% respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.72) or the recurrence rate (11 and 8% respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.52) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The cases of <span class="elsevierStyleItalic">C. difficile-associated</span> diarrhoea increased by 1.7 times during the study period, as it went from 90 cases in the first half of the period (2004 to June 2009) to 154 cases in the second half (July 2009–2014), with the total number of hospital admissions remaining stable (scheduled and emergency). Specifically, cases of hospital acquisition increased by 1.6 times (73–122) and community acquisition by 1.9 (17–32). <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows the number of cases per year.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0040" class="elsevierStylePara elsevierViewall">The epidemiology of <span class="elsevierStyleItalic">C. difficile</span> infections has changed in the last decade. While it remains the leading cause of hospital-acquired diarrhoea, it is also described in the community.<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">8–10</span></a> However, community acquisition is considered less severe than the hospital one, with less need for ICU admission<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">11</span></a> and low mortality (3%).<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">3</span></a> Community-acquired cases are younger in our series, with fewer comorbidities and risk factors, such as the use of antibiotics or PPIs, a fact already described by other authors.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">4,12</span></a> The mortality we registered was low (3%), lower than that found in our healthcare environment (12.5%),<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">13</span></a> but we thought it may be due to the fact that it refers to mortality attributable to infection in less than 30 days, and not for all causes.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Our study has limitations, since it only includes hospitalised patients, so we cannot analyse in depth the clinical profile of cases among the general population.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Moreover, we found an increase in cases over the study period, both in hospital as well as in community-acquired cases.
