id="par0005" class="elsevierStylePara elsevierViewall">Acute pancreatitis (AP) is an inflammatory disease characterised by abdominal pain associated with other symptoms with significant inter-individual heterogeneity. Its diagnosis is based on 3 criteria<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>: presence of abdominal pain, typically in the epigastrium; increased blood amylase or lipase 3 times its normal value; and characteristic findings on imaging (CT scan, MRI or ultrasound). It is a disease with a rising incidence, from 13 to 45 cases per 100,000 inhabitants/year in 2014<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> to 72 cases per 100,000 inhabitants/year<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> in 2018. Mortality is between 5% and 10% (associated with multi-organ failure, sepsis or pancreatic necrosis).<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Also, about 20% of patients with AP will have recurrent AP.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> A succession of different episodes of AP can eventually lead to chronic pancreatic dysfunction and various complications.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The most common aetiologies are biliary lithiasis and alcohol consumption, accounting for more than 60%.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Other aetiologies such as hypertriglyceridemia or hypercalcaemia are responsible for between 1% and 14% of cases.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Between 0.1% and 5% of pancreatitis is considered to be drug-induced, although it is very likely that this frequency is underestimated due to its diagnostic difficulty.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> Usually, the diagnosis of pharmacological pancreatitis is considered as a diagnosis of exclusion, once all other aetiologies have been ruled out and provided that the temporal relationship is compatible.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> The aetiological diagnosis becomes more definitive in cases where a positive re-exposure is observed.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> In cases where no aetiology is found, improvement on withdrawal of the drug will also support the pharmacological cause.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The results of a systematic review of drug-induced pancreatitis led to a classification of 4 groups based on the quality of evidence.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> More than 500 drugs were included as possible causes of AP, based on case-control studies or individual case descriptions.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The lack of conclusive data complicates the diagnosis and treatment of drug-induced AP and the identification of the causative drug, which could sometimes lead to recurrent pancreatitis. The review of patients admitted to the Hospital Universitario de Bellvitge (HUB) with a diagnosis of drug-attributed non-calculous AP provides information and improves the approach if other causes are not identified. We propose a study to describe the cases of suspected drug-associated non-calculous AP, the characteristics of these episodes and the associated drugs.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">This is a descriptive, observational and retrospective study of cases of suspected drug-related AP among patients requiring admission to the HUB between June 2007 and March 2023. The HUB is a tertiary reference hospital of the Gerencia Territorial Metropolitana Sud, under the Catalan Institute of Health, in the autonomous community of Catalonia (Spain).</p><p id="par0030" class="elsevierStylePara elsevierViewall">The source of identification and data collection of patients with suspected drug-induced AP was the HUB’s pharmacovigilance programme (PVP) database.</p><p id="par0035" class="elsevierStylePara elsevierViewall">This database collects suspected adverse drug reactions (ADRs) since 2007 from:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">•</span><p id="par0040" class="elsevierStylePara elsevierViewall">The daily list of admissions and diagnoses (coded using CIM-10) provided by the HUB Admissions Unit. A doctor, a specialist in clinical pharmacology, identifies cases with a diagnosis of pancreatitis from the list and makes an assessment using the information obtained from the patient's medical records.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">•</span><p id="par0045" class="elsevierStylePara elsevierViewall">Spontaneous reporting and therapeutic consultations of suspected ADRs by hospital healthcare professionals to the Clinical Pharmacology Department.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">•</span><p id="par0050" class="elsevierStylePara elsevierViewall">The monthly follow-up list of patients treated with hospital outpatient drugs whose medicine has been withdrawn due to unacceptable toxicity (this last source was included in March 2022). A doctor, a specialist in clinical pharmacology, uses the information in the patient's medical record to make an assessment of the cause of the withdrawal.</p></li></ul></p><p id="par0055" class="elsevierStylePara elsevierViewall">For the cases identified, the patient's medical records were reviewed from admission to discharge or death to document the results of the ancillary tests performed and the definitive diagnosis, as well as the outcome of the ADR. At case closure, a causality analysis of suspected ADRs was performed according to the methods of the Spanish pharmacovigilance system and a causality algorithm was established using the modified Karch and Lasagna algorithm (Meyboom et al., 2006). This algorithm scores 5 items (time sequence, prior knowledge, response to withdrawal, re-exposure, alternative cause) and classifies the relationship as definite, probable, possible, conditional, unlikely or unclassified.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Cases of patients who did not correspond to the hospital's reference health area, patients included in clinical trials, those without sufficient information for evaluation and patients with a diagnosis of AP with aetiologies unrelated to medication (lithiasis, neoplasms, alcoholism, hypertriglyceridemia) were excluded.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The database used the Anatomical therapeutic chemical classification dictionary for the coding and classification of active substances and the Medical dictionary for regulatory activities version 25.1 for the coding and classification of drug indications.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The following variables were collected: total number of ADRs and number of suspected pancreatitis during the study period (2007–2023); sex, age and date of birth; comorbidities such as hypertension, dyslipidaemia, diabetes mellitus, chronic kidney disease, liver disease and cardiovascular disease; date of admission, discharge and total days of admission; demand for care, source of reporting and department of admission or reporting; suspected drugs, regimen, date of drug initiation and indication; date of onset and end of ADR; total days between drug initiation and onset of ADR; withdrawal, re-exposure and outcome; causality algorithm; number of episodes of pancreatitis and whether the medical team had mentioned in the patient's medical records a possible association between pancreatitis and the drug.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Descriptive analyses were performed for the variables studied. Results were expressed as absolute and relative frequencies for qualitative variables and as mean, standard deviation, median, interquartile distance and range for quantitative variables. Comparisons were made with the following contrast tests: for quantitative variables, Student's <span class="elsevierStyleItalic">t</span>-test if parametric and Whitney Mann <span class="elsevierStyleItalic">U</span> test if non-parametric; for qualitative variables, the χ<span class="elsevierStyleSup">2</span> was used.</p><p id="par0080" class="elsevierStylePara elsevierViewall">With regard to ethical aspects, the retrospective design of the study using PVP data means that it is not necessary to obtain the informed consent of the patients included.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The information corresponding to each individual case was coded and entered into the HUB's PVP database. The database is part of the hospital's information systems and has the same guarantees of confidentiality protection as the rest of the HUB's computerised clinical records. This study has also been approved by the Research Ethics Committee for medicinal products.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The data contained in the study were pseudonymised, as neither the patient's name nor that of the person reporting the data will appear in any report, publication or communication of the study results. Before being used, the information contained in the PVP database was coded and extracted into a database specific to this study by the person responsible, who is the only link between the PVP and the specific database, so that the analysable files do not contain any personal data that would allow the identification of patients or those doing the reporting. Data may be re-identified at source where there is a real and concrete threat to the safety or health of an individual or group of individuals, a serious threat to their rights, or where it is necessary to ensure adequate health care.</p><p id="par0095" class="elsevierStylePara elsevierViewall">This study was conducted in accordance with the Declaration of Helsinki. The authors declare that they have no conflict of interest and no specific funding.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0100" class="elsevierStylePara elsevierViewall">Between 2007 and 2023, 38 patients with suspected drug-induced AP were identified. They represent 0.62% (n = 38) of the total (n = 6085) of adverse reactions collected in the same time period by the HUB’s PVP. Among the patients, 14 were female (36.8%) and the median age was 62.6 years (IQR 50.4–73.0). <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> summarises the demographic and clinical characteristics of the patients included (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">The median time between initiation of the suspected drug and adverse reaction was 160.5 days (IQR: 18 days–19.4 months), approximately 5.3 months, with significant variation between results.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The number of cases with a single suspected drug was 25 (65.8%) and those with 2 or more suspected drugs was 13 (34.2%). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> describes the differences between the 2 subgroups of cases.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">Regarding the origin of the report, 34 cases (89.4%) came from daily monitoring of admissions, 3 cases (7.9%) from spontaneous reporting by hospital staff and one case (2.6%) from outpatient drug monitoring. All were considered severe, with 37 patients (97.3%) requiring admission and one patient (2.6%) requiring emergency care. Median length of stay was 5 days (IQR: 3–7). Regarding the department responsible for the patient, 25 cases (65.8%) corresponded to General and Gastrointestinal Surgery, 9 (23.7%) to Gastroenterology, 2 (5.3%) to Intensive Care Medicine, one (2.6%) to Respiratory Medicine and one (2.6%) to the Emergency Department.</p><p id="par0120" class="elsevierStylePara elsevierViewall">With regard to the causative drug, 59 suspected drugs with 26 active substances (grouped into 18 therapeutic groups) were identified in the 38 patients. Azathioprine and atorvastatin with 9 cases (15.2%) and enalapril with 8 cases (13.6%) were the most common. HMG COA reductase inhibitors (statins) and ACE inhibitors with 13 cases (22%) and thiopurines with 10 cases (17%) were the most common therapeutic groups (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The indications for the suspected drugs were hypertension in 22 cases (37.3%), dyslipidaemia in 14 cases (23.7%), Crohn's disease in 9 cases (15.2%), diabetes mellitus in 5 cases (8,5%), depressive syndrome in 3 cases (5.1%), ulcerative colitis in 2 cases (3.9%) and pulmonary fibrosis, low back pain, rheumatoid arthritis and hypopituitarism in one case (1.7%) each. In terms of outcome, 36 cases (94.7%) recovered, while one case (2.6%) recovered with sequelae by developing chronic pancreatitis and one case was fatal (2.6%). According to the modified Karch and Lasagna algorithm, of the 38 cases evaluated, the association with the drug was considered probable in 29 cases, possible in 7 cases, conditional in 1 case and unlikely in 1 case.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">Based on the information available in the medical records, a pharmacological aetiology was included in the differential diagnosis in 23 cases (60.5%). Of these, the suspected drug was withdrawn in all cases. In one case it was withdrawn in the second episode of pancreatitis, in another in the third episode, and in 2 other patients the drug was withdrawn from primary care or outpatient clinics. In the remaining 15 cases (39.5%), no information was found in the medical records to indicate that the pharmacological aetiology was suspected as the cause of the episode or included in the differential diagnosis. In the group of subjects with only one suspected drug, the pharmacological aetiology was considered in 19 cases (76%), whereas in the group with 2 or more suspected drugs it was considered in 4 cases (30.8%).</p><p id="par0130" class="elsevierStylePara elsevierViewall">A total of 4 cases were considered to have positive drug re-exposure after resolution of pancreatitis. <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the characteristics of these cases, the drugs suspected and their course. In the first 2 cases of positive re-exposure, it was attributed to biliary sludge, microlithiasis or non-specific content, and they underwent surgery, either cholecystectomy or endoscopic retrograde cholangiopancreatography and papillotomy, with no recurrence of the episodes. In contrast, no alternative causes were found in the last two cases.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0135" class="elsevierStylePara elsevierViewall">The results of this study indicate that drug-induced APs represent 0.62% of all ADRs collected in our hospital and are more common in men. Twenty-six suspected active substances were identified. The most commonly associated drugs were HMG COA reductase inhibitors (atorvastatin), thiopurines (azathioprine) and ACE inhibitors (enalapril). Most of the drugs identified were already known as possible pharmacological causes of AP. In 60% of cases, the drugs were included in the differential diagnosis by the treating physicians. In these 23 cases, the suspected drug was withdrawn. Four cases were reported in which positive re-exposure was considered to have occurred.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Several studies have been published that have analysed how many cases of AP can be associated with the use of drugs. These studies have established the prevalence of drug-induced AP cases at 0.03% in Canada,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> 0.05% in Korea,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> 0.2% in France<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> and 0.3% in Switzerland.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> Our study design does not allow us to make the same estimate. In contrast, a study with a methodology similar to ours was conducted by Niinomi et al. in Japan, using the spontaneous adverse reaction reporting database. In that study, suspected drug-induced pancreatitis was estimated at 0.17% of the total number of ADRs collected, a figure similar to, but slightly lower than, that in our study.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">The demographic data show a median age of 62.6 years, in agreement with what has already been published, where drug-induced APs were more common in elderly patients with polypharmacy.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,18</span></a> In our study, the majority of cases corresponded to men (63.2%), in contrast to the data included in the articles reviewed, where the frequency is equal or higher in women.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,18,19</span></a> Regarding comorbidities, the prevalence of hypertension, dyslipidaemia and diabetes was similar to those described in the literature, except for the prevalence of cardiovascular disease (16%), which is lower in our study.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">The median latency time between drug initiation and the development of pancreatitis was 160.5 days (approximately 5 months), with significant variability between cases. These results contrast with those observed in other studies, where the latency was, in most patients, less than one month.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In the subgroup analysis according to the number of suspected drugs, we observed that subjects with one suspected drug had a median latency between drug initiation and ADR of 62 days, and subjects with 2 or more suspected drugs had a median latency of approximately one year. These differences may be due to the inability to differentiate between idiopathic pancreatitis in patients on chronic drug treatment and drug-induced pancreatitis, as well as in cases where more than one suspected drug is involved. In this situation, the complexity of attribution is greater, as the onset of the different suspected drugs is more heterogeneous. Median length of stay was 5 days, similar to other published studies.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,18–20</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">With regard to the suspected drugs, 17 of the 26 active substances listed are described in the review by Baladov et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> with varying levels of evidence. Only DPP-4 inhibitors (sitagliptin and vildagliptin), amlodipine, nintedanib, imidapril, torsemide, leflunomide, semaglutide and pregabalin are not included. These received marketing authorisation in Europe after 2007, so their experience of use was still limited at the time of the review by Baladov et al. The dates in brackets indicate the date of authorisation of these drugs in Spain: DPP-4 inhibitors (2007),<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,22</span></a> semaglutide (2018),<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> nintedanib (2015)<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> and leflunomide (2016).<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> For amlodipine, pregabalin and torsemide, ADRs in the form of APs are described in the SmPC, but we were only able to document them in specific cases or in the list of reviews. With regard to imidapril, ADR is described in the SmPC<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> and in various studies<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27,28</span></a> as a class effect of ACE inhibitors. Furthermore, in <span class="elsevierStyleItalic">ex-post</span> studies, all of them are described as a pharmacological cause of AP.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,13,29</span></a> The ADR in the form of AP is listed in the SmPC of all the drugs reported as suspected (n = 51; 100%). With regard to the high number of active substances collected per patient, the characteristics of our subjects, who are elderly, with comorbidities and chronic polypharmacy and with more than one possible drug involved, make the identification of a single drug difficult. In addition, a certain degree of additive effect between the different drugs under consideration should also be taken into account.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Limitations of this study include the fact that, as a pharmacovigilance programme based on spontaneous case reporting, it is not possible to be sure that all cases of AP are reported. This makes it difficult to know, for example, whether the higher association with atorvastatin or enalapril is due to a higher frequency of use of these drugs or to some other drug-specific factor. On the other hand, due to the method of identifying ADR cases, only drugs for which the association is known were identified. This could have led to an underestimation in the identification of unknown associations. Another limitation is that ancillary tests were not carried out in all patients, so we cannot know whether other causes were correctly ruled out in these cases. Finally, the cases were collected in a single centre, so the small number of cases prevents the results from being generalised to other hospitals or other settings.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Taking into account the limitations, the results of this study may help to improve knowledge of the episodes and active substances associated with drug-induced APs. Although it is a relatively rare ADR, pancreatitis is a serious condition involving hospital admission in most cases. Drugs are a known cause of AP, however, the associated risk and its aetiopathogenesis remains partially unknown to healthcare professionals caring for these patients.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a> We emphasise the need to use an algorithm for the diagnosis and assessment of suspected AP that includes drugs in the differential diagnosis.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0170" class="elsevierStylePara elsevierViewall">Pancreatitis is a relatively rare (0.65% of ADRs) but serious ADR, as all patients were hospitalised. The results presented here confirm that pharmacovigilance can be a useful tool to describe cases of drug-induced AP and to consider possible associations.</p><p id="par0175" class="elsevierStylePara elsevierViewall">These results may help healthcare professionals to consider the possibility that a drug is causing AP and, once other more common causes have been ruled out, to improve the identification and management of drug-induced pancreatitis.</p><p id="par0180" class="elsevierStylePara elsevierViewall">On the basis of the information obtained in this study and the literature consulted, we propose the following diagnostic algorithm, which includes the evaluation of the drug as a possible cause (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">CRediT authorship contribution statement</span><p id="par0185" class="elsevierStylePara elsevierViewall">All authors contributed to the conception and design of the study.
